Due to the central role of platelets in hemostasis, the clinical relevance of quantitative changes in platelet counts (< 150 G/l or > 450 G/l) may be significant. Thrombopoesis (= production of platelets) occurs in the bone marrow, and the hormone thrombopoetin takes control on its regulation. In thrombocytosis, primary causes have to be distinguished from the far more common reactive (= secondary) reasons. The most important form of primary thrombocytosis occurs in myeloproliferative neoplasms especially in essential thrombocythemia (ET). Clinically, increased thrombophilia, microcirculatory d

We describe an unusual case of acute paraplegia in a young adult (7.5-month-old) Long-Evans rat that resulted from a spontaneous T-cell lymphoma. At presentation, a neurologic exam revealed normal pelvic limb flexor reflexes, the absence of an anal reflex, and deep pain recognition. Radiographs did not identify any obvious spinal abnormality or osseous trauma, although the liver and spleen were prominent. Hematologic analysis disclosed leukocytosis with atypical lymphocytes. At necropsy, red, friable to gelatinous masses were found associated with the ventral aspect of the vertebral column at

The phenotype and, by inference, the cell of origin of some lymphocytic neoplasms has been defined by surface marker studies; however, the precise cellular origin of other neoplasms of the lymphoid system is still unknown. For example, with reference to hairy cell leukemia (HCL), cell marker data has been used in support of a monocytic, a T cell, or a B cell origin. If hairy cell leukemia is a B cell-derived neoplasm, the controversy may be resolved by genotyping the cells, using the rearrangement of immunoglobulin genes as a marker of the B cell nature of the process. Rearrangement of these g

Systemic mastocytosis (SM) is a rare blood disorder characterized by the clonal proliferation of mast cells in tissues. Mast cells release various vasoactive mediators, including histamine, leukotrienes, prostaglandins, platelet-activating factors, and cytokines such as tumor necrosis factor. Clinical manifestations can range from mild itching to severe distributive shock. In some rare cases, mastocytosis is associated with other blood disorders, such as systemic mastocytosis with associated hematologic neoplasm (SM-AHN). Almost all cases of SM exhibit a KIT point mutation. We report a rare ca

The occurrence of acute myeloid leukemia (AML) with a simultaneous diagnosis of breast cancer (BC) is rarely reported in the literature. The present study reports the case of a 50-year-old female patient diagnosed with AML coexisting with metastatic BC. Following one cycle of treatment with azacytidine in combination with oral venetoclax for AML, the patient achieved complete remission with incomplete hematological recovery. In addition, the mass in the left breast was smaller following adjuvant chemotherapy. However, due to a refusal from the patient to accept an allogeneic hematopoietic stem

Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a hematological malignancy characterized by recurrent skin nodules, an aggressive clinical course with rapid involvement of hematological organs, and a poor prognosis with overall survival. The rarity of the disease results in a few large-scale studies, a lack of controlled clinical trials for its management, and a lack of evidence-based guidelines. Here, we present a review of unmet clinical needs on the management of BPDCN by a panel of eleven experts involved in the research and clinical practice of BPDCN. Recommendations and proposals

Cutaneous lesions in the setting of myeloproliferative neoplasms and myelodysplastic syndromes are poorly understood. We report 6 patients with pruritic papular eruptions composed of mature T-lymphocytes with large clusters of CD123-positive cells. Double immunohistochemical studies demonstrated a lack of myeloid cell nuclear differentiation antigen in the CD123-positive cells, which expressed SPIB, confirming that they were mature plasmacytoid dendritic cells. Four patients were diagnosed with chronic myelomonocytic leukemia and 2 with myelodysplastic syndromes (AREB-I and myelodysplastic syn

The Janus kinase (JAK)2 gene, which is located on chromosome 9p24, is involved in the signaling transduction pathways of the hematopoietic and immune system. Mutations in the JAK2 gene have served as disease markers for myeloproliferative neoplasms (MPNs). The aim of the present study was to investigate the occurrence of the JAK2 gene mutation in 140 clinical samples, and to evaluate its clinical significance in MPNs and other hematological diseases. Genomic DNA was extracted from the peripheral blood leukocytes or bone marrow karyocytes of 140 clinical samples, which included 130 patients wit

In this issue of Blood, Savona and an international consortium of clinical investigators propose uniform response criteria for treatment trials enrolling adult patients with myelodysplastic/myeloproliferative neoplasms (MDS/MPNs). Such a proposal is needed because new drugs are finally being tested in these rare “overlap” syndromes that have both dysplastic and proliferative pathological features, and neither the International Working Group (IWG) response criteria for myelodysplastic syndromes nor the IWG Myeloproliferative Neoplasms Research and Treatment (IWG-MRT) response criteria for myelo

The tumor microenvironment (TME) plays a critical role at all stages of tumor progression. Cancer-associated fibroblasts (CAFs), as the main components of this environment, display diversities in origin, phenotype, and function, which contribute to cancer progression by regulating the tumor cell biology via cell-cell contact, releasing numerous regulatory elements such as growth factors (GFs), cytokines, and chemokines, and even remodeling the extracellular matrix (ECM). In this Review, we discuss the current understanding of CAFs' pro- or anti-tumor functions in various hematological malignan

Myeloid sarcoma, considered to herald the onset of a blast crisis in the setting of chronic myeloproliferative neoplasm/dysplasia, typically presents during the course of the disorder. Cutaneous involvement is uncommon and lesions on genital skin are seldom seen. We present a case of a well-differentiated myeloid sarcoma in the penile foreskin in an apparently healthy 29-year-old male presenting with phimosis. The unusual composition of the inflammatory cell infiltrate, and characteristic sparing of dermal blood vessels, nerves and smooth muscle fibres led to the correct diagnosis. Absence of

Myeloablative therapy followed by autologous hematopoietic stem cell (HSC) transplantation is a therapeutic option proposed for a variety of hematological and non-hematological diseases. However, although mortality due to this procedure is steadily decreasing, patients are still exposed to the risk of a number of complications negatively affecting their expectancy or quality of life. Adverse events due to HSC harvesting are rare and generally reversible. The early post-transplant complications include infections, mucositis, hepatic veno-occlusive disease and various acute organ toxicities. Imm

The likely first question to ask when encountering a soft tissue tumor for the first time is simply, "Is this carcinoma or sarcoma?" Lesions of unknown etiology should also nearly always include melanoma as a differential diagnosis due to its greatly elusive nature, and this case report reinforces the importance of also keeping a lymphoproliferative process at the forefront of one's mind while considering the possible overlapping features with other entities. We present a 64-year-old man with a soft tissue mass of the supraspinatus that was perceived as an epithelioid angiosarcoma due to the l

Mast cell leukemia (MCL) is the leukemic form of SM with at least 20% mostly immature mast cells on bone marrow aspirate. MCL may develop de novo, in the absence of a prior SM, or it may represent a progression from a previous SM. MCL may be sub-divided into the more frequent, aggressive acute form with signs of organ damage (C-findings) and the chronic form lacking C-findings and presenting a more stable course, although over time, progression to acute MCL is common. The 2022 WHO subtype of MCL with an associated hematological neoplasm was renamed MCL with an associated myeloid neoplasm in th

Mast cell disorders range from benign proliferations to systemic diseases that cause anaphylaxis and other diverse symptoms to mast cell neoplasms with varied clinical outcomes. Mastocytosis is the pathologic process of the accumulation of abnormal mast cells in different organs, mostly driven by KIT mutations, and can present as cutaneous mastocytosis, systemic mastocytosis (SM), and mast cell sarcoma. The WHO 5th edition classification divides systemic mastocytosis into bone marrow mastocytosis, indolent systemic mastocytosis, smoldering systemic mastocytosis, aggressive systemic mastocytosi

Acute myeloid leukemias (AMLs) are overlapping hematological neoplasms associated with rapid onset, progressive, and frequently chemo-resistant disease. At diagnosis, classification and risk stratification are critical for treatment decisions. A group with expertise in the clinical, pathologic, and genetic aspects of these disorders developed the International Consensus Classification (ICC) of acute leukemias. One of the major changes includes elimination of AML with myelodysplasia-related changes group, while creating new categories of AML with myelodysplasia-related cytogenetic abnormalities

Myeloproliferative disorders with eosinophilia may possess the FIP1L1-PDGFRα gene rearrangement. When this rearrangement is present, imatinib usually results in complete remission. In rare cases of imatinib resistance, there is poor evidence guiding second-line therapy. We present the case of a 71-year-old male who presented with abdominal discomfort, fevers, and leukocytosis with eosinophilia. The patient was diagnosed with a myeloproliferative neoplasm with eosinophilia and FIP1L1-PDGFRα rearrangement after a bone marrow evaluation revealed hypercellular marrow with eosinophilia and fluoresc

INTRODUCTION: The pancreas is an unusual but occasionally favored target for metastasis from other primary cancers. Metastases have been described for various non-hematologic neoplasms, such as renal-cell carcinoma, pulmonary small-cell carcinoma, melanoma and gastric carcinoma.
CASE REPORT: We report the case of a 77-year old man with a mass in the pancreatic head five years after anterior resection for adenocarcinoma of the colon and three years after resection of a lung metastasis. The patient underwent pylorus-preserving pancreaticoduodenectomy (Longmire-Traverso) and lymph-node dissectio…

A 47-year-old man, who had been diagnosed as having acute myelomonocytic leukemia (AMMOL) and had been treated with combination chemotherapy, was admitted to our hospital because he had developed melena. He had been judged to be in complete remission and had shown no signs of recurrence for years, Daunorubicin, vincristine 6-Mercaptopurine and Cyclophosphamide had been administered for maintenance and intensification therapy. He was well until January 1986, when this melena began. A barium enema was given and he was diagnosed as having rectal cancer. Amputation of the rectum and a permanent ab
