The degenerative Parkinsonian "Plus" syndromes form a heterogeneous spectrum of pathologies comprising multiple system atrophy, progressive supranuclear palsy, Lewy body disease and cortico-basal degeneration. Their developmental profile is distinguished from that of Parkinson's disease by the early appearance of gait and balance disorders, isolated freezing of gait, primary progressive freezing of gait or an isolated or "pure" akinesia. The origin of these symptoms however remains poorly understood. The association of nigrostriatal dopamine neuron loss with either cortical lesions, in the cas…

In this paper, we present a system design about a gait training device for monoplegia patients and aged people. In a conventional gait training program, the monoplegia patient takes a step leaning on the parallel guide bar or helped by a therapist's arm support. But this method often does not give satisfactory result since the method fails to correct the irregular posture of the patient. The new training device has been in need for gait disorder patients, monoplegia patients and aged people. The gait training device for the patients is divided into four main units, gait phase detecting unit, l…

We investigated variations in gait between women at high and at low conception probability, and how men rated those variations. Women participated in a motion capture study where we recorded the kinematics of their walking patterns. Women who were not using hormonal contraception (n = 19) repeated the study during the late follicular stage and the luteal stage of their menstrual cycle. Using a discriminant function analysis, we found significant differences in walking behavior between naturally cycling women at their follicular and luteal phases, with 71% of the walks classified correctly. How…

In humans, inability to stand and walk is the most limiting of motor disabilities. In humans, upright stance and gait is the most sensitive indicator of cerebellar disease. From animal and human studies, much has been learned about how the cerebellum coordinates normal movement, and how it may play roles in normal motor adaptation and learning. Much of this work suggests that different parts of the cerebellum control stance and gait in different ways, and differently located lesions cause different deficits. What is not known is whether the cerebellum can compensate for stance and gait disorde…

BACKGROUND/OBJECTIVE: Improving ambulation in patients with spinal cord injuries (SCIs) is a major challenge for the clinician. Physical examination can be supplemented with biomechanic information from the gait laboratory to comprehensively evaluate and improve walking. Gait analysis is an effective method to evaluate 3-dimensional dynamic joint range of motion (kinematics) and forces (kinetics) occurring at the hip, knee, and ankle/foot. Clinical gait analysis has proved to be essential for surgical and rehabilitative planning and posttreatment assessment in many neuromuscular disorders. Ben…

OBJECTIVE: A retrospective analysis was performed to assess gait in individuals with a long history of posterior knee instability.
DESIGN: Descriptive study.
BACKGROUND: There are few studies in the literature concerning evaluation of the biomechanics of the knee in patients with knee posterior instability.
METHODS: Nine individuals with posterior knee instability and a matched control group of uninjured subjects were tested in regards to knee kinematics and kinetics while walking and ascending and descending stairs. The mean follow up time for the individuals with posterior instability was…

Sequential changes in gait patterns were investigated retrospectively by analyzing the videotape recordings of 20 children with spastic diplegia and periventricular leukomalacia (PVL). The subjects began to walk at 1-5 years of age, and their walking was examined to 6-16 years of age. Many of the ambulatory children with spastic diplegia walked with flexed hips, knees and ankles. At the final walking examination, the knee was flexed during the stance phase in both legs and in all cases. In addition, the ankle was over-plantarflexed during the stance phase in nine cases or 14 legs, and dorsifle…

We report a 67-year-old man with progressive disturbance of gait. He was well until the spring of 1993 (62 years of the age), when he noted an onset of unsteady gait. He also noted that he started to have a difficulty in playing tennis, in which he became unable to hit the ball with his racket. He also noted parkinsonian features such as bradykinesia and loss of hand dexterity. He was treated with levodopa, which did not improve his symptoms. His MRI revealed marked atrophy of the cerebellum and the pons. The criss-cross high signal lesion was seen in the center of the pons. The third ventricl…

We report a 77-year-old Japanese man with progressive gait disturbance. He was well until his 71 years of the age (1992), when he noted an onset of disturbance in his speech, which was followed by difficulty in using his left hand. He did not attempt to use his left hand afterwards. He started to fall down in the spring of 1994. He was admitted to our service on October 6, 1994. Neurologic examination revealed an alert and oriented man. He showed limb-kinetic apraxia in his left hand with anosognosia for his apraxia. Vertical gaze was impaired. He walked in small steps. He had moderate axial a…

Shock absorption becomes very important in damaged joints with destroyed cartilage and progressive muscular imbalance as occurs in hemarthropathy. The effects of silicone heel cushioning on the ankle motion of hemophilic patients in different stages of hemarthropathy of the ankle joints was measured using an ultrasound motion analysis system. It is concluded that silicone heel cushioning has no influence on ankles in the late stage of hemarthropathy. Silicone heel cushioning will lead to uncontrolled changes of the ankle joint in the early hemarthropathic ankle, involving the tibiotalar and th

Clinical and step evaluations by a piezoelectric system board were performed in 54 patients who underwent microsurgical reconstruction of the thumb by great or second toe transfer. Forty-four patients were male and 10 were female. In 13 cases, the thumb was reconstructed by the Morrison wrap-around technique. In 27 cases, an extended variant of the Morrison technique was used in which the whole distal phalanx was harvested with skin and nail apparatus. Four patients were treated by great toe transfer and 10 were treated by second toe transfer. Follow-up ranged from 2 to 144 months. The group o

The syndrome of gait ignition failure is described in six patients in whom difficulty initiating walking was the major symptom. The gait had elements of parkinsonism with start and turn hesitation, shuffling, and freezing. Unlike parkinsonism, however, the gait was relatively normal once entrained; the posture was upright, and good arm swing, a normal stride length, and no festination were seen. Equilibrium was normal or near normal, and when seated or lying, rhythmic leg movements were generated normally. Facial expression, upper limb mobility, and whole body movements were well-preserved. Th…

Hemiparetic gait is characterized by slow speed and poorly coordinated movements. Because the values of gait parameters vary with changes in speed, the slow speed that is typical of hemiparetic gait necessitates applying controls for the influence of speed when comparing hemiparetic and able-bodied persons. Gait kinetics and kinematics were measured in seven hemiparetic and seven able-bodied adults to compare their gait patterns at similar speeds and to assess the effectiveness of ankle-foot orthoses which were double-stopped in 5 degrees of dorsiflexion or 5 degrees of plantarflexion. Hemipar…
