Sleep in heart failure.

Sleep plays a large role in patients with heart failure. In normal subjects, sleep is usually in a supine position with reduced sympathetic drive, elevated vagal tone and as such a relatively lower cardiac output and minute ventilation, allowing for recuperation. Patients with heart failure may not experience the same degree of autonomic activity change and the supine position may place a large strain on the pulmonary system. More than half of all heart failure patients have one of two types of sleep apnea: either obstructive or central sleep apnea. Some patients have both types. Obstructive s…

Hypoplastic left heart syndrome.

Hypoplastic left heart syndrome is a rare congenital heart defect in which the left side of the heart is underdeveloped. Surgical management of hypoplastic left heart syndrome has changed the prognosis of the condition that was previously regarded as fatal. We discuss surgical strategies based on staged procedures, with the right ventricle supporting both systemic and pulmonary circulation. We also discuss other management options, such as neonatal transplantation and the recent innovation of hybrid techniques. Surgical techniques and the understanding of the pathophysiology of this condition …

Assisting the failing heart.

Anesthesiologists increasingly encounter patients who have a spectrum of heart failure ranging from stable chronic heart failure to acute heart failure to cardiogenic shock. Improved medical therapy has increased the survival of patients who have chronic heart failure but not of patients who have acute heart failure. New surgical techniques and mechanical devices may offer alternatives to certain patients who have refractory heart failure This article provides an overview of established and newer pharmacologic and nonpharmacologic therapies and surgical interventions to manage patients who hav…

Tachycardia-induced heart failure.

Heart failure associated with tachyarrhythmias can very often be reversed by dealing with the underlying tachyarrhythmia. Typically characterized by left ventricular dilation and subsequent systolic dysfunction, this disorder can be caused by both atrial and ventricular arrhythmias, most commonly chronic atrial fibrillation. Whereas for most cardiomyopathies there is little that can be done to reverse the progression of the disease, in tachycardia-induced heart failure the patient's often debilitating symptoms can be ameliorated. This is particularly important in the primary care setting becau…

[Heart failure. Neurohormonal treatment].

Two hypothesis on the cause of heart failure (HF) have been postulated, one of hemodynamic character and another one of neurohormonal nature. According to the second one, HF is consequence of the overactivity of endogenous endocrine systems such as the renin-angiotensin aldosterone or the autonomic system. Angiotensin-II blockage is actually the first option in the medical treatment of HF. Since it reduces cardiac and vascular remodelation, synthesis of collagen and improves the endothelial function. Preceding to this option one can find the use of beta adrenergic blochers. Recently it was dis…

Computerized heart sounds analysis.

This paper is concerned with a synthesis study of the fast Fourier transform (FFT), the short-time Fourier transform (STFT), the Wigner distribution (WD) and the wavelet transform (WT) in analysing the phonocardiogram signal (PCG). It is shown that these transforms provide enough features of the PCG signals that will help clinics to obtain qualitative and quantitative measurements of the time-frequency (TF) PCG signal characteristics and consequently aid diagnosis. Similarly, it is shown that the frequency content of such a signal can be determined by the FFT without difficulties. The studied

Heart disease and aging.

CAD is the most common cause of death in older persons and was present in 43% of 1,160 men and in 41% of 2,464 women, mean age 81 years. Hypertension was present in 60% of these older women and in 57% of these older men. The prevalence of valvular aortic stenosis, aortic regurgitation, mitral regurgitation, and MAC increases with age in older men and in older women. The prevalence and incidence of CHF increase with age. CHF is the most common cause of hospitalization in persons aged 65 years and older. The prevalence of normal LV ejection fraction associated with CHF increases with age and is

[Diabetes and heart transplantation].

Diabetes is frequently encountered in patients presenting with end-stage heart failure to be listed for transplantation. While diabetes used to be a contra-indication for heart transplantation, careful preoperative evaluation and individualized postoperative medication lead to long-term outcome after heart transplantation equal to non-diabetic patients. About 1/3 of transplanted patients develop a post-transplant diabetes. Several risk factors have been identified leading to this condition. Mostly, post-transplant diabetes is of temporary nature. Several studies have shown no impact of diabete…

Simple congenital heart lesions.

Cardiac magnetic resonance imaging (CMR) can provide comprehensive anatomic and physiological information about the cardiovascular system. The review article describes the application of CMR to several simple congenital heart lesions: atrial septal defects and other interatrial communications, ventricular septal defects, patent ductus arteriosus, partially anomalous pulmonary venous connection, and coarctation of the aorta. The anatomy, clinical features, and management of these lesions are discussed. CMR techniques to evaluate patients with these lesions are described and relevant literature …

Heart-touching Chilaiditi's syndrome.

Symptomatic hepato-diaphragmatic interposition of a bowel loop or Chilaiditi's syndrome is a peculiar anatomical condition most often found … hepatic flexure-with the interposition of the diaphragm-came in contact with the right side of the heart in the left lateral, but not in the supine, decubitus. This finding was reproduced by echocardiography which also showed virtually unaltered hemodynamics after the change of posture. ECG, left and right ventricular global and regional function as we …

Progress in heart transplantation.

The field of heart transplantation was built upon the discoveries of immunity and tolerance by Landsteiner, Medawar, Burnet, and others, as well as technical advancements in surgical technique by Carrel. Since the first successful human heart transplant performed by Christiaan Barnard in 1967, there has been substantial progress in the field of heart transplantation, especially over the last several decades. With advances in immunosuppression and surgical techniques, the rates of acute rejection and infection leading to graft failure have declined. However, the detection of acute and chronic a…

Univentricular heart: management options.

The term ''Univentricular Heart'' encompasses a wide variety of heart defects that functionally and physiologically constitute a single ventricular chamber. The terminology ''univentricular repair'' is frequently used in the surgical literature to include those biventricular hearts that are not amenable for a final two ventricle repair and need to go through the same surgical stages as with a functionally univentricular heart, culminating finally in a total cavo-pulmonary connection. Broadly, treatment is focused on controlling the pulmonary blood flow in early infancy, by means of aorto-pulmo…

[Hypoplastic left heart syndrome].

Survival of infants born with hypoplastic left heart syndrome (HLHS) has steadily improved since Norwood and colleagues first reported a multistage reconstructive approach in 1983. Although several experienced centers have achieved operative survival for the Norwood procedure between 63-94%, this procedure still remains a challenging step with high mortality for many institutions. Despite successful reconstructive surgery, most deaths occur in the first 24 to 48 hours after surgery due to hemodynamic instability secondary to unpredictable rapid fall in pulmonary resistance. Therefore, many eff…
