Factors influencing professional home care utilization among the elderly.

Most explanatory research into the utilization of home care for the elderly has been carried out in a cross-sectional design with multiple regression as the main method of analysis. For methodological reasons we chose another design in a project called 'Professional home care and informal help for the elderly', which has been conducted in the northern part of the Netherlands. Two types of causal variables were distinguished to detect influencing factors on professional home care utilization among the elderly, other than physical limitation: person-bound variables and social network variables.

Low phase angle: A predictor of functional status and discharge disposition in acute stroke older patients.

BACKGROUND & AIMS: This study aimed to investigate the effects of low phase angle (PhA) on functional status and discharge disposition during the acute phase in older patients with acute stroke. METHODS: We included consecutive patients who experienced acute stroke between October 2021 and December 2022. The exclusion criteria included: age<65 years, admission from other than home, death during hospitalization, inability to measure bioelectrical impedance analysis owing to implantation, and missing data. We defined low PhA (<5.28° for male and <4.62° for female) and categorized them into the

Impact of home confinement due to the COVID-19 outbreak on vitamin D levels and trends among children with pneumonia aged 1-35 months.

Vitamin D plays a vital role in immunity and is related to susceptibility and the severity of pneumonia. The home confinement caused by the novel coronavirus disease (COVID-19) decreased sunlight exposure derived from outdoor activities in children, thereby possibly exerting an influence on 25-hydroxyvitamin D [25(OH)D] levels. The aim of this study is to quantify vitamin D level changes and trends among infants and toddlers with community-acquired pneumonia (CAP) during and post the home confinement period. This study included children who were hospitalized in the respiratory department of th

To what extent can we trust home blood pressure measurement? A randomized, controlled trial.

Increasingly, patients measure and record their home blood pressure. However, the accuracy with which they report their readings to their physicians is largely unknown. The authors assessed the accuracy and quality of self-reported home blood pressure values in an ambulatory managed care population. Forty-eight hypertensive outpatients were randomly allocated to either receive information about the storage capabilities of a home blood pressure measuring device or not to receive such information. All patients were asked to record the measurement results in a logbook twice daily over a 7-day per

White coat hypertension: a follow-up.

Home blood pressure readings by self-monitoring (14 readings in 7 days) have been compared to readings taken in the clinic in 937 participants of the Tecumseh Blood Pressure Study. In the absence of firm criteria "hypertension at home" was defined as having home readings in the upper decile of the whole population. If a clinic reading exceeded 140 and/or 90 mmHg a subject was categorized as having clinic "hypertension". Two hypertensive groups emerged; one with both clinic and home hypertension ("sustained" N = 47) and one with high clinic but normal home blood pressure ("white coat" N = 50).

Effects of home disease management strategies based on the dyadic illness management theory on elderly patients with chronic heart failure and informal caregivers' physical and psychological outcomes: protocol of a randomized controlled trial.

BACKGROUND: Currently, most elderly chronic heart failure (CHF) patients go home for rehabilitation after certain treatment in hospitals. However, the results of their rehabilitation at home are not satisfactory. According to studies, dyadic treatments can increase the efficiency of home rehabilitation, enhance both partners' quality of life, lessen the caregiver's load of care, and alleviate the strain of medical resources. Thus, the aim of our research is to design a study protocol that included elderly CHF patients and their informal caregivers as an intervention unit and to explore the imp

Cardiovascular Health Practices Among Black Patients in an Urban Underserved Clinic.

BACKGROUND: An understanding of cardiovascular health practices among black patients can provide more comprehensive patient-centered care and reduce health disparities. We studied home blood pressure monitoring in our underserved patient population to determine feasibility of providing automatic blood pressure monitors to our hypertensive patients. We believed that if we provided blood pressure monitors for home use, our patients would increase home blood pressure assessments, frequency of blood pressure assessments, and provide more data on managing their hypertension. METHODS: Forty patient

How personal and professional characteristics shape the link between work engagement and work-home interactions during the COVID-19 transition: Evidence from Ecuador.

Understanding how work-home interactions relate to work engagement is essential in organizational research. This study examined associations between bidirectional positive and negative work-home interactions and work engagement and tested whether these relationships are moderated by gender or differ across work modalities (in-person, hybrid, and remote) during the COVID-19 transition. A non-experimental, cross-sectional design was employed in Ecuador, utilizing snowball sampling to recruit 541 working individuals. We estimated the model using Partial Least Squares Structural Equation Modeling

Feasibility study of home-based rehabilitation training for language disorders in infants aged 0-1 year.

OBJECTIVE: Delayed language development in infants under one year of age can lead to persistent developmental consequences. The language of infants under one year primarily develops through face-to-face communication with their parents, such as spontaneous smiling and eye contact with their mother, which fosters language communication and promotes development. This motivates us to explore home-based alternatives. This study aims to assess the feasibility and preliminary effects of a home-based language rehabilitation program for infants aged 0-1, implemented by caregivers. METHODS: A retrospe

Can a mobile app technology reduce emergency department visits and readmissions after lung resection? A prospective cohort study.

BACKGROUND: Emergency department (ED) visits and readmissions after thoracic surgery are a major health care problem. We hypothesized that the addition of a novel post-discharge mobile app specific to thoracic surgery to an existing home care program would reduce ED visits and readmissions compared to a home care program alone. METHODS: We conducted a prospective cohort study of patients undergoing major lung resection for malignant disease between November 2016 and May 2018. Patients received either home care alone (control group) or home care plus a patient-input mobile app (intervention gr

At-Home BCI Rehabilitation Therapy for Chronic Upper Extremity Deficit After Stroke (BCI-REHAB): A Randomized Trial.

BACKGROUND: At-home stroke rehabilitation methods have strong potential to supplement limited clinical resources, but realizing that potential for survivors in the chronic phase of recovery requires developing and validating more effective options than the home exercise programs typically used today. BCI-REHAB (At-Home BCI Rehabilitation Therapy for Chronic Upper Extremity Deficit After Stroke) compared an at-home brain-computer interface (BCI) therapy system (IpsiHand System) versus an at-home exercise program for improving upper extremity function in patients with chronic hemiparetic stroke.

Support for age related firearm policies among Black American firearm owners: examining the role of children in the home.

BACKGROUND: In recent years, ownership of firearms has increased to approximately one-third of American households. Firearm access is a threat to child safety, with the most common location of fatal childhood firearm injury being at home. Exploring differences in opinions on gun violence prevention policies among Black firearm owners could inform community-supported interventions as one approach to reduce violent injuries among disproportionately affected youth. This study examined support for child-safety-related firearm policies among Black gun owners and whether support differed between tho
