Feasibility of supracricoid laryngectomy based on pathological examination.

Partial horizontal supracricoid laryngectomy with cricohyoidopexy at the Department of Otolaryngology, Medical University of Białystok, involves resection of the entire thyroid cartilage with the pre-epiglottic and paraglottic spaces. Experience has shown that this technique will permit adequate phonation, respiration and deglutition in selected advanced cases of supraglottic and glottic carcinoma. The purpose of this study was to verify histologically the indications for this surgery by examining sections of whole-organ laryngeal specimens. Post-laryngectomy specimens from 90 patients with ot

Maxillofacial surgery and obstructive sleep apnea: a review of 80 patients.

To determine the effectiveness of the surgical treatment of obstructive sleep apnea, we retrospectively evaluated 80 patients. One group consisted of 55 patients who had undergone a limited osteotomy of the anterior mandible: inferior sagittal osteotomy with hyoid myotomy and suspension. Important criteria for the selection of these patients were normal pulmonary function, normal mandibular skeletal development, and the absence of morbid obesity. Polysomnography revealed that 37 patients (67%) had responded to the surgical treatment and 18 patients (33%) had not. The second group of 25 patient

Sternohyoid myo-osseous flap for acquired subglottic stenosis in children.

Endotracheal intubation can occasionally result in acquired subglottic stenosis (ASGS) in infants and children. Twenty-one consecutive patients, ages 3 months to 13 years, with ASGS secondary to endotracheal intubation and severe enough to require tracheotomy, are reviewed. Of this population, eight cases of ASGS were refractory to endoscopic excision and dilatation. These eight patients, five of whom had complete stenosis, underwent subglottic reconstruction using a sternohyoid myo-osseus flap. Six of the eight patients had been successfully decannulated with no evidence of recurrent stenosis

Otitis media and interna and temporohyoid osteoarthropathy.

Otitis media/interna in the equine most commonly is a chronic, insidious infection with the unique sequella of temporohyoid osteoarthropathy in some horses. Head shaking may be the only clinical sign of the early stages of this disease. The arthritic condition often leads to fusion of the temporohyoid joint with resultant stress fractures of the petrous temporal bone. When this occurs, the horse presents as an acute neurologic case with clinical signs of vestibular and facial nerve dysfunction. Diagnosis, treatment, and prognosis are discussed. Recognition of this syndrome is important because

Influence of tonsil size on sagittal cephalometric measurements.

INTRODUCTION: The role of tonsils in the origin of sagittal skeletodental abnormalities has been widely discussed in the literature but remains controversial. Data on the probable relationship between enlarged tonsils and the presence of these abnormalities were subjective. The aim of this study was to quantify the relationship between the space occupied by the palatine tonsils and sagittal cephalometric measurements. MATERIALS AND METHODS: A cross-sectional study was performed on a group of children divided into 5 subgroups according to the standardized tonsillar hypertrophy grading scale. C

Anterior cervical pain syndromes.

The paucity of clinical findings in patients with glossopharyngeal neuralgia, superior laryngeal neuralgia, styloid process syndrome, hyoid syndrome, or carotidynia presents an enigma to the patient and the physician. Manifest symptoms appear extraneous or incongruous unless the essential element is identified. Common to all these syndromes is the radiation of pain over the neck and face, starting from the anterior cervical area of the neck. Case histories of seven patients are presented. The rationale of underlying pathophysiologic mechanisms is discussed and supported by relevant recent basi

Persistent thyroglossal duct with a retrohyoid cyst containing pearl-like structures revealed by cadaveric dissection.

The thyroglossal duct (TGD) is an embryonic fibrous tract that guides the descent of the thyroid gland from the tongue base to its definitive pretracheal location. Cadaveric studies reporting persistent TGD associated with cyst formation are rare. During routine dissection of the thyroid gland in an elderly female cadaver, we identified a persistent TGD arising from the left thyroid lobe and extending superiorly toward the left lateral border of the thyroid cartilage. A midsagittal section of the cervicocephalic region revealed superior extension of the TGD coursing posterior to the hyoid bone…

Hyomental distance in the different head positions and hyomental distance ratio in predicting difficult intubation.

The hyomental distance ratio (HMDR) is the ratio between the hyomental distance (HMD) (the distance between the hyoid bone and the tip of the chin) at the extreme of head extension (HMDe) and the one in the neutral position (HMDn). The objective of the study was to examine the predictive value, sensitivity, and specificity of HMDe, HMDn, and HMDR in predicting difficult endotracheal intubation (DI). A prospective study included 262 patients that underwent elective surgical operations. The following parameters were observed as possible predictors of DI: HMDR, HMDe, HMDn, Mallampati score, and b…

Postmortem computed tomography findings in suicide victims.

Suicide is the eighth cause of mortality in France and the leading cause in people aged between 25 and 34 years. The most common methods of suicide are hanging, self-poisoning with medicines and firearms. Postmortem computed tomography (CT) is a useful adjunct to autopsy to confirm suicide and exclude other causes of death. At autopsy, fractures of the hyoid bone or thyroid cartilage, or both, are found in more than 50% of suicidal hangings. Cervical vertebra fractures are rare and only seen in suicide victims jumping from a great height. Three-dimensional reconstructions from CT data are usef…

The use of computed tomography in determining developmental changes, anomalies, and trauma of the thyroid cartilage.

Recognition of injury to the hyoid bone and thyroid and cricoid cartilages is intrinsic to post-mortem examination. Due to its increasing brittleness with age the thyroid cartilage is particularly susceptible to injury following neck trauma, although there is inconsistency in the patterns of injury reported. In this study computed tomography scans of the head and neck of 431 deceased persons (235 males and 196 females) between the ages of 1 day and 100 years (mean age 35.93 ± 24.15), and including 25 victims of hangings, were examined to reveal the pattern of age-related change and the types o…
