Value of nasogastric tube after colorectal surgery.

To evaluate the effect of tube decompression of the stomach after elective colorectal operations, 97 patients were randomly allocated to postoperative treatment with or without nasogastric tubes. Flatus passed earlier in the patient group without tubes. However, no significant differences were found between the two groups with respect to duration and severity of postoperative paralysis, as measured by occurrence and duration of nausea and vomiting, postoperative peroral fluid intake and time for defecation. The frequencies of postoperative complications were small in both groups and without an

The effect of posture on the emptying of the intrathoracic vagotomized stomach.

Ten patients were studied at periods ranging from 6 months to 25 years after oesophagogastrostomy and gastric drainage. Gastric emptying studies and overnight gastric aspirates in both the supine and erect positions were carried out. Evidence was found in these patients of increased duodenogastric reflux and poor gastric emptying in the supine posture. All of the patients had gastritis, and some had gastric mucosal ulceration. These findings have led to the conclusion that in the supine posture the combination of duodenogastric reflux and poor gastric emptying leads to gastric mucosal damage.

Deep sedation using propofol target-controlled infusion for gastrointestinal endoscopic procedures: a retrospective cohort study.

BACKGROUND: Propofol sedation is effective for gastrointestinal endoscopic procedures, but its narrow therapeutic window highlights the importance of identifying an optimal administration technique regarding effectiveness and safety. This study aimed to determine the incidence of significant adverse events in adult patients scheduled for gastrointestinal endoscopy under anaesthetist-performed sedation using propofol target-controlled infusion and determine the existence of associations between these events and potentially related variables. METHODS: This single-centre, retrospective cohort st

Ultrasound detection of the sliding lung sign by prehospital critical care providers.

BACKGROUND: The presence of the sonographic sliding lung sign (SLS) is a sensitive indicator for the absence of a pneumothorax. OBJECTIVE: The aim of this study was to determine if prehospital critical care providers (PHCPs) can acquire and maintain the necessary skills to determine the presence or absence of the SLS following a brief tutorial. METHODS: This was a blinded randomized observational trial using a cadaveric model. The model was randomized to esophageal intubation (negative SLS) or tracheal intubation (positive SLS), and a SonoSite 180 plus (Bothel, WA) machine was used for the e

Size 2 ProSeal laryngeal mask airway: a randomized, crossover investigation with the standard laryngeal mask airway in paediatric patients.

BACKGROUND: One of the main concerns with the use of the standard laryngeal mask airway (SLMA) in small infants is that its low-pressure seal might be inadequate for positive pressure ventilation so that there is a risk of gas leakage into the stomach with the subsequent risk of regurgitation. The new ProSeal LMA (PLMA) has been shown to form a more effective seal than the SLMA and to facilitate gastric tube placement in adults. The first paediatric size PLMA became available recently. METHODS: Thirty anaesthetized, non-paralysed children aged 46 (19) months, weighing 16 (10-21) kg, were stud

Aortoesophageal fistula and double aortic arch: two important points in management.

Two children with double aortic arch and aortoesophageal fistula (AEF) are reported to warn of this lethal complication of double aortic arch and to stress important points in the diagnosis and management. A review of the records of 30 children with double aortic arch disclosed two patients who had AEF. The first patient had respiratory distress and repair of a vascular ring (double aortic arch) at 5 weeks of age. At 9 weeks of age, because of difficulty with tracheal extubation, aortopexy was performed. Ten days later, profuse upper gastrointestinal bleeding required control by a Sengstaken-B

Does this patient have a severe upper gastrointestinal bleed?

CONTEXT: Emergency physicians must determine both the location and the severity of acute gastrointestinal bleeding (GIB) to optimize the diagnostic and therapeutic approaches. OBJECTIVES: To identify the historical features, symptoms, signs, bedside maneuvers, and basic laboratory test results that distinguish acute upper GIB (UGIB) from acute lower GIB (LGIB) and to risk stratify those patients with a UGIB least likely to have severe bleeding that necessitates an urgent intervention. DATA SOURCES: A structured search of MEDLINE (1966-September 2011) and reference lists from retrieved articl

Chloroform ingestion causing severe gastrointestinal injury, hepatotoxicity and dermatitis confirmed with plasma chloroform concentrations.

CONTEXT: Poisoning due to chloroform ingestion is rare. The classic features of acute chloroform toxicity include central nervous system (CNS) and respiratory depression, and delayed hepatotoxicity. CASE DETAILS: A 30-year-old female ingested 20-30 mL of 99% chloroform solution, which caused rapid loss of consciousness, transient hypotension and severe respiratory depression requiring endotracheal intubation and ventilation. She was alert by 12 h and extubated 16 h post-overdose. At 38-h post-ingestion, her liver function tests started to rise and she was commenced on intravenous acetylcystei

Patient experience and resource use analysis of balloon-retained vs. disc-retained gastro-jejunal tubes in children.

PURPOSE: Building on our previous work demonstrating that balloon-retained gastro-jejunostomy tubes (BGJ) avoid complications unique to disc-retained devices but require more frequent replacement, we evaluated caregiver experience and healthcare resource use to better inform tube selection in children. METHODS: Families whose children had experienced both Freka® percutaneous endoscopic gastro-jejunostomy (FPEGJ) and BGJ completed a paired questionnaire assessing nine experience domains (0-5 Likert-type scales) with free-text comments. Scores were compared using the Wilcoxon signed-rank test a

Gastrostomy plus fundoplication or gastro-jejunal tube or gastrostomy alone for treating or preventing gastro-esophageal reflux in children and adolescents with neurological impairment.

RATIONALE: Children with neurological impairment (NI) represent a vulnerable pediatric population who often experience multiple co-occurring chronic conditions and may develop worsening comorbidities over time due to primary dysfunctions of the nervous system. Among the most significant of these are gastro-esophageal reflux (GER) and dysphagia, which likely account for much of the associated morbidity and mortality. There is substantial variation in the management of GER, and few studies have been conducted to evaluate the effects of antireflux procedures in preventing and improving outcomes i

Cortrak feeding tube safety: Criteria for interpreting lung misplacement.

BACKGROUND: Pneumothorax occurs in 0.52% of blind tube placements, with 97% occurring in-procedure. Post-procedure pH or x-ray checks cannot prevent these, but CO2 checks or guided tube placement can. Cortrak guided tube placement is widespread, but manufacturer guidance to interpret lung placement is subjective. AIM: Develop objective criteria to differentiate lung from oesophageal tube placement from measurements and patterns in Cortrak traces. STUDY DESIGN: Paired comparison of lung and oesophageal Cortrak traces using a retrospective analysis of prospectively collected data in critically

Does Non-Beneficial Nasogastric Tube Feeding Occur During End-of-Life Care? An Audit of Outcomes for Those with a Malnutrition Universal Screening Tool Score of 2.

Background: The consideration of artificial nutrition and hydration (ANH) is recommended for individuals with reduced nutritional intake. However, placing long-term nonoral feeding tubes is not appropriate in those with advanced dementia or if the individual is likely to die imminently-"within hours or days" (GMC, 2010;58). In some instances, the provision of ANH in the weeks leading up to death may be considered a "Non-Beneficial Treatment." Objective: To determine the extent to which "non-beneficial" nasogastric tube (NGT) feeding occurs at the end of life. Design: An audit of outcomes for i

A Randomized Clinical Trial to Compare Three Different Methods for Estimating Orogastric Tube Insertion Length in Newborns: A Single-Center Experience in China.

Background: Orogastric (OG) and nasogastric (NG) tubes are frequently used in the NICU. Obtaining a relatively accurate estimated length before insertion could significantly reduce complications. While previous studies have mainly focused on the NG tube, OG tubes are more commonly used in China. Purpose: The objective was to determine whether there were differences in the rate of accurate placement among the adapted nose-ear-xiphoid (NEX) method, nose-ear-midway to the umbilicus (NEMU) method, and weight-based (WB) equation in estimating the OG tube insertion distance. Methods: A randomized, c

Factors Associated With Gastrostomy Tube Complications in Infants With Congenital Heart Disease.

INTRODUCTION: Children with congenital heart disease (CHD) often experience feeding intolerance due to aspiration, inability to tolerate feed volume, or reflux within the first few months of life, requiring a surgically placed gastrostomy tube (GT) for durable enteral access. However, complications such as GT dislodgement, cellulitis, and leakage related to GT use are common. GT-related complications can lead to unscheduled pediatric surgery clinic or emergency room (ER) visits, which can be time consuming for the family and increase overall healthcare costs. We sought to identify factors asso
