Protecting the interests of the child bone marrow donor.

At a time when designer babies have been created to act as cord blood donors to sick siblings, ethical debate has focused predominantly on the extent to which it is acceptable to create one human being to assist another. However, children are frequently used this way, by their families and doctors who extract their bone marrow, to try to save the life of another, usually a sibling. With any life-threatening illness, there is the possibility that the urgency of the sick sibling's need means that the short-term welfare of the donor child receives less attention than it should by parents and doct

A nightmare for King Solomon: the new reproductive technologies.

King Solomon had only two claimants for the baby whose fate he was to decide. With the new reproductive technologies, several people may assert claims to a child whose existence would have been impossible until only recently, and a mass of legal and ethical problems have been created that could barely have been envisioned even half a century ago. It can, for example, no longer be assumed that the woman who carries and gives birth to a baby is that child's biological mother. The legal claims threaten to turn a child into a piece of property rather than a human being with rights and needs. Exist

Medical conditions, risk exposure, and truck drivers' accidents: an analysis with count data regression models.

Recent studies do not agree on the possible relationship between medical conditions and traffic safety; most of them do not control for exposure factors. This problem has become more pertinent for scientific studies because of litigation that showed that present regulations about access to driver permits might contravene human rights legislation. In our study, we estimate the effect of different medical conditions on truck drivers' distributions of accidents. Our data and our models permit simultaneous control for age; medical conditions; exposure factors measured by hours, kilometer, and qual…

Cavopulmonary Anastomosis in a Patient With Arrhythmogenic Right Ventricular Cardiomyopathy With Severe Right Ventricular Dysfunction.

A 26-year-old lady presented with exertional dyspnea, palpitations, central cyanosis, and oxygen saturations of 80% in room air. Her electrocardiogram, echocardiogram, and cardiac magnetic resonance were diagnostic of arrhythmogenic right ventricular dysplasia. There was no documented ventricular arrhythmia or syncopal episodes and Holter recordings were repeatedly normal. Cardiac hemodynamics showed right to left shunt through atrial septal defect, low pulmonary blood flow, normal atrial pressures, and minimally elevated right ventricular end-diastolic pressures. Since her presenting symptoms

The Iran Healthcare Professionals' Charter of Rights.

BACKGROUND: In today's rapidly changing social and cultural landscape, recognizing the rights of Healthcare Professionals (HCPs) alongside those of the patients has become essential for a fair and sustainable healthcare system. While patient rights are widely acknowledged, HCPs in Iran often face heavy workloads, poor work-life balance, and reduced professional dignity. These challenges not only affect their well-being but can also compromise the quality of care the patients receive. This report presents the development of the "Charter of Rights for HCPs of Iran," prepared by the Supreme Counc

Arrhythmias in patients with mechanical ventricular dysfunction and myocardial stretch: role of mechano-electric feedback.

Patients with dilated cardiomyopathy, ventricular volume or pressure overload, or dysynergistic ventricular contraction and relaxation are prone to develop severe ventricular arrhythmias. In these patients it has been suggested that the abnormal mechanics of contraction can disturb 'mechano-electric feedback', also known as 'contraction-excitation feedback', which is defined as the development of electrophysiological changes during or after changes in mechanical loading. This electrical instability, expressed by significant changes in ventricular repolarization and refractoriness and by the de

Right ventricular systolic function and mechanical dyssynchrony in ischemic or non-ischemic dilated cardiomyopathy: A speckle-tracking study.

AIM: This study assessed RV dyssynchrony (irrespective to QRS duration) and RV systolic function in non-ischemic dilated cardiomyopathy (NIDCM) versus ischemic dilated cardiomyopathy (IDCM) patients by using different echo-Doppler modalities. METHODS: Eighty-five cases (48 patients with DCM [whether ischemic or non-ischemic] and 37 age-matched healthy controls) were studied. Conventional echo-Doppler study, tissue Doppler (TDI), and speckle tracking (STE) were carried out to measure LV and RV systolic function. Time-to-peak negative longitudinal strain at the four RV sites were assessed by TD

Assessment of the effects of inspiratory load on right ventricular function.

PURPOSE OF REVIEW: The right ventricle (RV) plays a pivotal role during respiratory failure because of its high sensitivity to small loading changes during inspiration. Both RVs, preload and afterload, are altered during inspiration, either in spontaneous breathing or during mechanical ventilation. Some clinical situations especially affect RV load during inspiration, for example acute asthma and acute respiratory distress syndrome. The aim of this review is to explain and to summarize the different mechanisms leading to RV failure in these situations. RECENT FINDINGS: Research has recently r

Women's preferences for contraceptive counseling in Mexico: Results from a focus group study.

BACKGROUND: Client-centered contraceptive counseling is critical to meeting demand for contraception and protecting human rights. However, despite various efforts to optimize counseling, little is known outside of the United States about what individuals themselves value in counseling. In the present study we investigate women's preferences for contraceptive counseling in Mexico to inform efforts to improve service quality. METHODS: We conducted applied qualitative research, using six focus group discussions with 43 women in two cities in Mexico with distinct sizes and sociocultural contexts …

Ancillary cognitive demands impact children's behavior in common possibility reasoning tasks.

The ability to consider multiple, incompatible possibilities supports human decision making, but there is considerable variability in estimates of the age of emergence and development of this ability in young children. What accounts for this variability? We suggest that one major source is task-specific demands on developing cognitive processes that are not solely dedicated to possibility reasoning. In this article, we outline a set of ancillary cognitive demands in a variety of common tasks that are currently used to study young children's possibility reasoning (cups, gumball, slides, branchi

Women's Autonomy in Obstetric Care: A Qualitative Study on Violations and Experiences.

To explore how women experience and interpret autonomy in obstetric care across pregnancy, childbirth, abortion, and the postpartum period. A qualitative descriptive study was conducted using semi-structured interviews with women who had received obstetric-gynecological care. Interviews were audio-recorded, transcribed verbatim, and analyzed through hybrid thematic analysis. Participants described multiple constraints on autonomy associated with hierarchical professional relationships, limited information, and protocol-driven care practices. Autonomy emerged as a multidimensional and relationa

The Safety and Comfort of Home: Can Medical Deportation Alleviate Suffering?

AbstractMedical deportation, or the forced removal of severely injured and chronically ill, low-income, uninsured migrants to their countries of origin, remains a practice implemented by hospitals throughout the United States. This practice has been rightfully highly criticized by immigration advocates and human rights organizations. In 2019, the U.S. government eliminated a deferred action program allowing migrants without documentation to avoid deportation while they or their relatives were undergoing lifesaving medical treatment, forcing patients with highly complex medical needs to give up…
