Ectopic pregnancy results in an urgent, sometimes lifethreatening condition and always leads to pregnancy loss. We conducted a literature search on mental health after ectopic pregnancy. The level of anxiety, depression, posttraumatic stress and grief was high, comparable to the level after miscarriage. Several interventions such as good communication, information or follow-up counselling sessions improved mental health. We recommend that clinicians focus on these aspects during admission and follow-up at the general practitioners.

Sixty-seven women were interviewed four weeks after spontaneous abortion. As determined by the Present State Examination, 32 of these women were psychiatric cases. This rate is four times higher than in the general population of women. In each case the diagnosis was depressive disorder, a finding confirmed by scores on three depression rating scales. Many women showed typical features of grief. Depressive symptoms were significantly associated with a history of previous spontaneous abortion, and less so with childlessness.

The current point and lifetime prevalence rates of affective disorders, based on the application of Research Diagnostic Criteria to a US urban community sample, are reported. The affective disorders studied included major and minor depression, mania, hypomania, bipolar I and II, primary and secondary depression, schizo-affective disorder, depressive and cyclothymic personality, and grief reactions. Epidemiologic surveys that include treated and untreated persons to obtain rates of specific psychiatric disorders are needed for scientific purposes and health care planning.

This intervention created by Brian Schreck at Cincinnati Children's Hospital Medical Center involves audio recording the patient's heartbeat or other respiratory sounds with a digital stethoscope to construct and preserve the patient's legacy, as well as to act as a therapeutic tool.

Although suicide is a leading cause of death for children and adolescents, there is a dearth of literature on clinician responses to suicides in that age group. This article identifies potential barriers that nurses and other health care professionals face when approaching families to offer the option of organ and tissue donation. Issues that are examined include personal feelings regarding dying, death, and organ donation; unclear roles for nurses and other health care personnel; nonsupportive institutions or physicians; difficulty in explaining brain death to families; fear of intruding during grief; lack of knowledge; and the need for support. Strategies are suggested to help nurses and other professionals to overcome these obstacles.

Just over half of the sixty women who were interviewed ten and twenty years after they were delivered of a stillborn malformed baby, seem to have resolved their grief completely and have accepted the stillbirth. The remainder have become clinically depressed or have shown physical symptoms typical of anxiety states. Factors contributing to their unresolved mourning were: 1) failure on the doctor's part to explain, comprehensively, the reason for the stillbirth; 2) lack of emotional support during and after the stillbirth.

Basis knowledge of genetics is necessary for the pediatrician, but he also must be experienced in talking with grieving parents. Grief reactions should be facilitated and by this help to the parents the best basis for future genetic counselling is established. The clinical diagnosis is a prerequisite for genetic counselling. Parents seeking help must be prepared to find their own decision. If the counsellor makes typical mistakes he provokes more disappointment than relief.
