A Review of Systemic Hematological Manifestations and Stent Thrombosis.

Stent thrombosis (ST) has significant morbidity and mortality following percutaneous coronary intervention (PCI). ST is uncommon making the identification of nonprocedural predictors challenging. Numerous case reports of ST in both benign and malignant hematological conditions exist. Given ST could be a Herald event of an undiagnosed condition and condition-specific treatment may be available, it is important to consider specialist testing in an unexplained ST. This review included a case presentation of ST in previously undiagnosed paroxysmal nocturnal hemoglobinuria (PNH) and a literature re…

Novel Insights into Factor D Inhibition.

Complement-mediated diseases or complementopathies, such as Paroxysmal nocturnal hemoglobinuria (PNH), cold agglutinin disease (CAD), and transplant-associated thrombotic microangiopathy (TA-TMA), demand advanced complement diagnostics and therapeutics be adopted in a vast field of medical specialties, such as hematology, transplantation, rheumatology, and nephrology. The miracle of complement inhibitors as "orphan drugs" has dramatically improved morbidity and mortality in patients with otherwise life-threatening complementopathies. Efficacy has been significantly improved by upstream inhibit…

Fatal acute babesiosis associated with Babesia venatorum infection (Babesia sp. EU1) in a captive reindeer calf in Switzerland.

Babesia venatorum was isolated from a captive reindeer calf in Switzerland. The clinical signs consistent with acute babesiosis included hemolytic anemia and hemoglobinuria. The diagnosis was made based on visualization of intraerythrocytic parasites in the stained blood smears and confirmed by PCR analysis of the 18S rRNA gene, with subsequent species identification within Babesia confirmed by sequencing. The reindeer calf was initially treated with supportive care and an antiprotozoal drug (imidocarb dipropionate) but died a few days after hospitalization. Babesia venatorum is also known as …

[Exertion-related abnormalities in the urine].

Strenuous exercise can lead to the occurrence of various abnormalities in the urine. Hematuria and proteinuria are those most frequently described. Other less common possibilities are pigmenturia like hemoglobinuria and myoglobinuria. These urine abnormalities are found not only in sportsmen/women or soldiers, but also in percussionists. Other possible causes are hereditary metabolic myopathy and the use of drugs or medication. Exertion-related urine abnormalities occur frequently and are usually benign. Any abnormalities should disappear after a period of 24-72 h of rest. Exertional rhabdomyo…

Case report and review: alloimmunization, delayed hemolytic transfusion reaction, and clinically significant anti-Yt(a) in a patient with Beta-thalassemia/sickle cell anemia.

A 26-year-old female with Beta-thalassemia/sickle cell anemia was admitted to the hospital with symptoms of a painful crisis. During the next 4 days her hematocrit decreased to 13 percent, and there was reticulocytopenia. She was transfused with four units of red blood cells that were microscopically incompatible, and the hematocrit increased to 29 percent. Eight days later the patient was readmitted with back pain, hemoglobinuria, and a hematocrit of 27 percent. Anti-E, -c, -Jka, and -Yta were identified. The direct antiglobulin test was positive, and the eluate contained anti-c and -Jka. The…

Ethylene glycol toxicosis in cattle.

A 1-month-old Jersey calf died of oxalate nephropathy. The calf had access to antifreeze (ethylene glycol) 3 days prior to death. Since ethylene glycol toxicosis had not been reported in cattle, the effects or oral administration of ethylene glycol were studied in 7 calves and 3 cows. The toxic dose ranged from 2 to 10 ml of ethylene glycol per kg of body weight. Clinical signs were increased respiration, staggering gait, paraparesis, depression and later, recumbency and death. Hemoglobinuria and epistaxis were seen at doses of 10mg/kg of body weight. Azotemia, hypocalcemia and neutrophilia we…

Blood transfusions and the Rh factor.

When Rh-negative persons are given transfusions of Rh-positive blood, more than 50 per cent are sensitized to the Rh(0) factor. Such sensitization of female children may be the cause of hemolytic disease in their offspring many years later, while severe hemolytic reactions may follow a second transfusion of Rh-positive blood in either sex. The gross hemolysis of transfused blood may be entirely asymptomatic, however. In one case a pint of blood was completely hemolyzed within two hours without producing symptoms. The only signs were hemoglobinuria, low grade jaundice, urobilinogenuria and a ri…

Immune-mediated hemolytic anemia.

Hemolytic anemia due to immune function is one of the major causes of acquired hemolytic anemia. In recent years, as more is known about the immune system, these entities have become better understood and their treatment improved. In this section, we will discuss three areas in which this progress has been apparent. In Section I, Dr. Peter Hillmen outlines the recent findings in the pathogenesis of paroxysmal nocturnal hemoglobinuria (PNH), relating the biochemical defect (the lack of glycosylphosphatidylinositol [GPI]-linked proteins on the cell surface) to the clinical manifestations, partic…

Zilucoplan is effective for ravulizumab-refractory generalized myasthenia gravis with the C5 p.Arg885His variant: a case report.

Complement inhibition is highly effective for complement-mediated diseases such as paroxysmal nocturnal hemoglobinuria (PNH) and generalized myasthenia gravis (gMG). However, certain C5 variants impair target engagement. In vitro, eculizumab and ravulizumab fail to achieve complete terminal complement blockade, and, in PNH, clinical efficacy is actually compromised in carriers. We report an anti-acetylcholine receptor antibody-positive gMG patient harboring C5 c.2654G>A (p.Arg885His) who exhibited persistent disease activity and only partial suppression of hemolytic complement activity (CH50) …

Clinical predictive significance of biomarker molecules elevation during the transition period in cattle suffering from different pathological states: A review.

The transition period (TP), which extends from 3 weeks before 3 weeks post parturition, is a critical period regarding the health, productivity, and profitability of dairy animals, during which most health disorders arise, including lameness, mastitis, rumen acidosis, ketosis, hypocalcemia (HC) (milk fever), left-displaced abomasum, fatty liver, hypophosphatemia and post-parturient hemoglobinuria, subacute ruminal acidosis, RP, and metritis. Biomarkers are biological molecules distributed in blood, body fluids, or tissues that represent physiological or pathophysiological indicators of events,…

Immune-mediated hemolytic anemia in a bottlenose dolphin: case report and literature review.

A 15-y-old captive-born female bottlenose dolphin (Tursiops truncatus) had slowly progressive lethargy, inappetence, and pallor of the oral mucosa. Serial hemograms and serum biochemistry analysis revealed hemolyzed plasma, gradually severe anemia with reticulocytosis, spherocytosis, pronounced neutrophilia with left shift and toxic change, and indirect hyperbilirubinemia. Urinalysis identified both hemoglobinuria and bilirubinuria. Hyperglobulinemia with polyclonal gammopathy was evident by protein electrophoresis, and the saline agglutination test was positive. Splenomegaly and hyperechogeni…

Novel Molecular Therapies and Genetic Landscape in Selected Rare Diseases with Hematologic Manifestations: A Review of the Literature.

Rare diseases affect less than 1 in 2000 people and are characterized by a serious, chronic, and progressive course. Among the described diseases, a mutation in a single gene caused mastocytosis, thrombotic thrombocytopenic purpura, Gaucher disease, and paroxysmal nocturnal hemoglobinuria (KIT, ADAMTS13, GBA1, and PIG-A genes, respectively). In Castleman disease, improper ETS1, PTPN6, TGFBR2, DNMT3A, and PDGFRB genes cause the appearance of symptoms. In histiocytosis, several mutation variants are described: BRAF, MAP2K1, MAP3K1, ARAF, ERBB3, NRAS, KRAS, PICK1, PIK3R2, and PIK3CA. Genes like H…

Glucose-6-phosphate dehydrogenase variants associated with favism in Thai children.

In a study conducted at Songklanagarind Hospital in the south of Thailand, the subjects were 225 patients (210 boys and 15 girls) with glucose-6-phosphate dehydrogenase (G6PD) deficiency. Favism was found in 3.6% of the G6PD-deficient children. Approximately one half of the G6PD-deficient patients with favism were younger than 2 years. Sudden onset of anemia was found within 1 to 3 days after ingestion of dried fava beans. The classic features of favism, which are pallor, hemoglobinuria, and jaundice, were detected in all cases. To characterize the known G6PD mutations in Thai children, molecu…
