PHARYNGO-ILEO-COLO-ANASTOMOSIS WITH MICRO-VASCULAR BLOOD SUPPLY AUGMENTATION FOR ESOPHAGO-GASTRIC REPLACEMENT DUE TO ESOPHAGO-GASTRIC NECROSIS AFTER CAUSTIC INGESTION.

BACKGROUND: Complete esophago-gastric necrosis after caustic ingestion is a challenging surgical scenario for reconstruction of the upper di … ed initially total esophago-gastrectomy, closure at the level of the crico-pharyngeal sphincter and jejunostomy for enteral feeding with a highly deteriorated quality of life . The procedure was performed later and there were no major early and late postoperative complications and normal nutritional conditions were re-stablished. CONCLUSION: The procedure is …

[Type IV paraesophageal hernia with 60% of gastric necrosis. Case report].

BACKGROUND: Paraesophageal hernias are rare and, when associated with symptoms, the risk of complications increases, becoming a surgical eme … rect gastrectomy dehiscence and ending with a third procedure for cervical esophagostomy and Witzel jejunostomy. CONCLUSIONS: Elective repair is recommended in all patients with asymptomatic paraesophageal hernia in order to avoid possible complications. The approach method is dependent on the surgeon's experience and the conditions of the hernia and involve …

Preliminary report of a new technique for temporary faecal diversion after extraperitoneal colorectal anastomosis.

AIM: Loop ileostomy is a suitable procedure for transitory faecal diversion after colorectal or coloanal anastomosis. We describe here an ea … osis. RESULTS: Radiological control carried out on day 11 evidenced a premature dislocation of the jejunostomy tube in 1 patient, thus the tube was correctly removed without any complications. In another patient a delayed closure of the ileo cutaneous fistula was recorded that required simple medication over 15 days in the out patient clinic. No signs of ana …

Successful living related liver transplantation in a case with biliary atresia associated with corrected transposition of the great arteries.

Little is known about the safety of LRLTx in children with end stage liver disease associated with congenital cardiac anomalies. We report t … on of the portal vein. However, a sudden decrease in blood oxygenation was observed during hepatico-jejunostomy, which was easily normalized by graft mobilization. Post-operatively, neither heart failure nor cerebral infarction because of air embolism was observed. In conclusion, together with preserved cardiac function and carbon dioxide insufflation, LRLTx …

Laparoscopic transhiatal esophago-gastrectomy after corrosive injury.

Esophago-gastric necrosis is a surgical emergency associated with high morbidity and mortality. We report a laparoscopic transhiatal esophag … he procedure finished with an esophagostomy on the left side of the neck and a laparoscopic feeding jejunostomy (video). Total operative time was 235 minutes. After six months a digestive reconstruction with esophagocoloplasty by laparotomy and cervicotomy was easily realized thanks to the advantages (few adhesions, bloodless, and simple colic mobilization) o …

The Roux stasis syndrome. Treatment by pacing and prevention by use of an 'uncut' Roux limb.

Ectopic pacemakers in the Roux limb are associated with delayed gastric emptying after Roux gastrectomy. The aim herein was to suppress the ectopic pacemakers by electrical pacing or to prevent them by maintaining enteric myoneural continuity with an "uncut" Roux limb, and so improve the delayed emptying. Among eight dogs with truncal vagotomy and Roux hemigastrectomy, four dogs had a pacing electrode applied to the proximal end of the Roux limb. The other four dogs had a gastrojejunostomy to an uncut Roux limb. In them, the afferent jejunal limb was occluded by staples but not divided, and a …

Jejunal conduits: technique and complications.

Thirty patients underwent jejunal urinary diversion: 27 bilateral cutaneous ureterojejunostomies, 2 cutaneous pyeloureterojejunostomies and 1 bilateral pyelocutaneous jejunostomy. In the majority of the cases this high diversion was indicated for malignant disease with preoperative and postoperative irradiation of the pelvis. Postoperative morbidity in these cases is not different from thatin cases of ileal conduit operation, except for a high incidence of reversible hypochloremic acidosis with hyponatremia, hyperkalemia and uremia. This electrolytic syndrome is the consequence of a continuous…

Robot-assisted resection of choledochal cyst in neonates.

OBJECTIVE: Laparoscopic choledochectomy and hepatojejunostomy have been reported in children since 1995, but this procedure is technically demanding. Robotic surgical systems can simplify complex minimally invasive procedures. Currently, few reports have been made on neonates. We present the experience of 6 cases of neonatal CC(choledochal cysts). STUDY DESIGN: Between January 2022 and December 2023, 6 neonates underwent robotic resection of choledochal cyst and hepaticojejunostomy using the Da Vinci surgical system at Children's Hospital, Zhejiang University School of Medicine, a high-volume…

Further Development of Magnetic Compression for Gastrojejunostomy in Rabbits.

BACKGROUND: Technical improvement of gastrojejunostomy is critical in bariatric and metabolic surgery. In this study, a novel magnetic compression approach for gastrojejunostomy was evaluated. MATERIALS AND METHODS: Both cylindrical and rectangular magnets were used in rabbits, and the magnets were named according to their location. All the magnets were perorally introduced into the stomach. The position of the jejunal magnet was controlled by a connecting line. When the jejunal magnet spontaneously entered the jejunum, the gastric magnet was introduced into the stomach. An extracorporeal mag…

Vascular coil erosion into hepaticojejunostomy following hepatic arterial embolisation.

BACKGROUND: Right hepatic arterial injury (RHAI) is the most common vascular injury sustained during laparoscopic cholecystectomy, occurring in up to 7% of cholecystectomies. RHAI is also the most common vascular injury associated with a bile duct injury (BDI) and is reported to occur in up to 41 - 61% of cases when routine angiography is employed following a BDI. We present an unusual case of erosion of vascular coils from a previously embolised right hepatic artery into bilio-enteric anastomoses causing biliary obstruction. This is on a background of biliary reconstruction following a major

Use of vacuum-assisted closure device in a disastrous form of abdominal sepsis and stoma site infection: systematic review and report of a case.

Use of Vacuum-Assisted Closure (VAC) for treatment of open abdomen has been established predominantly in cases of severe abdominal trauma, resulting with high percentage of primary fascial closure. The role of VAC technique in cases of severe diffuse peritonitis is not definitely incorrigible. However, in cases of severe complicated abdominal sepsis VAC come up as a last resort.
