There are many reasons why transgender and gender diverse (TGD) patients may opt for genital gender affirming surgery. As the TGD population continues to grow, there is ongoing demand for competent providers who can not only perform these surgeries, but also provide long-term care for patients after surgery. As such, this article hopes to provide gynecologists and other healthcare providers with an overview of feminizing genital surgeries and a framework for delivery of comprehensive ongoing care, screening, and support.

A 56-year-old woman, previously treated for breast cancer, presented to gynecologists with a lump of the left labium major of the vulva. The FDG pattern resembled as a cIV stage vulvar cancer, whereas biopsy indicated metastases from breast cancer. Metastatic disease to the vulva is particularly uncommon, representing 5% to 8% of all vulvar cancer lesions. However, gynecological uptakes, including vulvar site, have to be kept in mind as a possible site of metastatic lesions from breast cancer.

Modern methods of diagnosis and treatment allow for better survival outcomes and, more importantly, for higher curability of cancer. Female cancer survivors often need effective advice concerning the choice of birth control methods. The majority of gynecologists are reluctant to propose anything other than barrier methods due to lack of information concerning safe use of more effective contraceptives. The aim of the paper was to summarize indications and contraindications to different methods of contraception available to cancer survivors in Poland.

BACKGROUND: Mesenteric cysts are rare intraabdominal lesions of childhood that may vary in presentation from an asymptomatic mass to an acute abdomen.
CASE: We encountered a rare case of peritonitis caused by a ruptured, infected mesenteric cyst of the mesocolon in a female child that had been initially interpreted as a ruptured ovarian cyst.
CONCLUSION: When confronted with a large unilocular or septated echolucent mass in the lower abdomen in female children, gynecologists should consider a mesenteric cyst in the differential diagnosis.

One-stage combined surgery in a female patient with total extrophy of the urinary bladder after uretherosigmostomy was performed for the first time with participation of surgeons, gynecologists and urologists. Corrective surgeries in these patients must be performed for the purpose of creation of isolated urinary reservoir for prophylaxis of pyelonephritis and chronic renal insufficiency. Correction of symphysis defect was carried out with polypropylene endograft. This surgery was performed to provide quality of life and reproductive function of the patient.

Surgery for sex reassignment is an established procedure in both female-to-male and male-to-female transsexuals. While good cosmetic and functional results can be achieved in most patients of the former group in 2 sessions, in the latter the best possible results necessitate numerous interventions and scarring is inevitable. Our own preferred surgical techniques are described in detail and illustrated. Such complex interventions should be done only in specialized centres in cooperation between urologists, gynecologists and plastic surgeons.

Postmenopausal osteoporosis is caused by an increased bone loss after menopause. Both estrogen and bisphosphonate have a potent anti-resorptive effect on bone and are proved to be useful for treatment of osteoporosis. Treatment regimen for osteoporosis should be determined after considering presence of estrogen-deficiency related symptoms such as climacteric symptoms, hyperlipidemia and so on. From the view point of gynecologist, HRT should be chosen first for prevention and treatment of postmenopausal osteoporosis.

A total of 156 adolescent and young girls, delivered at term with birth weights below 3000 g were examined. In puberty, they showed a considerable incidence of skeletal disproportions, a retardation in the increment of pelvic dimensions, disturbed menarche, disrupted sequence and degree of development of secondary sexual characters. Continuity of medical surveillance of children, delivered with low birth weights, by a pediatrician and a pediatric gynecologist is essential in order to prevent reproductive disorders.

The diagnosis, pathophysiology, and treatment of adult respiratory distress syndrome (ARDS) are discussed and 2 cases are presented. ARDS has varied etiology but common clinical, roentgenographic, and pathologic findings; it has been increasingly recognized as an important cause of morbidity and mortality in seriously ill patients. The obstetrician-gynecologist must be aware of this condition, for survival depends upon prompt diagnosis and meticulous therapy of the condition and its underlying cause.

A case report of a young female who had a biopsy of a cervical lesion suspicious for malignancy is presented. After further evaluation, an initial diagnosis of Stage III sarcoma of the cervix was considered. The patient was eventually appropriately treated for a syphilitic chancre of the cervix. The pitfalls of diagnosis in this patient and other patients in the literature who have leutic cervicitis are stressed in order to alert the practicing gynecologist to this unusual location of a common veneral disease.

INTRODUCTION: OpenAI's GPT-4 (artificial intelligence [AI]) is being studied for its use as a medical decision support tool. This research examines its accuracy in refining referrals for fetal echocardiography (FE) to improve early detection and outcomes related to congenital heart defects (CHDs).

In May 2022, the World Health Organization reported an emerging global outbreak of monkeypox virus infection. Clinical manifestations of monkeypox allow us to quickly suspect the disease. Until now, no pregnant women infected with this virus have been reported; however, because of its speed of spread worldwide, it is possible that we will soon observe such cases. Thus, it is necessary for obstetrician-gynecologists to know the disease, its clinical manifestations, and the experiences reported in the few previous cases in pregnant women.

Primary Langerhans cell histiocytosis (LCH) of the vulva is rare. Fifteen cases of primary cutaneous vulvar LCH have been reported in English literature. We report an additional case of LCH confined to the vulva. In this article, we describe the clinical presentation, histopathology and immunohistochemistry findings of vulvar LCH that are helpful to both gynecologists and pathologists in the diagnosis of this entity. We briefly discuss the pathogenesis of LCH. The debate whether LCH is a reactive or neoplastic entity is still ongoing.
