BACKGROUND: Thoracic injuries, especially cardiac, vascular, and transmediastinal injuries, are amongst the most lethal of penetrating injuries.
METHOD: Our experience at Groote Schuur Hospital is reviewed, where up to 1,000 patients were admitted annually with penetrating chest wounds between 1982 and 1997.
RESULTS: The approximate pre-hospital mortality was 86% with penetrating cardiac injuries, 92 % with extrapericardial vascular injuries, and 11 % with pulmonary injuries. Less than 2% of pneumothorax cases and less than 10% of haemothorax cases required surgical intervention. Thoracoscop

Two patients are described with hemorrhagic thoracic effusions who required central venous catheterization. Presumed subclavian and internal jugular vein cannulation, ipsilateral to the hemorrhagic thoracic effusions, was confirmed by the operators in each case by the persistent aspiration of blood. Subsequent clinical and radiologic evaluation revealed that the vascular catheters were introduced into the pleural space. In both individuals, the persistent aspiration of extravascular hemorrhagic fluid mimicked intravascular catheter positioning. Physicians treating patients with hemorrhagic tho

The diagnostic features of subpulmonary haemothorax (SH) are often overlooked. A retrospective review of 399 consecutive patients discharged with diagnoses of post-traumatic haemo- or haemopneumothorax yielded 51 (12.8 per cent) instances of SH on the initial radiographs. In 24 (47.1 per cent) of these initial diagnosis of SH had been missed. A concerted teach-in followed in order to reduce the rate of error. In a subsequent prospective study of 1773 consecutive patients with haemothorax (510) or haemopneumothorax (1263), mostly due to penetrating injuries, and who had undergone pre-interventi

Haemothorax can occur as a result of thoracic trauma or following diagnostic or therapeutic pleural aspiration. Early removal of blood from the thoracic cavity is essential to prevent coagulation and fibrin deposition within the pleural cavity. The case described is one in which a large coagulated haemothorax was drained via a standard pleural drain following liquefaction by the use of a proprietary fibrinolytic agent more commonly used in the desloughing of wounds.

A total of 60 cases of thoracic trauma in children, treated over a period of 4.5 years (January 1983 to June 1987), were analysed. Blunt-impact trauma was the major cause in 58.33% cases, stressing that bullock-cart accidents and bull-horn and buffalo-hoof injuries are still a continuing cause of chest injury in developing countries. Haemo- and/or pneumothorax were the main injuries (78.33%), but cardiac and oesophageal involvement were seen, in two cases each. Isolated first rib fracture was another surprising finding. Except for three, all the cases presented within 24 h of injury. Cyanosis

During an 8-month period, 242 patients with thoracic injuries were admitted to one of the general surgical units at Baragwanath Hospital. Of these, 7.4% required surgical exploration, while 92.6% had pneumothorax treated conservatively by insertion of a single drain at the mid-axillary line in the 5th intercostal space. Of patients managed conservatively 76 had pneumothorax and 148 haemopneumothorax. There were no deaths in this series. All patients explored recovered well from the operation and were discharged after a mean period of 6 days. Of those with a pneumothorax managed conservatively

One hundred and fifty battle casualties with injuries of the chest are reported; most of them resulted from shell fragments. More than 70 per cent were treated by closed drainage by a tube. Early thoracotomy was required in 7.3 per cent of cases, and 5 per cent required a late thoracotomy for complications. Selected cases of flail chest injuries were treated without operation. The mortality rate in our series was 3.3 per cent.

We report the case of a four year old female patient who underwent an unsuccessful puncture of the subclavian vein in another department with an adult subclavian catheter. Unilateral haematothorax with massive bleeding from the right Art. thoracica interna occurred as a sequel of this attempt and was successfully treated in our department.

A case of hereditary haemorrhagic telangiectasia (Osler-Weber-Rendu disease) is described who presented with severe, central chest pain mimicking acute myocardial infarction, a presentation which has not been described before. He was found to have developed spontaneous haemothorax which is a very rare complication of this disease.

A massive haemothorax occurring acutely during haemodialysis in a child of 8 1/2 years is reported. The pleural effusion occurred synchronously with a round, dense shadow in the parenchyma similar to an intra-pulmonary haematoma. The density became bullous in six weeks and disappeared in three months. The suggested cause of this haemothorax is a pulmonary embolism whose site of origin was the arteriovenous fistula. This would have been responsible for the adjacent pleural and parenchymal haemorrhage in the lung. The heparin given intravenously at the beginning of the haemodialysis session prob

A report is given about 360 percutaneous transthoracic lung biopsies in diffuse affections of the lung framework. By means of a good cooperation between skilled lung physician and experienced lung pathologist it was able to get a definitive diagnosis in 88.3% of the cases. The expense concerning this diagnostic procedure is small. As for complications there occurred pneumothoraces (15.8%), lung haemorrhage (5.3%) and haemothorax (1 case).

Find needle biopsy of tumours of the lung and mediastinum represents an accurate method which can be performed pain-free and rapidly without undue effort and cost. It can be repeated at any desired time. Absolute contraindications are very rare. This method can be applied also in outpatients, provided certain precautions are observed. In many cases, it eliminates the far greater risk of exploratory thoracotomy. Both peripheral and central lung tumours, including mediastinal tumours, can be identified and differentiated. More than nine out of ten bronchial carcinomas are clearly identified. In

Pelvic pain is a common presentation to the emergency department (ED). For female patients, endometriosis can be difficult to diagnose and can have life-threatening complications if missed. In this case report, we present a case of a patient initially presenting to the ED with a few days of crampy lower abdominal pain. After initial imaging, she was found to have a large pelvic hematoma with concern for active extravasation and a large hemothorax. After further evaluation, she was suspected of having endometriosis leading to thoracic endometriosis and a catamenial pneumothorax. Although endome…

Among the different types of central venous access, 2 are mainly recognized, the internal jugular and the subclavian. The most common complications of these invasive procedures are hematoma, vascular injury, pneumothorax, and hemothorax. This review presents 2 atypical complications in the central vascular approach in a patient seen in the emergency department. Complications were air and thrombotic embolisms. Physicians dealing with central venous catheter placement (emergency department, intensive care unit, surgical room, interventionist procedures) should be aware that vascular air embolism…
