INTRODUCTION: Hyperphosphatemia is frequent in hemodialysis (HD) patients and associates with cardiovascular mortality. Parameters of online hemodiafiltration (OL-HDF) prescription influencing phosphate removal are poorly understood in real-world clinical settings.
METHODS: We conducted a prospective observational study including chronic HD patients dialyzed at a single UK university hospital between December 2021 and July 2023. Phosphate removal was quantified from spent dialysate during a midweek session.
RESULTS: Dialysate phosphate was measured during 161 dialysis sessions in 128 unique …

CASE: A 13-year-old boy presented with a painful limp for 2 weeks without trauma and guarded (L) hip movements with spasm. Radiographs revealed a valgus slipped capital femoral epiphysis (SCFE) of the left hip with generalized osteosclerosis. Elevated serum calcium, phosphate, and parathyroid hormone levels were noted. Whole-exome sequencing identified a homozygous FGF23 variant, confirming hyperphosphatemic tumoral calcinosis. He was managed with phosphate binders, dietary restriction, and in situ fixation of both hips. At the 2-year follow-up, alignment and function were normal.
CONCLUSION:

PURPOSE: Medication use may affect imaging results. In this case study, we report a case of lanthanum ingestion resulting in imaging consistent with ingested metallic foreign bodies.
SUMMARY: Hyperphosphatemia affects most patients with end-stage renal disease (ESRD) and is associated with morbidity and mortality. Lanthanum carbonate reduces daily phosphate absorption and is indicated as a non-calcium-based phosphate binder in patients with ESRD. A 58-year-old man with a medical history of stage 5 chronic kidney disease was admitted to the intensive care unit (ICU) for hyperkalemia and acute …

A 49-year-old woman developed eosinophilic peritonitis 2 months after starting continuous ambulatory peritoneal dialysis because of congenital right kidney hypoplasia and chronic glomerulonephritis. This was shown to have been induced by sucroferric oxyhydroxide, an iron-based phosphate binder, using a drug-induced lymphocyte stimulation test. Her eosinophilic peritonitis was improved after stopping the administration of sucroferric oxyhydroxide without providing any immunosuppressive agents.

BACKGROUND: Although most hypocalcemia with hypomagenesemia in the neonatal period is due to transient neonatal hypoparathyroidism, magnesium channel defects should also be considered.
CASE: We report a case of persistent hypomagnesemia in an 8-day-old Hispanic male who presented with generalized seizures. He was initially found to have hypomagnesemia, hypocalcemia, hyperphosphatemia and normal parathyroid hormone. Serum calcium normalized with administration of calcitriol and calcium carbonate. Serum magnesium improved with oral magnesium sulfate. However, 1 week after magnesium was disconti…

INTRODUCTION: There is high prevalence of calcium supplementation in the general population and some recent data suggest that this may increase the risk of vascular calcification. Calcium-based binders have been a standard treatment for hyperphosphatemia in patients with chronic kidney disease (CKD). Though as effective as phosphate binders, they provide a source of substantial calcium intake.
AREAS COVERED: In addition to the balance studies recently completed to assess the implications of calcium loading in CKD, we also review observational studies and clinical trials involving calcium-base…

INTRODUCTION: Disturbances of the bone and mineral metabolism are a common complication of chronic kidney disease (CKD); these disturbances are known as CKD-mineral bone disorder (CKD-MBD). A better understanding of the pathophysiological mechanisms of CKD-MBD, along with its negative impact on other organs and systems, as well as on survival, has led to a shift in the treatment paradigm of this disorder. The use of phosphate binders changed dramatically over the last decade when noncalcium-containing phosphate binders, such as sevelamer and lanthanum carbonate, became possible alternative tre

OBJECTIVES: Hypoparathyroidism is a rare disorder which is predominantly of idiopathic or genetic origin in children. The diagnosis is made from the biochemical measurement of parathyroid hormone (PTH), and the key findings include a low PTH in combination with hypocalcemia and hyperphosphatemia. However, the level of PTH encountered in patients with hypoparathyroidism may be dependent on the underlying genetic cause of the disorder as well as the biochemical assay used for assessment of PTH.
CASE PRESENTATION: A three-year-old child with asymptomatic primary hypoparathyroidism was identified…

We present an unusual case of SHOX deficiency associated with Léri-Weill dyschondrosteosis (LWD), Hashimoto's thyroiditis and pseudohypoparathyroidism 1B in a young woman. To our knowledge, this is the first ever report of these disorders coexisting. At the age of nine years, the proband was diagnosed of hypothyroidism due to Hashimoto's thyroiditis, and developed biochemical abnormalities consistent with hyperphosphatemia, mild hypocalcemia and elevated parathyroid hormone without any clinical symptoms except short stature. Replacement therapy with levothyroxine, calcium and alphacalcidol was…

The prevention and correction of hyperphosphatemia are major goals of the treatment of chronic kidney disease (CKD)-bone mineral disorders, and thus, Pi balance requires special attention. Pi balance is maintained by intestinal absorption, renal excretion, and bone accretion. The kidney is mainly responsible for the plasma Pi concentration. In CKD, reduced glomerular filtration rate leads to various Pi metabolism abnormalities, and Pi absorption in the small intestine also has an important role in Pi metabolism. Disturbances in Pi metabolism are mediated by a series of complex changes in regul…

UNLABELLED: INTRODUCTION Cardiovascular disease is a leading cause of death among hemodialysis patients.
SECONDARY HYPERPARATHYROIDISM: Secondary hyperparathyroidism is one of the main factors for the development of cardiovascular complications among these patients. A high concentration of parathormone, hypercalcemia, hyperphosphatemia and high calcium x phosphate product among dialysis patients play a crucial role in the development of vascular (calcification of coronary artery) and valvular calcifications. DISCUSSION AND CONCLUSION With every new patient on hemodialysis it is necessary to s…

Secondary hyperparathyroidism (SHPT) is a classical feature of chronic kidney disease (CKD). Commonly, hypocalcemia, hyperphosphatemia, and vitamin D deficiency are involved into the pathogenesis of SHPT. Parathyroid (PT) glands are characterized by a low turnover and rarely undergo mitoses. However, in the presence of low calcium, high phosphorus, vitamin D deficiency, and uremia, PT cells leave quiescence. In the last decade, both new molecular and cellular mechanisms have been investigated in the pathophysiology of SHPT, between them the emerging role of the PT vitamin D receptor and calciu…
