Impact of infertility and infertility treatments on quality of life and levels of anxiety and depression in women undergoing in vitro fertilization.

This study aims to evaluate levels of anxiety and depression in women, correlated with infertility per se and with infertility treatments, highlighting predictors of higher levels of distress. Two validated standardized questionnaires, the Hospital Anxiety and Depression Scale (HADS) and the Fertility Quality of Life (FertiQoL), were administered to 89 women both before their first cycle of infertility treatment and again at the end of the ovarian stimulation for in vitro fertilization (IVF). Women's levels of anxiety were significantly higher before the treatment than during the treatment its…

Scanning electron microscopic study on the shape of infertile seminiferous tubules: a hypothesis of pathogenesis of idiopathic male infertility.

The shape of infertile seminiferous tubules was studied on testicular biopsied specimens from patients with idiopathic male infertility using the scanning electron microscope. A parallel study of the same sections was performed by ordinary light microscopy. Normal seminiferous tubules were revealed to be shaped like a weaving cylinder, whereas infertile seminiferous tubules were strangulated in various places, or appeared similar to a tail-like rope which became gradually more slender. These strangulated or slender parts of tubules seemed to correspond to the smaller tubules with thickened tub…

Timing of puberty and infertility in women and men.

OBJECTIVE: To study the association between puberty and infertility. STUDY DESIGN: The study population consisted of 3,303 women and 2,522 men born into the Danish Healthy Habits for Two cohort (1984-1987). In 2005, the participants reported age at menarche and voice break. In the main analysis, pubertal timing was categorized into three groups of approximately equal size. Women were categorized as having earlier (<13 years), average (13 years) or later (>13 years) age at menarche (mean: 13.3 years), and men as having earlier (<14 years), average (14 years) or later (>14 years) age at voice b…

Influence of metabolic syndrome on infertility: A review.

BACKGROUND: Metabolic Syndrome (MetS) is a cluster of metabolic abnormalities including abdominal obesity, impaired glucose metabolism, hypertriglyceridemia, low high-density lipoprotein (HDL) cholesterol, and hypertension. It is diagnosed when at least three of these five criteria are present. Individuals with MetS are at increased risk of developing type 2 diabetes mellitus and cardiovascular disease. Beyond these well-established complications, infertility has emerged as an underrecognized consequence of metabolic dysfunction. Infertility, defined as failure to achieve pregnancy after one y…

Isotretinoin for the treatment of severe male infertility.

PURPOSE OF REVIEW: To achieve pregnancy, men with azoospermia must currently undergo a testicular procedure to retrieve sperm for intracytoplasmic sperm injection (ICSI). However, sperm are successfully retrieved only ~50% of the time. Recently, isotretinoin administration has been investigated as a means of increasing sperm production in azoospermic men. This review explains the rationale for isotretinoin use and will update readers on the results from early studies of isotretinoin treatment of male infertility. RECENT FINDINGS: Pilot studies suggest that isotretinoin treatment for 3-6 month…

The biology and clinical aspects of female infertility.

Female infertility is a multifactorial condition with complex biological and clinical underpinnings. Biologically, female-related infertility may stem from disruptions in the hypothalamic-pituitary-ovarian (HPO) axis, impaired folliculogenesis, oocyte maturation defects, uterine abnormalities, endometrial dysfunction, and fallopian tube abnormalities. This review highlights key genetic mechanisms contributing to reproductive dysfunction and their relevance to diagnosis and treatment. Chromosomal abnormalities, including Turner syndrome and X-autosome translocations, also contribute to infertil…

Ovarian fibrosis as a common mechanism of infertility.

Female fertility has long been explained through observation of the ovarian follicle: the number of residing follicles and the quality of th … ytes and embryos in women with premature ovarian insufficiency. While the current focus in standard infertility treatment aims to improve follicle and hormone health, fibrosis may be reversible and preventable, making it a candidate therapeutic target that could complement existing treatments. However, key gaps remain: no standardized measure of ovarian fibro …

A case of infertility diagnosed with tuberculous endometritis.

A 30-year-old woman presented with infertility and irregular menstruation. Physical examination revealed obesity and slightly pale conjunctivae. Transvaginal ultrasound revealed polycystic ovary, bilateral hydrosalpinx, and endometrial thickening. Magnetic resonance imaging of the pelvis showed several large cystic lesions and elongated fallopian tubes. An endometrial biopsy was performed to investigate the endometrial thickening. Pathological findings included small non-caseating epithelioid cell granulomas with scattered plasma cell infiltration. The microbiological analysis of the endometri…

The genetic basis of male and female infertility.

This review provides a comprehensive overview of the genetic factors underlying male and female infertility. Infertility affects an estimated one in six couples worldwide, with both male and female factors contributing equally to its prevalence. Approximately, 50% of infertility cases are attributed to genetic causes. We explore three main categories of genetic causes: chromosomal abnormalities, monogenic disorders, and syndromic conditions. Chromosomal causes, including numerical and structural aberrations, are discussed with a focus on their impact on gametogenesis and reproductive outcomes.…

Advances in the genetic etiology of female infertility.

Human reproduction is a complex process involving gamete maturation, fertilization, embryo cleavage and development, blastocyst formation, implantation, and live birth. If any of these processes are abnormal or arrest, reproductive failure will occur. Infertility is a state of reproductive dysfunction caused by various factors. Advances in molecular genetics, including cell and molecular genetics, and high-throughput sequencing technologies, have found that genetic factors are important causes of infertility. Genetic variants have been identified in infertile women or men and can cause gamete …

Efferocytosis and infertility: Implications for diagnosis and therapy.

Recent research has shed light on the intricate connection between efferocytosis and infertility, revealing its dysregulation as a contributing factor in various reproductive diseases. Despite the multifaceted nature of infertility etiology, the impact of insufficient clearance of apoptotic cells on fertility has emerged as a focal point. Notably, the removal of apoptotic cells through phagocytosis in the female reproductive system has been a subject of extensive investigation in the field of infertility. Additionally, special functions performed by immune system cell types, such as macrophage…

Primer on Female Infertility for the Reproductive Urologist.

This review is intended to serve as an aid in decision-making and patient counseling for the reproductive urologist when female factor infertility is found concurrently with male factor infertility. This review pairs the pathophysiology of female infertility with its implications for the treatment of male infertility, which most commonly includes ovulatory disorders, tubal abnormalities, and uterine abnormalities. By gaining a deeper understanding of these factors, reproductive urologists can employ a tailored approach to managing male factor infertility, taking into account the female partner…

The management of infertility for primary care physicians.

To conduct this review of primary care, we looked for related papers in PubMed from the last 15 years. WHO's initial concept of Health defined a condition of physical, mental, and social well-being, nowadays extended to quality of life. Infertility or not being able to form a family fits perfectly into the definition. Primary care is responsible for mandatory discussions about fertility as part of a broader aspect regarding reproductive health issues. Having children is a decision taken by heterosexual couples, same sex couples, or single individuals. Understanding factors associated with infe…
