A new and reliable classification system for fractures of the navicular and associated injuries to the midfoot.

AIMS: Fractures of the navicular can occur in isolation but, owing to the intimate anatomical and biomechanical relationships, are often associated with other injuries to the neighbouring bones and joints in the foot. As a result, they can lead to long-term morbidity and poor function. Our aim in this study was to identify patterns of injury in a new classification system of traumatic fractures of the navicular, with consideration being given to the commonly associated injuries to the midfoot. PATIENTS AND METHODS: We undertook a retrospective review of 285 consecutive patients presenting ove

Trans-medial gastrocnemius approach (Badet approach) for displaced posterior cruciate ligament tibial avulsion.

Avulsions of the retrospinal surface are rare injuries resulting from high-energy trauma. Displacement of this fracture frequently indicates a surgical treatment to restore posterior cruciate ligament function. Several approaches have been proposed in the literature, either open or arthroscopic, which can be tricky due to the fracture's proximity to the popliteal vascular-nervous elements. Badet's open approach is a medial trans-gastrocnemius approach, providing a direct access to the retro-spinal surface for osteosynthesis. In this technique, an L-shaped incision is made along precise skin li

Surgical treatment of posterior cruciate ligament lesions does not cause growth disturbances in pediatric patients.

PURPOSE: The aim of the present study was to describe epidemiology, management and outcome of pediatric and adolescent patients with posterior cruciate ligament (PCL) injuries. METHODS: Sixteen patients of less than 18 years of age with 7 PCL avulsion fractures and 9 PCL tears were included over a 10-year period. Trauma mechanism, additional injuries and treatment methods were analyzed. Follow-up examination included range of motion and ability to perform squats. Pedi-IKDC and Lysholm score were obtained and posterior shift was measured in kneeling view radiographs and compared to the contral

Thoracic spinal cord injury without radiographic abnormality in an adult patient.

BACKGROUND CONTEXT: Thoracic spinal cord injury (SCI) without fractures or discoligamentary injuries in adults is extremely uncommon. This is because of the inherent stability of the thoracic spine provided by the structure of the ribcage, and the increasing stiffness of the overall spine with rising age. Till date, the pathogenesis of this disorder in adults is still unclear. PURPOSE: To present a case of thoracic SCI without any bony or ligamentous injury of the vertebral column in an adult patient together with an overview of the literature. STUDY DESIGN: Case report. METHODS: Retrospect

Unusual presentation of sciatica in a 14-year-old girl.

The sciatic nerve can be compressed by a variety of causes, while intervertebral disc herniation is the most common cause of sciatica [Surg. Neurol. 46 (1996) 14], other documented causes include, infection, neoplasm, degenerative disease of a spine, congenital anomalies and traumatic posterior hip dislocation [BMJ 287 (1983) 157]. Sciatic neuropathy in children is uncommon. We present an unusual case of sciatic nerve compression in a 14-year-old-girl that was caused by an avulsion fracture of the ischial tuberosity. The compression was relieved by surgical excision of the avulsed ischial tube

[Iatrogenic avulsion of the greater trochanter during prosthetic replacement of the hip].

An iatrogenic avulsion of the greater trochanter occurring during prosthetic hip replacement after a fracture of the neck of the femur is a rare complication. The need for maximum internal rotation of the limb during the operation imposes serious mechanical stress on the greater trochanter in patients who are often elderly and whose bones are osteoporotic. Two case reports illustrate that a conservative management--without internal fixation--combined with early mobilisation is successful. Our view is based on the current, international opinion that conservative management with early mobilisati

Timing of orthodontic tooth movement in a case with traumatized and avulsed anterior teeth.

A traumatic injury to a patient undergoing orthodontic treatment resulted in avulsion, dislodgement, and fracture of upper anterior teeth. Deciding when to institute endodontic treatment and when to resume orthodontic treatment was based upon minimizing the changes for ankyloses and root resorption. Endodontic treatment was carried out two weeks after trauma, and orthodontic treatment continued three weeks after trauma. Radiographs taken six months, one year, and two years after orthodontic completion showed progressive root resorption to the avulsed tooth and minimum root resorption to the re

Traumatic 'mallet toe' of the hallux: a case report.

We describe a case of a traumatic avulsion fracture of the distal phalanx of the hallux. In the upper limb this fracture leads to a "mallet" deformity. Such an injury in the great toe has not been reported previously in the English orthopaedic literature to our knowledge. The injury was treated in a dorsal extension splint for eight weeks. At six months follow-up, a satisfactory clinical outcome was achieved.

Nailbed injuries with hallucal phalangeal fractures--evaluation and treatment.

Adequate assessment of crush injuries to the hallux is necessary before appropriate management can be initiated. When distal phalangeal fractures of the hallux are detected by routine radiography and subungual hematomas are present, open fractures of the hallux may be overlooked if the physician does not routinely avulse the nailplate. The authors review current modalities for the detection and evaluation of these injuries and present a protocol for treatment.

Arthroscopic tibial spine fracture fixation: Novel techniques.

Proper knee kinematics depends upon the integrity of ACL at its femoral and medial tibial spine attachments. Tibial spine fractures disrupt this complex and if untreated can lead to pain, instability and functional limitation. So proper diagnosis and early treatment of tibial spine avulsion fracture is important. Tibial spine fractures are often associated with intra-articular injuries which are more common in adults. Diagnosis and treatment of these associated injuries along with proper fixation of tibial spine fractures are essential. Surgical options for tibial spine fractures include reduc

Results after replantation of avulsed permanent teeth. I. Endodontic considerations.

Following avulsion and replantation, teeth are at risk for infection and infection related resorption (IRR). Severe discolorations of tooth crowns and cervical root fractures are common. This study presents data on endodontic related complications of avulsed teeth replanted following an extraoral endodontic treatment. Periodontal aspects will be discussed in the second part of the present publication. Twenty-eight permanent teeth in 24 patients aged seven to 17 years were replanted after avulsion. All teeth could be evaluated. In all teeth extraoral endodontic treatment by retrograde insertion

Spontaneous fracture of the lower angle of scapula. Possible role of cough.

We report a case of a fracture of the inferior angle of the scapula related to prolonged cough, without triggering trauma, sport, or occupational activity. Bone insufficiency was present in this patient. Stress fractures of the scapula are rare; the fatigue type is the most frequent, associated with sports or occupational activity. We do not find any published case report about scapula stress fracture related to cough, until rib fractures are frequent in this situation. Repetitive muscular activity of the serratus anterior and major rhomboid on the inferior angle of the scapula during cough ma

Inversion injuries of the ankle: clinical assessment and radiographic review.

Inversion injuries of the ankle are a common cause of referral and presentation to accident units. They impose a load on radiographic services. A prospective trial was carried out to determine the accuracy of clinical examination. All patients were assessed clinically then examined radiographically, the clinical assessment missed 5% of the fractures. These, however, were all minor avulsion fractures or crush fractures and Tubigrip support was sufficient. Clinical examinations is, therefore, accurate and the need for most x-ray examination is questionable. X-ray examination should be reserved f

Assessing Pediatric Pelvic Fracture Patterns: New Insights and Unique Fracture Characteristics.

BACKGROUND: Pediatric pelvic ring fractures are different in biomechanics and anatomy compared with adults. Existing classification systems are insufficient in assessing the mechanical stability of these fractures due to a variety of reasons, leading to a potential underestimation of the injury severity, resulting in suboptimal treatment with the risk of long-term dysfunctions. This study aims to address this problem by comprehensively describing a cohort of patients with pediatric pelvic fractures, identifying specific pediatric pelvic fractures and patterns. METHODS: A retrospective cohort

Rare consecutive femoral lesser trochanteric avulsion in an adolescent following sporting activities; a case report.

Avulsion of the lesser trochanter is a rare but disturbing condition, which usually occurs in males between the ages of 7-16 years with significant physical activities or in athletes. Diagnosis is more often than not challenging, but with a good history, physical examination, and imaging modality, diagnosis can be clinched, and the prognosis is good even with a conservative management approach. This case report is that of a 12-year-old male who suddenly fell while participating in sporting activities in school. As a consequence of the fall, he felt severe pain in the left groin region with an

Ankle fractures with Chaput fragment: A new classification system with insights into morphology and relation to surgical treatment.

BACKGROUND: The Chaput fragment, a bony avulsion of the anterolateral margin of the distal tibia, is a less commonly discussed fracture pattern in ankle injuries. Its significance in ankle fractures and the optimal fixation technique remains unclear due to limited literature. This study aims to describe the morphology of ankle fractures with Chaput fragment and introduce a new classification system. MATERIALS AND METHODS: We retrospectively analyzed 33 patients with ankle fractures with associated Chaput fragment treated at our institute over a 3-year period. Data on patient demographics, fra

Current Concepts in Ring Avulsion Injuries.

Ring avulsion injuries, though rare, represent a uniquely devastating subset of hand trauma, with circumferential damage to skin, tendon, bone, and neurovascular structures. Historically, these injuries were almost universally treated with amputation; however, advances in microsurgical repair have transformed management and opened the possibility of finger salvage. We review the epidemiology, classification systems, treatment options, and biomechanical studies that have prompted innovations in ring manufacturing. Over time, classification systems have shifted treatment emphasis from survival t
