Patients with cirrhosis are at increased risk of developing infections due to bacterial translocation. This process depends on three principal factors: bacterial overgrowth, immunodepression, and altered intestinal permeability. Intestinal barrier functions may be disturbed in cirrhosis, related to the toxic effects of alcohol (on mucosa and biological membranes) and portal hypertensive enteropathy. Few studies on the assessment of intestinal permeability in cirrhotic patients are available, and contradictory results may be explained by methodological differences. However, four studies using a

Nonsteroidal anti-inflammatory drugs (NSAIDs) are capable of damaging the whole gastrointestinal tract. Small and large intestinal injuries manifest as acute changes in permeability with endoscopic erosions, chronic erosions and ulcers, diaphragms in the small bowel, and an increase in small and large bowel complications including perforation and diverticular bleeding. It is quite likely, though not proven, that such lesions contribute to anemia in patients taking them. A growing body of data shows that selective inhibitors of the cyclooxygenase-2 enzyme have much reduced toxicity in this resp

BACKGROUND: Intestinal neoplasia of horses is inadequately described.
HYPOTHESIS: Intestinal neoplasia of horses has characteristic clinicopathologic features.
ANIMALS: Thirty-four horses with intestinal neoplasia.
METHODS: Retrospective study.
RESULTS: Anamnesis, clinical signs, clinicopathologic and pathologic findings in 34 adult horses diagnosed histologically with intestinal neoplasia were reviewed. The horses ranged in age from 2 to 30 years (mean 16.6 years at presentation). The Arabian breed was most represented and there was no sex predisposition. The most common presenting compla

PURPOSE: The aim of this work is to discuss the pathogenesis of the segmental dilatation of the intestine (SDI) and to review its clinical presentation and the ways to confirm the diagnosis.
METHODS: Eight cases of pathologically proven SDI from 1987 to 2003 were reviewed and discussed. There were 7 newborns and a 1-year-old boy.
RESULTS: Our patients are 5 boys and 3 girls. In all cases, the diagnosis was not suspected before surgery. Two patients presented with a low neonatal bowel obstruction. Six patients were operated for omphalocele, which was the most frequent associated malformation.

PURPOSE OF REVIEW: As outcomes with intestinal transplantation continue to improve, its role in the management of intestinal failure patients becomes clearer. Some intestinal failure patients do well with long-term total parenteral nutrition (TPN), while others develop life-threatening complications and need to be evaluated for intestinal transplants (ITs). The purpose of this review is to update the current status of intestinal transplantation and its role in the management of intestinal failure patients.
RECENT FINDINGS: Current outcomes indicate that intestinal transplants should be consid

Diagnostic techniques investigating intestinal functions are a main component of gastroenterological diagnostic procedures and are mainly indicated if intestinal motility disturbances or malabsorption syndromes are suspected. H2 breath tests using lactulose or lactitol as substrates measure orocecal transit time. Intestinal manometry allows more detailed analysis of intestinal motility disturbances but is only available at specialized centres. A generalized malabsorption syndrome can be diagnosed by D-xylose test. If it is caused by bacterial overgrowth, the H2 breath test with glucose may be

We present a case of reactive angioendotheliomatosis (RAE) of the colon, featuring intravascular proliferation of endothelial cells with histologic resemblance to glomeruloid hemangioma. A 19-year-old Japanese male with an anal fistula was diagnosed endoscopically with Crohn's disease. Six months later, he was hospitalized for fever and abdominal pain. Emergency resection of ileocecum and splenic flexure of the colon was undertaken to control massive intestinal hemorrhage, and in all parts of the resected colon, foci of many small vessels with intravascular proliferation of endothelial cells w

Intestinal failure (IF) can be defined as the reduction of functional gut mass below the minimal amount necessary for digestion and absorption adequate to satisfy the nutrient and fluid requirements for maintenance in adults or growth in children. In developed countries, IF mainly includes individuals with the congenital or early onset of conditions requiring protracted or indefinite parenteral nutrition (PN). Short bowel syndrome was the first commonly recognized cause of protracted IF. The normal physiologic process of intestinal adaptation after extensive resection usually allows for recove

Intestinal parasites cause significant morbidity and mortality. Diseases caused by Enterobius vermicularis, Giardia lamblia, Ancylostoma duodenale, Necator americanus, and Entamoeba histolytica occur in the United States. E. vermicularis, or pinworm, causes irritation and sleep disturbances. Diagnosis can be made using the "cellophane tape test." Treatment includes mebendazole and household sanitation. Giardia causes nausea, vomiting, malabsorption, diarrhea, and weight loss. Stool ova and parasite studies are diagnostic. Treatment includes metronidazole. Sewage treatment, proper handwashing,

Intestinal lymphomas are almost exclusively non-Hodgkin lymphomas (NHL) which occur at a slightly lower frequency than those arising in the stomach. Intestinal NHLs differ from nodal lymphomas because they retain some properties of the mucosa-associated lymphoid tissue (MALT) from which they arise. The recently proposed WHO classification of lymphoid neoplasias encompasses and defines all the NHLs occurring in the gastrointestinal tract as lymphoma entities. The histopathological diagnosis relies on the combination of morphology and immunohistochemistry, which represents the gold standard. By

Some types of intestinal duplication are an infrequent clinical condition in the gastroenterology tract that do not meet all classic requisites for their definition. We present a case of independent intestinal duplication from small intestine that starts with acute abdomen; cystic, perforated and separated tumor from wall of normal intestine was founded during surgery. It was totally resected with preservation of normal intestine adjacent, because it was an independent vascular supply. We review a new vascular classification of intestinal duplication and their importance in surgical treatment

Small intestinal neoplasms are uncommonly encountered in clinical practice. They may occur sporadically, in association with genetic diseases (e.g., familial adenomatous polyposis coli or Peutz-Jeghers syndrome), or in association with chronic intestinal inflammatory disorders (e.g., Crohn's disease or celiac sprue). Benign small intestinal tumors (e.g., leiomyoma, lipoma, hamartoma, or desmoid tumor) usually are asymptomatic but may present with intussusception. Primary malignancies of the small intestine-including adenocarcinoma, leiomyosarcoma, carcinoid, and lymphoma-may present with intes

The objective of the present study was to assess intestinal permeability in patients with infection caused by Strongyloides stercoralis. Twenty-six patients (16 women and 10 men), mean age 45.9, with a diagnosis of strongyloidiasis were evaluated. For comparison, 25 healthy volunteers (18 women and 7 men), mean age 44.9, without digestive disorders or intestinal parasites served as normal controls. Intestinal permeability was measured on the basis of urinary radioactivity levels during the 24 h following oral administration of chromium-labeled ethylenediaminetetraacetic acid ((51)Cr-EDTA) expr

Intestinal protozoal infections cause significant disease not only in the tropics but also in immunocompromised hosts and returning travellers in the developed world. Precise diagnosis of protozoal intestinal infection by microscopy can be difficult. Enzyme immunoassays for antigen detection are being used for some protozoal infections with some limitations which will, it is hoped, be overcome by molecular techniques. Nucleic acid amplification techniques could help improve detection of microsporidial species, which are difficult to detect microscopically and allow differentiation between Enta
