Understanding support needs for people carrying pregnancies with life-limiting fetal conditions: a qualitative study.

OBJECTIVE: To understand the experiences and possible gaps in care, including availability of perinatal palliative care, in patients carrying a pregnancy diagnosed with life-limiting fetal anomalies who sought abortion. STUDY DESIGN: From April 2020 to May 2021, we conducted in-depth semi-structured interviews 2-8 weeks after participants had aborted a pregnancy with a life-limiting fetal condition. We recruited English-speaking participants 18 years or older from multiple states with varying legislative support and restrictions towards abortion. Interviews were conducted over the telephone,

Perinatal nutrition and hormone-dependent programming of food intake.

It is increasingly accepted that alterations of the intrauterine and early postnatal nutritional, metabolic and hormonal environment may predispose individuals to development of diseases in later life. Results from studies of the offspring of diabetic mothers strongly support this hypothesis. It has also been suggested that being light at birth leads to an increased risk of the metabolic syndrome (Syndrome X) in later life (the Barker hypothesis). The pathophysiological mechanisms that underlie this programming are unclear. However, hormones are important environment-dependent organizers of th

Critical time intervention to prevent homelessness and improve health for people leaving prison: the PHaCT pilot RCT.

BACKGROUND: People leaving prison face significant barriers to reintegration, often resulting in homelessness, which exacerbates health issues and increases recidivism. Critical time interventions aim to support vulnerable individuals during significant life transitions by providing time-limited, emotional and practical support. While effective in other contexts, the impact of housing-led critical time interventions for people leaving prison in the United Kingdom remains unclear. The PHaCT study was a pilot randomised controlled trial of a housing-led critical time intervention for people leav

Pancreatic cancer-A disease in need: Optimizing and integrating supportive care.

Pancreatic ductal adenocarcinoma (PDAC) is an aggressive malignancy that continues to be challenging to treat. PDAC has the lowest 5-year relative survival rate compared with all other solid tumor malignancies and is expected to become the second-leading cause of cancer-related death in the United States by 2030. Given the high mortality, there is an increasing role for concurrent anticancer and supportive care in the management of patients with PDAC with the aims of maximizing length of life, quality of life, and symptom control. Emerging trends in supportive care that can be integrated into

Multi-level Factors Influencing Decisions About Stopping Surveillance Colonoscopy in Older Adults: a Qualitative Study.

BACKGROUND: Little is known about patient or provider experience and perceptions of stopping surveillance among older adults with a history of colon polyps. While guidelines recommend ceasing routine colorectal cancer screening in adults  > 75 years and those with limited life expectancy, guidance for ceasing surveillance colonoscopy in those with prior colon polyps suggests individualizing recommendations. OBJECTIVE: Identify processes, experiences, and gaps around individualizing decisions to stop or continue surveillance colonoscopy for older adults and areas for improvement. DESIGN: Phen

Nutritional support in patients with systemic scleroderma.

Systemic scleroderma often involves the gastrointestinal tract and can lead to significant malnutrition. Specialized home nutritional support was provided to five patients (one enteral and four parenteral) with restoration of adequate nutritional status, improved quality of life, and few metabolic or technical complications over a period of 12 to 86 months. Three patients eventually died of their disease, one was converted to an oral diet, and one patient remains dependent on total parenteral nutrition. This experience would recommend consideration of home nutritional support in patients with

The unsung heroes of traditional healers in healing and traditional medicine in dementia care for Indigenous populations in North America: an exploration of a compassionate, community-led care model.

This paper explores the critical role, experience, and wisdom of traditional healers in providing culturally-safe dementia care for Indigenous populations in North America, particularly as the prevalence of dementia in these communities continues to rise. It highlights the varying perceptions of dementia across cultures; for instance, while Western medicine often views it as a disease, many Indigenous cultures interpret it as a spiritual transition within the life cycle. A literature review conducted from 2000 to 2025 indicates that the marginalization of traditional healers within formal heal

Early Detection Methods for Autism Spectrum Disorder: From Clinical Screening to Multimodal AI.

Early detection of autism spectrum disorder (ASD) in young children is essential for timely referral, developmental monitoring, and access to early intervention. However, conventional screening and diagnostic pathways often depend on parent-report instruments, episodic clinical observation, and specialist-administered assessments, which may delay identification during the first years of life. This scoping review maps the methodological landscape of early ASD detection from traditional clinical screening to multimodal artificial intelligence (AI). A structured literature search was conducted ac

Chapter - Follow-up and functional scores: PROMs.

Spinal metastases represent a major event in the course of cancer, frequently resulting in pain, neurological impairment, reduced functional capacity, and diminished quality of life. Traditional assessment of these patients has largely relied on clinical examination, radiological findings, and prognostic scoring systems. However, these approaches may not fully capture the patient's experience of disease and treatment. Patient-reported outcome measures (PROMs) provide a standardised method for evaluating symptoms, functional status, and health-related quality of life from the patient's perspect

The immune system as a guardian of health: micronutrient support and lifelong protection.

The immune system constitutes a physiological network that preserves organismal stability through continuous interactions with tissues and regulatory systems. Immune activity is an integral component of normal tissue physiology, contributing to host defense, structural integrity, metabolic balance, and functional homeostasis. The immune system integrates molecular recognition, effector mechanisms, and inflammation resolution to control infections, limit tissue damage, and reduce the progression of chronic and degenerative diseases. These combined defensive and physiological functions underpin

Developmental Neuroendocrinology of Early-Life Stress: Impact on Child Development and Behavior.

Our internal balance, or homeostasis, is threatened or perceived as threatened by stressful stimuli, the stressors. The stress system is a highly conserved system that adjusts homeostasis to the resting state. Through the concurrent activation of the hypothalamic-pituitary-adrenal axis and the locus coeruleus/norepinephrine-autonomic nervous systems, the stress system provides the appropriate physical and behavioral responses, collectively termed as "stress response", to restore homeostasis. If the stress response is prolonged, excessive or even inadequate, several acute or chronic stress-rela

Uncovering the Use of Family-Centered Care Interventions Among Children With Cancer and Their Families: A Scoping Review of the Characteristics of Interventions and Effectiveness.

Family-centered care (FCC) interventions have gained increasing attention in pediatric oncology, yet their characteristics, effectiveness, and implementation challenges remain insufficiently synthesized. This scoping review systematically mapped existing FCC interventions for children with cancer and their families following JBI methodological guidance and the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) reporting standards. Twenty-two studies involving 1,049 pediatric patients and 1,504 family members were included. Intervention

Perineal injuries during vaginal birth in low-resource countries.

Birth related perineal trauma (BRPT) and obstetric anal sphincter injuries (OASIS) are leading causes of anal incontinence in women, which negatively impacts on their quality of life, resulting in low self-esteem and abandonment. In low resource countries (LRC), the true incidence is not known and since most births are unattended or occur in community-based health care systems, one can anticipate that it is a significant problem. Dissemination of information to women, education of traditional birth attendants, improvement of resources and transport, and training of health professionals on the

Acute liver failure.

Acute liver failure (ALF) is a life-threatening disorder characterised by rapid deterioration of liver function, coagulopathy, and hepatic encephalopathy in the absence of pre-existing liver disease. The cause of ALF varies across the world. Common causes of ALF in adults include drug toxicity, hepatotropic and non-hepatotropic viruses, herbal and dietary supplements, antituberculosis drugs, and autoimmune hepatitis. The cause of liver failure affects the management and prognosis, and therefore extensive investigation for cause is strongly suggested. Sepsis with multiorgan failure and cerebral

Telehealth in long-term neurological conditions: the potential, the challenges and the key recommendations.

Long-term neurological conditions (LTNCs) cause physical and psychological symptoms that have a significant impact on activities of daily living and quality of life. Multidisciplinary teams are effective at providing treatment for people with LTNCs; however, access to such services by people with disabilities can be difficult and as a result, good quality care is not universal. One potential solution is telehealth. This review describes the potential of telehealth to support people with LTNCs, the challenges of designing and implementing these systems, and the key recommendations for those inv

Tailoring peripartum nursing care for women of advanced maternal age.

Births to women of advanced maternal age have increased dramatically over the last decade in both the United States. The majority of women who deliver their first baby after age 35 are healthy and experience positive birth outcomes. According to current research, primigravidas over 35 tend to be educated consumers. Their physical and psychosocial needs differ from those of the mother in her 20s, due to advanced age and factors related to difficulty conceiving and life circumstances. This paper presents (a) an overview of the possible risks to outcomes of childbearing for women over the age of
