Fatal pulmonary oedema following oxytocin administration in a pregnant woman with acute myocardial infarction.

Acute myocardial infarction is a rare event in pregnant patients. Mechanical valves are naturally thrombogenic and require careful anticoagulation. Pregnancy produces a hypercoagulable situation and necessitates close follow-up in pregnant patients with mechanical heart valves. We present a 34-year-old pregnant woman who had mitral and aortic valve prosthesis. She developed resistant pulmonary oedema in the post-partum period after myocardial infarction. Oxytocin was used in this patient to induce midterm labour and prevent post-partum bleeding. Issues surrounding management of pulmonary oedem

Valve replacement in children under 15 years with rheumatic heart disease.

From 1967 through 1984, single- or double-valve replacement for rheumatic heart disease (RHD) was undertaken in 184 children (aged 4 to 15 years) in Tunis. At the time of operation most patients were in an advanced state of cardiac disability, but most of them were still in sinus rhythm. A total of 222 valves were inserted. The most commonly used replacement valve was the Starr-Edwards caged-ball prosthesis, with 12 hospital deaths (6.5%). There were no hospital deaths among 19 children who had a (glutaraldehyde-treated porcine) xenograft replacement valve; however, a much higher rate of valve

Sutureless aortic valves: timing, technique, and evidence.

PURPOSE OF REVIEW: This review synthesizes contemporary evidence for sutureless aortic valve replacement (SuAVR). It focuses on indications, imaging-guided sizing, operative technique, outcomes, and complications to clarify where SuAVR meaningfully complements or substitutes conventional surgical aortic valve replacement and transcatheter aortic valve implantation. RECENT FINDINGS: Modern studies show SuAVR consistently shortens cardiopulmonary bypass and cross-clamp times by 25-50%, produces low discharge gradients and larger effective orifice areas, and is particularly advantageous in elder

Balloon Seal Separation Leading to SAPIEN 3 Transcatheter Heart Valve Deployment Failure: Complications and Management.

We present two cases of failure of balloon inflation secondary to balloon separation from the delivery catheter when implanting the SAPIEN 3 transcatheter heart valve (Edwards Lifesciences, Irvine, CA, USA). Although very uncommon, this is a potentially disastrous complication of transcatheter intervention. Case 1 highlights the complexity of the problem when it occurs and subsequent complications. Case 2 highlights how to manage this issue successfully.

Transcatheter Transseptal Mitral Valve-in-Valve Replacement: An Early Australian Case Series and Literature Review.

BACKGROUND: Transcatheter mitral valve implantation for degenerated bioprostheses has recently emerged as an alternative to redo mitral valve surgery, particularly in patients at high risk for reoperative cardiac surgery. We sought to examine our early experience of transcatheter transseptal mitral valve-in-valve procedures. METHODS: Prospectively collected data was retrospectively reviewed in patients undergoing transcatheter transseptal mitral valve-in-valve implantation using the Edwards Sapien 3 balloon expandable bioprosthesis (Edwards Lifesciences, Irvine, CA, USA). RESULTS: Seven (7)

Transcatheter mitral valve interventions: current status and future perspective.

AIMS: With the recent developments in the field of transcatheter aortic valve replacement for aortic stenosis there has been a similar advance in the field of transcatheter mitral valve therapy for mitral regurgitation (MR). Both the anatomy of the mitral apparatus and the spectrum of pathology of MR are more complex than for aortic valve disease, and thus the development of MR therapies has been more complicated and less rapid. METHODS AND RESULTS: The purpose of this review of recent literature is to provide a synopsis of the present technologies under development for percutaneous therapy f

Long-Term Outcomes of Patients with Self-Expandable Transcatheter Heart Valve Embolized in the Aorta.

This study assesses the long-term outcomes of patients who suffered from self-expandable transcatheter heart valve (THV) embolized in the aorta in transcatheter aortic valve implantation (TAVI).We retrospectively reviewed the patients with self-expandable THV embolized in the aorta. Follow-up computed tomography was performed to assess the THV migration, struct fractures, and device-related aortic complications.Of the 539 TAVI patients, 11 suffered from self-expandable THV embolized in the aorta. Two patients underwent open-heart surgery to remove the embolized THVs in the ascending aorta. Emb

Propitious temporal changes in clinical outcomes after transcatheter compared to surgical aortic valve replacement; a meta-analysis of over 65,000 patients.

BACKGROUND: The treatment of symptomatic severe aortic stenosis (AS) has rapidly evolved over the past decade, in both transcatheter (TAVR) and surgical aortic valve replacement (SAVR), resulting in reported improved clinical outcomes. Operator experience and technical improvements have improved outcomes especially for patients undergoing TAVR. We sought to determine and compare 1-year outcomes using a contemporary meta-analysis. METHOD: We searched the Medline (MESH), Cochrane and Google scholar databases using keywords "AS", "atrial fibrillation" (AFib) and "stroke". We performed a meta-ana

Modes of failure of Trifecta aortic valve prosthesis.

OBJECTIVES: Several concerns have been recently raised regarding the durability of Trifecta prostheses. Different mechanisms of early failure were reported. Our aim was to study in a large population the modes of failure of Trifecta valves. METHODS: We conducted a retrospective analysis of patients who underwent surgical aortic valve replacement with a Trifecta prosthesis during the period 2010-2018. Details regarding the mode of failure and haemodynamic dysfunction were collected for patients who underwent reintervention for structural valve failure. The Kaplan-Meier method was used to calcu

Transcatheter valve-in-valve implantation for failed mitral prosthesis: the first experience in Japan.

An 82-year-old woman had a history of mitral valve replacement with a 25-mm MOSAIC (Medtronic, USA) for severe mitral regurgitation (MR) 8 years previously. Recently, she developed heart failure due to MR secondary to prosthetic valve failure. She underwent transcatheter valve-in-valve implantation with a 23-mm SAPIEN XT (Edwards Lifesciences, USA) to the prosthetic mitral valve by transapical approach. To our knowledge, this is the first reported case of transcatheter valve implantation for failed mitral prosthetic valve using valve-in-valve technique in Japan.

Bench evaluation of MyVal OCTACOR THVs at expansion limits for paediatric and valve-in-valve applications.

Balloon-expandable MyVal OCTACOR valves were evaluated in 3D-printed conduits under simulated flow. The 20 mm valve functioned from 16-24 mm, rupturing at 27 mm. The 29 mm expanded to 33 mm with mild insufficiency. Findings suggest OCTACOR valves may support repeated valve-in-valve procedures, with the 20 mm showing greater versatility for small-infant mitral replacements and as a potential alternative to Melody valves.
