[Post-traumatic lower limb pain in children].

Injuries to the lower extremity are frequent. They present as pain, reduction of joint function, alteration in gait. Most of the orthopaedic emergencies for teenagers are related to trauma. For younger children, a history of recent fall is prevalent but the non traumatic orthopaedic conditions are especially important to investigate when the reported injury is minor or when onset of symptoms is delayed.

Office treatment of lower extremity injuries. A view of feasibility, limitations and hazards.

The injuries to the lower extremities seen in a surgical office may be classified as contusions, lacerations, sprains, lesions of tendons and their sheaths, involvement of bursae, chronic muscle fatigue, infections and deformities of the nails, leg ulcers, and fractures of the ankle, foot and toes. The treatment of these conditions in an office will vary under different circumstances, but one should be guided by certain fundamental rules. Contusions are best treated by the application of elastic compression bandages. Extensive lacerations should be explored under local anesthesia with the tour

Successful replantation of both legs in a child--5-year followup: case report.

Replantation of lower extremities is an infrequent procedure, and is rarely indicated, due to the usual severity of the primary injury plus the possibility of good prosthetic substitution. We report a case of successful replantation of both legs in an 8-year-old boy (despite Aspergillus flavus septicemia) with followup of 5 years. This case suggests that leg replantation should be considered in young patients. This appears to be the second reported case of successful replantation of both legs.

The Effect of Isometric Exercise Position on the Effectiveness of Isolated Work of the Thigh Flexor Muscles Based on the Results of the sEMG Study.

BACKGROUD: The main function of the hamstring muscles is to bend the knee joint and support the function of the hip extensors. Their frequent injuries are the result of overload related to, among others, dynamic running or jumping, and inadequate preparation for athletics activities. The asymmetry of the work of individual flexor muscle groups is clearly marked in the case of valgus or varus of the knee joint, i.e., in different positions of the lower limb. The aim of the study was to determine the position and form of a rehabilitation exercise in which an isolated group of muscles flexing the

The effects of two different fatigue protocols on movement quality during anticipated and unanticipated change of directions in female soccer players.

OBJECTIVE: This study compared neuromuscular control under two fatigue protocols during anticipated and unanticipated change of direction (COD) maneuvers and evaluated their effects on the risk of non-contact ACL injuries. METHOD: Forty-five female soccer players (mean age: 22.22 ± 2.24 years; mean height: 166.24 ± 3.33 cm; mean mass: 59.84 ± 5.03 kg) were divided into three groups: functional fatigue (Soccer specific fatigue ptotocol-SOFT90), non-functional fatigue (Bruce protocol), and control group. Before and after the implementation of neuromuscular control fatigue protocols were evaluat

Cross-foot island instep flap: a new use of instep skin flap for management of persistent wounds after complex plantar foot reconstruction.

When presented with an extensive soft-tissue defect involving the sole of the foot, reconstruction with free muscle flaps covered by a split-thickness skin graft is the proposed method of treatment. However, persistent graft breakdown and a chronic wound of the weight-bearing flap is a challenging problem during the late postoperative period, as experienced by the authors in their patients with high-energy-induced lower extremity injuries. The authors used the instep flap as an island cross-foot flap to manage persistent graft breakdown that involved skin-grafted muscle flaps transferred previ

Quantifying changes in individual-specific template-based representations of center-of-mass dynamics during walking with ankle exoskeletons using Hybrid-SINDy.

Ankle exoskeletons alter whole-body walking mechanics, energetics, and stability by altering center-of-mass (CoM) motion. Controlling the dynamics governing CoM motion is, therefore, critical for maintaining efficient and stable gait. However, how CoM dynamics change with ankle exoskeletons is unknown, and how to optimally model individual-specific CoM dynamics, especially in individuals with neurological injuries, remains a challenge. Here, we evaluated individual-specific changes in CoM dynamics in unimpaired adults and one individual with post-stroke hemiparesis while walking in shoes-only

Innovative Techniques for Maximizing Limb Salvage and Function.

Concurrent injuries to multiple extremities present unique challenges to the reconstructive surgeon. The primary goal in such scenarios is to optimize functional outcomes. The goal of this article is to present an overview of various techniques necessary to provide sufficient soft tissue and preserve amputation limb lengths and function. The concept of innovative techniques for maximizing limb savage and function is presented using an index patient with multiple extremity third- and fourth-degree burn injuries resulting in nonsalvageable lower extremities and severe left-hand wounds. A review

Bilateral Alterations in Isokinetic Strength and Knee Biomechanics During Side-Cutting 1 Year After Unilateral ACL Reconstruction.

BACKGROUND: Individuals with anterior cruciate ligament (ACL) reconstruction (ACLR) are a population that has a higher risk for ACL injury compared with the general population. To reduce the reinjury rate and improve the rehabilitation outcome after ACLR, risk factors for ACL injury have to be addressed. PURPOSE: To compare the knee biomechanics during side-cutting and isokinetic strength of the thigh muscle of the reconstructed leg with those of the contralateral leg and healthy controls and investigate the knee movement asymmetries in individuals with ACLR. STUDY DESIGN: Controlled laborat

Compartment syndrome of the lower leg and foot.

UNLABELLED: Compartment syndrome of the lower leg or foot, a severe complication with a low incidence, is mostly caused by high-energy deceleration trauma. The diagnosis is based on clinical examination and intracompartmental pressure measurement. The most sensitive clinical symptom of compartment syndrome is severe pain. Clinical findings must be documented carefully. A fasciotomy should be performed when the difference between compartment pressure and diastolic blood pressure is less than 30 mm Hg or when clinical symptoms are obvious. Once the diagnosis is made, immediate fasciotomy of all

Neuromuscular Strategies in Dominant and Non-Dominant Legs in Dancers During Dynamic Balance Tasks.

Introduction: Ballet-based dance training emphasizes the equal development of both legs. However, dancers often perceive differences between their legs during balance or landing. There still needs to be more consensus on the functional difference between dominant (D) and non-dominant legs (ND). Therefore, this study investigated both legs' neuromuscular strategy in single-leg balance and landing based on leg dominance. Methods: Thirteen female dancers (age: 22.2 ± 2.8 years) with no history of ankle injuries in the past year participated in the study. Based on the questionnaire, the dominant l
