[Vascular calcification in advanced secondary hyperparathyroidism].

Cardiovascular disease is one of the largest cause of mortality in maintenance hemodialysis patients. Vascular calcifications frequently encountered in hemodialysis patients. In advanced uremic secondary hyperparathyroidism model mice, vascular calcifications were observed in their aorta, by Kossa staining. This mouse model may be a useful model to study vascular calcification in maintenance hemodialysis patients.

[Surgical treatment of recurrent secondary hyperparathyroidism].

INTRODUCTION: Secondary renal hyperparathyroidism (SHPT) affects nearly all patients on regular hemodialysis. Between 5% and 10% will need surgery to control the disease. Despite a meticulous technique, the rate of recurrent/persistent hyperparathyroidism after surgery remains between 2% and 12%. PATIENTS AND METHOD: We reviewed the reoperations of 7 patients with recurrent or persistent SHPT from a series of 56 patients, 3 of which were referred from other units for treatment of recurrence. Subtotal parathyroidectomy (ST-PTX) was performed in 51 patients and total parathyroidectomy with auto…

Arterial stiffness in mild primary hyperparathyroidism.

When primary hyperparathyroidism was a more symptomatic disease, it was often associated with increased cardiovascular risk. As the clinical manifestations of the disease have changed to a milder, more asymptomatic disorder, investigation is shifting to more subtle cardiovascular abnormalities. We measured arterial stiffness in 39 patients with mild primary hyperparathyroidism [serum calcium, 2.66 +/- 0.2 mmol/liter (10.7 +/- 0.6 mg/dl); PTH, 21.7 +/- 9.5 pmol/liter (89 +/- 39 pg/ml)] and in 134 controls. Arterial stiffness was measured mathematically at the radial artery with a noninvasive de…

Unilateral neck exploration for primary hyperparathyroidism.

Focused unilateral cervical exploration is a controversial alternative to conventional bilateral neck exploration for primary hyperparathyroidism (HPT) due to solitary adenoma. Development of preoperative localization techniques, notably isotope scintigraphy and small-part, real-time ultrasonography, has increased preoperative parathyroid tumor identification. Critics of scan directed unilateral neck exploration argue it may overlook enlarged parathyroid glands on the unexplored side, increasing the incidence of persistent and recurrent hypercalcemia. Our experience of this operation and prolo…

Asymptomatic primary hyperparathyroidism: a surgical perspective.

Since the advent of multichannel autoanalyzers and routine screening of serum calcium levels, prevalence of primary hyperparathyroidism (pHPT) has increased to between 0.1% and 0.4%. As more patients present with "asymptomatic" pHPT, ideal treatment of "mild" disease becomes more controversial, with the possibility of safe, nonoperative management in a selected group of patients. Accumulated evidence confirms that the majority of these patients suffer from vague, nonspecific complaints that are very real and can improve following parathyroidectomy. Furthermore, parathyroidectomy in patients wi…

Not all patients presenting a primary hyperparathyroidism are candidates for this surgery. Contraindications are mainly due to a large goiter, previous surgery in the parathyroid vicinity, suspicious multiglandular disease and equivocal preoperative localising studies. Currently 60% of patients with primar …

Chloride/phosphate ratio in primary hyperparathyroidism.

The chloride/phosphate (Cl:PO4) ratio is known to help distinguish between the hypercalcemia of primary hyperparathyroidism (HPT) and hypercalcemia from other causes. The Cl:PO4 ratio of 106 patients with surgically proven primary HPT was compared with that of 126 normocalcemic healthy outpatients to examine its usefulness as a confirmatory test for primary HPT. The Cl:PO4 ratio was significantly higher in patients with HPT (42.5 +/- 7.0) compared with healthy controls (28.7 +/- 4.6). Patients with HPT and mild renal insufficiency (serum creatinine, 1.5-2.4 mg/dL) also showed a significant inc…

Primary hyperparathyroidism: controversies in surgical management.

Controversy surrounds the optimal management of patients with mild or asymptomatic primary hyperparathyroidism (PHPT). Although many new medical therapies for this disorder have emerged in the past decade, existing evidence indicates that parathyroidectomy is the only curative option and reverses not only many of the classic, but also the more subtle, manifestations of PHPT. Moreover, parathyroidectomy can be performed successfully with minimal morbidity by experienced surgeons. With the recent development and widespread use of several preoperative localization studies, various focused approac…

New operative strategies in primary hyperparathyroidism.

More than 95% of patients with primary hyperparathyroidism have been treated with bilateral neck exploration by experienced surgeons. This procedure has been performed without employing preoperative localization tests or specialized techniques of intraoperative measurement. A renewed interest in unilateral neck exploration for primary hyperparathyroidism emerged (in three developments), in an attempt to maintain the excellent cure rate and to minimize the invasiveness of the procedure. The first development was the introduction of sestamibi scintigrams as a new preoperative localization techni…

Primary hyperparathyroidism: lessons from bone histomorphometry.

The chronic excessive hypersecretion of parathyroid hormone (PTH) in primary hyperparathyroidism (PHPT) has significant impact on bone remodeling. Bone turnover increases by about 50%, leading to increased resorption at the endosteal envelope, increased cortical porosity, and thinning of cortical bone. In cancellous bone a different response is seen. Despite stimulation of bone resorption and formation at the tissue level, osteoclastic resorption and osteoblastic bone formation at individual bone multicellular units (BMUs) are reduced. This causes reduced erosion depth, reduced bone formation …

Vitamin D analogues for secondary hyperparathyroidism.

Secondary hyperparathyroidism (2HPT), a common disorder in patients with chronic renal failure, develops in response to phosphate retention and low serum 1,25-dihydroxyvitamin D(3) (1,25(OH)(2)D(3), calcitriol). Replacement therapy with calcitriol or its precursor 1alpha-hydroxyvitamin D(3) (1alphaOHD(3), alfacalcidol) often produces hypercalcaemia, especially when combined with calcium-based phosphate binders. In addition, these vitamin D compounds can aggravate the hyperphosphataemia in these patients. Several vitamin D analogues have been developed that retain the direct suppressive action …

Vitamin D deficiency and severe hyperparathyroidism.

The typical manifestations of severe hypercalcemia with osteitis fibrosa cystica have become exceedingly rare. We describe the case of a woman hospitalized for a tibial tumor with functional impotence, leading to a diagnosis of primary hyperparathyroidism (HPT I) associated with profound vitamin D deficiency. This 31-year-old woman was admitted, after two pregnancies complicated by the HELLP syndrome. Preoperative laboratory values were as follows: calcemia 4.05 mmol/l (2.2-2.6); urinary calcium 30 mmol/24 h (1.25-7.5); parathormone (PTH) 1 195 pg/ml (10-60); and 25 OH-vitamin D 13 nmol/l (22-…

Pancreatitis secondary to hyperparathyroidism during pregnancy.

BACKGROUND: It has been suggested that magnesium can be used to reduce serum calcium levels seen with hyperparathyroidism during pregnancy, thus reducing maternal and fetal risk. CASE: A young woman presented at 32 weeks' gestation with abdominal pain from pancreatitis caused by hyperparathyroidism from a parathyroid adenoma. She was started on magnesium sulfate tocolysis for preterm labor. During treatment, serum parathyroid hormone was undetectable, but serum calcium and vitamin D-1,25 were elevated. When magnesium was discontinued, her vitamin D-1,25 was suppressed and the parathyroid horm…

Coincident diabetes mellitus and primary hyperparathyroidism.

The prevalence of diabetes mellitus in primary hyperparathyroidism is approximately 8% and that of primary hyperparathyroidism in diabetic patients is approximately 1%. Both values are about three-fold higher than the respective expected prevalences in general populations. Patients with both disorders are over 40 years of age and 80% are female; 22% have type 1 and 78% type 2 diabetes. Primary hyperparathyroidism presents first in approximately 20% of patients, and diabetes mellitus in 40%; both disorders present together, or within 1 year, in 40%. Approximately 40% of patients with primary hy…

[Preoperative parathyroid scintigraphy in primary hyperparathyroidism].

BACKGROUND: The primary benefit of preoperative scintigraphy in patients with primary hyperparathyroidism not previously surgically explored seems to be the possibility of unilateral or limited access surgery in selected patients. MATERIAL AND METHODS: We studied the accuracy of scintigraphy using 99mTc sestamibi and dual phase imaging protocol combined with thyroid scintigraphy using 99mTc pertechnetate before conventional neck exploration and histologic verification in 78 consecutive patients. RESULTS: In 65 patients with solitary, focal uptake on scintigraphy, we found a solitary adenoma …

Non-surgical therapy of primary hyperparathyroidism.

BACKGROUND: Primary hyperparathyroidism is a commonly encountered illness that can usually be treated successfully with surgery. However, non-surgical ablation by either infusion of contrast media via angiography or percutaneous injection of ethanol has been reported from a few medical centers. METHODS: All patients undergoing non-surgical treatment of primary hyperthyroidism at the University of Oklahoma Health Sciences Center were identified and their records reviewed. RESULTS: We have used non-surgical techniques to treat three patients with primary hyperparathyroidism. The patients were …
