Sudden onset of nephrotic syndrome in an asymptomatic Fabry patient: a case report.

BACKGROUND: Fabry disease (FD) is an X-linked lysosomal storage disorder caused by the mutation of the GLA gene, encoding the α-galactosidase, which is responsible for the catabolism of neutral glycosphingolipids. Microalbuminuria or low-grade proteinuria, and continuously progressive renal failure are common manifestations in FD males. However, sudden onset of nephrotic syndrome in FD, is rarely reported. CASE REPORT: A 32-year-old Chinese man was admitted to our hospital because of sudden onset of generalized edema due to nephrotic syndrome. He denied hypohidrosis, nocturia, and any history

Novel Arthrogryposis Multiplex Congenita Presentation in a Newborn With Pierpont Syndrome.

Pierpont syndrome is a rare and recently described multiple congenital anomaly syndrome, classically characterized by global developmental delay, distinctive facial dysmorphic features, and abnormal fat distribution in distal limbs. Only few cases were previously documented. We report a case of a term male neonate admitted to the neonatal intensive care unit because of feeding difficulties. Intrauterine growth restriction, microcephaly, and bilateral equinovarus foot were diagnosed in the second trimester, and prenatal array comparative genomic hybridization showed no abnormality. Physical exa

[Pulmonary adenocarcinoma and Bazex syndrome (paraneoplastic acrokeratosis)].

Bazex syndrome, or paraneoplastic acrokeratosis, is a rare psoriasis-like paraneoplastic skin disease, characterized by erythema and scaling, which is accompanied by hyperkeratotic lesions. The tumors most frequently associated with Bazex syndrome are squamous cell carcinomas of the upper respiratory and digestive tracts, whereas lung cancers, particularly adenocarcinomas, are rarely associated. We present a case in which both pulmonary adenocarcinoma and Bazex syndrome were present.

FGFR1 mutations cause Hartsfield syndrome, the unique association of holoprosencephaly and ectrodactyly.

BACKGROUND: Harstfield syndrome is the rare and unique association of holoprosencephaly (HPE) and ectrodactyly, with or without cleft lip and palate, and variable additional features. All the reported cases occurred sporadically. Although several causal genes of HPE and ectrodactyly have been identified, the genetic cause of Hartsfield syndrome remains unknown. We hypothesised that a single key developmental gene may underlie the co-occurrence of HPE and ectrodactyly. METHODS: We used whole exome sequencing in four isolated cases including one case-parents trio, and direct Sanger sequencing o

A 54 Mb 11qter duplication and 0.9 Mb 1q44 deletion in a child with laryngomalacia and agenesis of corpus callosum.

BACKGROUND: Partial Trisomy 11q syndrome (or Duplication 11q) has defined clinical features and is documented as a rare syndrome by National Organization of Rare Disorders (NORD). Deletion 1q44 (or Monosomy 1q44) is a well-defined syndrome, but there is controversy about the genes lying in 1q44 region, responsible for agenesis of the corpus callosum. We report a female child with the rare Partial Trisomy 11q syndrome and Deletion 1q44 syndrome. The genomic imbalance in the proband was used for molecular characterization of the critical genes in 1q44 region for agenesis of corpus callosum. Some

Ulnar polydactyly.

A retrospective review of 148 patients with ulnar polydactyly was conducted to analyze the types, patterns of involvement, associated anomalies, treatments, and outcomes of this malformation. The hands only were involved in 123 patients, both hands and feet in 20 patients, and five patients had mixed radial and ulnar polydactyly. Ulnar polydactyly was more prevalent among males. Among African Americans, the condition was often bilateral. When unilateral, ulnar polydactyly occurred more often on the left side. The racial distribution was 103 African Americans (70 percent), 37 Caucasians (25 per

Absent meibomian glands and cone dystrophy in ADULT syndrome: identification by whole exome sequencing of pathogenic variants in two causal genes TP63 and CNGB3.

BACKGROUND: Ectrodactyly is a rare congenital limb malformation characterized by a deep median cleft of the hand and/or foot due to the absence of central rays. It could be isolated or depicts a part of diverse syndromic forms. Heterozygous pathogenic variants in the TP63 gene are responsible for at least four rare syndromic human disorders associated with ectrodactyly. Among them, ADULT (Acro-Dermato-Ungual-Lacrimal-Tooth) syndrome is characterized by ectodermal dysplasia, excessive freckling, nail dysplasia, and lacrimal duct obstruction, in addition to ectrodactyly and/or syndactyly. Split Hand-Foot and Deafness in a Patient with 7q21.13-q21.3 Deletion Not Including the DLX5/6 Genes.

Split Hand-Foot Malformation (SHFM) is a congenital limb defect characterized by a median cleft of the hands and/or feet due to the absence/hypoplasia of the central rays. It may occur as part of a syndromic condition or as an isolated malformation. The most common of the six genetic loci identified for this condition is correlated to SHFM1 and maps in the 7q21q22 region. SHFM1 is characterized by autosomal dominant transmission, incomplete penetrance and variable expressivity. Associated features often include hearing loss, intellectual disability/developmental delay and craniofacial abnormal

Sorafenib induces delayed-onset cutaneous hypersensitivity: a case series.

Sorafenib is an oral multikinase inhibitor with clinical activity against hepatocellular carcinoma (HCC) and renal cell carcinoma. Administration of sorafenib carries a variety of adverse cutaneous reactions. Common adverse effects induced by sorafenib include hand-foot skin reactions, facial erythema, splinter subungual hemorrhage, and alopecia. Although erythema multiforme (EM) related to sorafenib has been reported, delayed-type cutaneous hypersensitivity reactions are rare in patients treated with sorafenib and there has been no case of Stevens-Johnson syndrome (SJS) reported so far. We re

[Prevalence of congenital split hand/split foot malformation in Chinese population].

OBJECTIVE: To study the epidemiological and clinical features of congenital split hand/split foot malformation (SHFM) in Chinese population. METHODS: Data used in this study were provided by the Chinese Birth Defects Monitoring Network. The SHFM cases were categorized into two groups: isolated (SHFM only and SHFM with other limb defects) and syndromic (SHFM with non-limb defects). Prevalence rates were calculated by residential area (urban versus rural) and by gender. Further analyses were conducted to identify the characteristics of SHFM with related to gestational age, birth weight, perinat

[Diagnosis and treatment of tethered spinal cord syndrome--based on experience of 77 cases].

Since 1976, the author has experienced 77 cases that have been diagnosed as the tethered spinal cord syndrome. These cases could be divided into two groups by the relative level of conus medullaris to that of L3 body, i.e. 34 cases in which the conus medullaris is tethered below the superior border of L3 body were classified into low placed conus medullaris (LPCM) and 43 in which the conus medullaris is located above the same level into the tight filum terminale (TFT). As to symptoms and signs, incontinence, gait disturbance, foot and toe deformities, lumbosacral tumor and sacral dimple were s

Exploring Upper Limb Malformation Associated With Cornelia De Lange Syndrome: A Clinical Case Report.

INTRODUCTION: Upper extremity deformation may come in isolation, such as polydactyly, or be associated with a syndrome involving other body parts. Cornelia de Lange syndrome (CDLS) is a syndrome encompassing many abnormalities with some variation and a spectrum of severity. This case report presents a case of a baby with suspected CDLS. CASE PRESENTATION: A 28-year-old woman, Gravida 3 Para 2, came to the polyclinic referred because of her small gestational age and a possibility of congenital abnormality. The ultrasound examination showed upper extremity malformation with hypoplasia of the le

Clinical description of a patient carrying the smallest reported deletion involving 10p14 region.

Haploinsufficiency of a region located distal to 10p14 designated HDR1, is responsible for hypoparathyroidism, sensorineural deafness, and renal anomalies (HDR syndrome). Haploinsufficiency of a more proximal region, located on 10p13-10p14, designated as DGCR2 is associated with congenital heart defects and thymus hypoplasia/aplasia or T cell defect. We describe a patient showing facial dysmorphisms, delayed psychomotor development and bilateral sensorineural hearing loss and carrying a 10p14 deletion, the smallest deletion found in the literature so far. Our patient, carrying a partial deleti

Plasticity in human somatosensory thalamus as a result of deafferentation.

Experimental studies indicate that deafferentation results in reorganization of the somatosensory map at various levels of the CNS, such that the representation of a body part adjacent to a region that is denervated expands into the deafferented area. Recent data suggest that in the human this occurs at the cortical level, but subcortical structures have not been systematically investigated. To test the hypothesis that the human thalamus is capable of significant reorganization as a result of changes in afferent input, microelectrode recording and stimulating techniques were used to define tha
