Exposure keratopathy in sedated and ventilated patients.

PURPOSE: The purpose of this study is to determine the frequency of exposure keratopathy in sedated/mechanically ventilated patients in the intensive care unit and its risk factors. MATERIALS AND METHODS: This is a prospective cohort study including all patients admitted to an adult intensive care unit department between March and October 2010 who were sedated and mechanically ventilated. Patients were examined by an ophthalmologist 1 to 5 days after commencing ventilation and subsequently every day. Examination included assessment of lid position, conjunctival edema (chemosis), and corneal c

Congenital diaphragmatic hernia: quality improvement using a maximal lung protection strategy and early surgery-improved survival.

UNLABELLED: To evaluate the effectiveness of a novel protocol, adopted in our institution, as a quality improvement project for congenital diaphragmatic hernia (CDH). A maximal lung protection (MLP) protocol was implemented in 2019. This strategy included immediate use of high-frequency oscillatory ventilation (HFOV) after birth, during the stay at the Neonatal Intensive Care Unit (NICU), and during surgical repair. HFOV strategy included low distending pressures and higher frequencies (15 Hz) with subsequent lower tidal volumes. Surgical repair was performed early, within 24 h of birth, if po

Bench-to-bedside review: Ventilator strategies to reduce lung injury -- lessons from pediatric and neonatal intensive care.

As in the adult with acute lung injury and acute respiratory distress syndrome, the use of lung-protective ventilation has improved outcomes for neonatal lung diseases. Animal models of neonatal respiratory distress syndrome and congenital diaphragmatic hernia have provided evidence that 'gentle ventilation' with low tidal volumes and 'open-lung' strategies of using positive end-expiratory pressure or high-frequency oscillatory ventilation result in less lung injury than do the traditional modes of mechanical ventilation with high inflating pressures and volumes. Although findings of retrospec

First week of life respiratory management and pulmonary ventilation/perfusion matching in infants with bronchopulmonary dysplasia: a retrospective observational study.

OBJECTIVE: To investigate the association between early neonatal respiratory management in infants with bronchopulmonary dysplasia (BPD) and the degree of pulmonary ventilation perfusion-matching (V/Q) at term. METHODS: 30 preterm infants with a diagnosis of BPD who were initially treated with either controlled mechanical ventilation/continuous positive airway pressure (CMV/CPAP) (n = 14) or high-frequency oscillatory ventilation (HFOV) using a high lung-volume strategy (n = 16) were retrospectively included in this study. All infants underwent pulmonary V/Q single photon emission computed to

Neonatal ventilatory techniques - which are best for infants born at term?

Few studies have examined ventilatory modes exclusively in infants born at term. Synchronous intermittent mandatory ventilation (SIMV) compared to intermittent mandatory ventilation (IMV) is associated with a shorter duration of ventilation. The limited data on pressure support, volume targeted ventilation and neurally adjusted ventilatory assist demonstrate only short term benefits in term born infants. Favourable results of high-frequency oscillatory ventilation (HFOV) in infants with severe respiratory failure were not confirmed in the two randomised trials. Nitric oxide (NO) in term born i

[Main points in intensive care medicine].

PURPOSE: Progresses in intensive care medicine have concerned major syndromes associated with life threatening organ dysfunction, like the acute respiratory distress syndrome (ARDS) or septic shock. CURRENT KNOWLEDGE AND KEY POINTS: A better understanding of interactions between mechanical ventilation and the underlying lung lesions, has lead to define a so-called "lung protective ventilation", which resulted in an improved prognosis of ARDS. In numerous situations of acute respiratory failure, including acute exacerbation of chronic obstructive pulmonary disorder or immunosuppressed patients

25 years of respiratory support of newborn infants.

Respiratory support of newborn infants has changed in the last 25 years, because of new knowledge of patho-physiology, controlled studies of respiratory therapy and the realisation of perinatal centers. Respiratory support has changed from the "blow in--suck out" approach, inevitably leading to severe atelectasis, high morbidity and mortality to a now very sophisticated therapy with reduced mortality and morbidity also in very-low-birth-weight infants, who were hopeless patients 25 years ago. Major milestones of this development were the introduction of continuous distending pressure to surfac

Imaging of ventilation and lung injury with low-frequency tomographic ultrasound.

BACKGROUND: Mechanical ventilation in the intensive care unit (ICU) is a life-saving technique for patients with acute respiratory failure, but is also associated with a high incidence of complications in the injured lung. Currently, there is no widely used monitoring technique to guide the ventilator setting to facilitate a precision medicine approach or to provide a real-time alert for developing adverse pulmonary conditions. Conventional ultrasound has been used as a thoracic bedside technology, but the lack of signal penetration into lung tissue results in images that often contain more in

High-flow nasal cannula and extubation success in the premature infant: a comparison of two modalities.

OBJECTIVE: To compare the effectiveness of Fisher and Paykel (FP) and Vapotherm (VT) high-flow nasal cannula (HFNC) in preventing reintubation either within 72 h or <7 days after extubation of premature infants. The primary outcome was the rate of extubation failure defined as reintubation within 72 h. Secondary outcomes included reintubation after ≤7 days. STUDY DESIGN: This was a prospective, randomized pilot study comparing the extubation success of 40 infants born between 26 and 29 weeks of gestation. RESULT: The rate of extubation failure at 72 h was 18% for FP and 9% for VT. The failur

Lung-protective ventilatory strategies in intubated preterm neonates with RDS.

This article provides a narrative review of lung-protective ventilatory strategies (LPVS) in intubated preterm infants with RDS. A description of strategies is followed by results on short-and long-term respiratory and neurodevelopmental outcomes. Strategies will include patient-triggered or synchronized ventilation, volume targeted ventilation, the technique of intubation, surfactant administration and rapid extubation to NCPAP (INSURE), the open lung concept, strategies of high-frequency ventilation, and permissive hypercapnia. Based on this review single recommendations on optimal LPVS cann…

Selective fiberoptic left main-stem intubation to treat bronchial laceration in an extremely low birth weight neonate.

A neonate born at 24 weeks gestation developed a right tension pneumothorax that persisted despite the placement of multiple thoracostomy tubes, the use of high-frequency ventilation and postural therapy. On day-of-life 11, bedside bronchoscopy revealed a laceration at the junction of the trachea and right main-stem bronchus. Under fiberoptic guidance the left main-stem bronchus was intubated. Single-lung ventilation then was employed for 24h, after which time the pneumothorax never recurred. We postulate that the diversion of positive-pressure ventilation away from the laceration site allowed…

Novel therapies for acute respiratory failure.

Mortality in acute respiratory failure in the non-neonatal pediatric patient has not changed substantially in 20 years, despite advances and refinements in conventional therapeutic strategies and technology. A host of innovative therapies are currently in various stages of investigation, including high frequency ventilation, pressure control ventilation, permissive hypercapnia, extracorporeal membrane oxygenation, exogenous surfactant administration, inhaled nitric oxide, and liquid ventilation. While none of these therapies has yet been prospectively studied in non-neonatal pediatric patients

Neurogenic hypertension characterizes children with congenital central hypoventilation syndrome and is aggravated by alveolar hypoventilation during sleep.

OBJECTIVES: Autonomic nervous system (ANS) dysfunction characterizes congenital central hypoventilation syndrome (CCHS). The objectives were to describe ambulatory blood pressure monitoring (ABPM) of children with CCHS, to assess cardiac ANS dysfunction as compared with control participants and to search for relationships between ANS dysfunction and blood pressure (BP) or night-time PCO 2 measurements. METHODS: Retrospective study of ABPM of children with CCHS and case (CCHS)-control (healthy children) study of heart rate variability (HRV) indices obtained during polysomnography (wakefulness,

High tidal volume ventilation is not deleterious in infant rats exposed to severe hemorrhage.

BACKGROUND: Both high tidal volume (V(T)) ventilation and hemorrhage induce acute lung injury in adult rodents. It is not known whether injurious ventilation augments lung injury in infant rats exposed to severe hemorrhage. METHODS: Two-week-old rats were allocated for ventilation with VT 7 mL/kg and positive end-expiratory pressure (PEEP) 5 cm H₂O (low V(T)) or V(T) 21 mL/kg and PEEP 1 (high V(T)) for 4 hours. Additional rats were subjected to volume-controlled hemorrhage and delayed saline resuscitation, followed by low V(T) or high V(T) ventilation for 4 hours. Nonventilated control groups

Flow-controlled One-lung Ventilation During Carinal Sleeve Reconstruction: Anaesthetic Management and Technical Feasibility.

Carinal sleeve resection with reconstruction represents one of the most challenging thoracic surgical procedures and requires meticulous anaesthetic management to maintain adequate gas exchange within a shared and surgically interrupted airway while preserving an unobstructed surgical field. Conventional ventilation strategies, including cross-field ventilation and high-frequency jet ventilation, may interfere with surgical exposure or result in unstable ventilation. We describe the anaesthetic management of a 64-year-old male patient in whom flow-controlled ventilation- one-lung ventilation (

Airway and Ventilation Rescue During Tracheal Resection: A Case Report of Intraoperative Troubleshooting.

Tracheal resection requires shared airway management when conventional ventilation fails. The present paper describes a 73-year-old man with chronic obstructive pulmonary disease (COPD), hypertension, and prior neck radiotherapy who underwent tracheal resection with primary anastomosis for chondrosarcoma. Initial intubation with a single-lumen endobronchial tube produced a major leak, requiring exchange to an endotracheal tube. Lung isolation with an EZ blocker® (Teleflex Life Sciences Ltd., Athlone, Ireland) was achieved but lost during thoracotomy in the lateral decubitus position with a chi

Modified double lumen tube for a unique bronchial and carinal resection in a patient undergoing uniportal VATS for tumour: A case report.

For bronchial sleeve and carinal resection and reconstruction during uniportal video-assisted thoracic surgery (VATS), ventilation technique remains a demanding challenge for the anesthesiologists. The ventilation techniques require maintaining adequate gas exchange while providing a good surgical exposure. The case we present was a 58-year-old female with carcinoma in the right upper lobe involving the right main bronchus and the lower trachea. Right upper sleeve lobectomy, carinal resection and reconstruction was performed under uniportal VATS. A modified double-lumen tube (DLT) was inserted

The effect of mechanical ventilation compared to high-flow nasal cannula on gastric residual volume and reflux events using novel automated technology.

BACKGROUND & AIM: High flow nasal cannula (HFNC) oxygen therapy is frequently used following extubation. A case report, utilizing an innovative medical technology (The smART + Platform, ART MEDICAL Ltd., Netanya, Israel) that enables the detection of gastric refluxes and gastric residual volumes (GRV), has suggested that HFNC may be associated with increased reflux events and GRV. This study measured reflux events and GRV using smART+ in mechanically ventilated patients before and after extubation while they were receiving HFNC therapy. We aim to show if there is a significant difference in re
