FDG PET/CT in a Case of Isolated Drain Site Metastasis After Laparotomy Surgery for Rectal Cancer.

Isolated abdominal-wall recurrence in the drainage tube site after surgery for colorectal cancer is extremely rare. We describe contrast-enhanced CT and FDG PET/CT findings in a case of isolated abdominal-wall metastasis in the drainage tube site after laparotomy surgery for rectal cancer. The abdominal-wall metastasis showed focal hyperenhancement on contrast-enhanced CT and increased activity on FDG PET/CT.

Prophylactic Mesh Reinforcement Versus Primary Suture for Midline Laparotomy Closure: An Updated Systematic Review and Meta-Analysis.

Incisional hernia remains a common postoperative complication following elective midline laparotomy. Prophylactic mesh reinforcement has been proposed to reduce hernia formation, although concerns regarding postoperative complications and variation in operative technique have limited routine adoption. This systematic review and meta-analysis of randomized controlled trials (RCTs) was conducted according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines and registered on PROSPERO (CRD420251172749). A comprehensive search identified RCTs comparing pro…

Effect of closed negative pressure wound therapy after laparotomy for gastrointestinal perforation: A single-center observational study.

AIM: Laparotomy for gastrointestinal perforation is associated with a high risk of surgical site infection (SSI). The present study aimed to evaluate the effect of closed negative pressure wound therapy (NPWT) against SSI in patients undergoing laparotomy for gastrointestinal perforation. METHODS: This single-center, retrospective, observational study was carried out in a tertiary emergency medical center. We performed closed NPWT following emergency laparotomy for gastrointestinal perforation between May 2019 and May 2024, and compared it to standard dressings used between January 2016 and A…

Prophylactic Application of Closed-Incision Negative Pressure Therapy in a High-Risk Emergency Laparotomy: A Case Report.

Surgical site infections (SSIs) and wound dehiscence are common complications following laparotomy, particularly in patients with comorbidities such as diabetes and obesity. Closed-incision negative pressure therapy (ciNPT) is increasingly recognised for its potential to improve surgical site outcomes by reducing infection rates and promoting wound healing. This case report describes the use of ciNPT in a patient with a high-risk surgical incision, highlighting its clinical benefits and practical application. We report the case of a 68-year-old obese female with type 2 diabetes mellitus who un…

Isolated Hepatic Tuberculosis Discovered Incidentally During Laparotomy for Gastric Perforation in an Immunocompetent Patient: A Case Report.

Isolated hepatic tuberculosis is a rare clinical entity, particularly in immunocompetent patients. It is characterized by frequently silent symptoms and nonspecific radiological abnormalities, which complicates its diagnosis in the absence of specific clinical guidance. We present the case of a 39-year-old man with no significant medical history who was admitted to the emergency department with acute peritonitis resulting from gastric perforation. An exploratory laparotomy confirmed the prepyloric perforation and incidentally revealed the presence of two subcapsular hepatic nodules located in …

Evaluation of Biomarkers from Peritoneal Fluid as Predictors of Severity for Abdominal Sepsis Patients Following Emergency Laparotomy.

PURPOSE: Intra-abdominal infection is considered the second most common cause of sepsis and results in localized or diffused inflammation of the peritoneum. The main treatment for abdominal sepsis is an emergency laparotomy for source control. However, surgical trauma also causes inflammation, and patients become susceptible to postoperative complications. Therefore, it is necessary to identify biomarkers that can be used to distinguish sepsis from abdominal infection. This prospective study investigated whether cytokine levels in the peritoneum could predict complications and indicate severit…

Predictive Models for Equine Emergency Exploratory Laparotomy in Spain: Pre-, Intra-, and Post-Operative-Mortality-Associated Factors.

The extrinsic and intrinsic characteristics of an equine population may influence the onset of gastrointestinal lesions and affect the survival rate of patients. The equine population in Spain has been the focus of a small number of studies, none of which have involved more than one surgical center. In this retrospective cohort study, we aimed to analyze the survival rate, identify the variables that influenced death, and generate multivariate models using clinical variables. Data were collected from the clinical records of two surgical referral centers in the same region, and a total of 566 h

Incisional Negative Pressure Wound Therapy to Decrease the Incidence of Surgical Site Infections in Trauma Laparotomy Wounds.

BACKGROUND: Surgical site infection (SSI) is a common post-operative complication, especially in trauma laparotomies. Incisional negative pressure wound therapy (iNPWT) is a novel technique in reducing SSIs. We aim to study the rate of wound complications in trauma laparotomy with standard primary closure with staples vs iNPWT. METHODS: We had 152 patients meeting inclusion criteria who underwent emergent trauma laparotomies performed at Sentara Norfolk General from 2017 to 2020. We had 79 patients in the standard staple group and 73 patients in the iNWPT group. We then analyzed surgical site…

Reduced Rate of Dehiscence After Implementation of a Standardized Fascial Closure Technique in Patients Undergoing Emergency Laparotomy.

OBJECTIVE: In elective surgery, it is well documented that a midline laparotomy should be closed with a slowly absorbable monofilament suture material in a continuous technique, in a ratio of at least 4 : 1. The evidence concerning the suture material or suturing technique in the emergency setting is lacking. We aimed to investigate whether this technique would reduce the rate of dehiscence. METHODS: A standardized procedure of closing the midline laparotomy by using a "small steps" technique of continuous suturing with a slowly absorbable (polydioxanone) suture material in a wound-suture rat…
