Klüver-bucy syndrome with isolated bilateral hippocampal atrophy following status epilepticus.

Klüver-Bucy syndrome may result from affection of various location of brain. We report a case of Klüver-Bucy syndrome associated with isolated bilateral hippocampal atrophy without any abnormal lesion in other areas following status epilepticus. A 31-year-old man who had no significant medical history presented with status epilepticus after encephalitis of unknown etiology. He had been recovered from status epilepticus three weeks later, but afterwards he developed Klüver-Bucy syndrome: hyperphagia, hypersexuality, hypermetamorphosis, anterograde amnesia and dysosmia. Initial brain MRI showed

Clinico-radiological spectrum of bilateral temporal lobe hyperintensity: a retrospective review.

Bilateral temporal lobe hyperintensity (BTH) is a commonly encountered MRI finding in a wide spectrum of clinical conditions and often poses … osomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy) and paraneoplastic (limbic encephalitis) disorders. The conventional MRI findings with advanced MRI such as diffusion-weighted imaging, susceptibility-weighted imaging and MR spectroscopy along with laboratory results are potentially helpful in distinguishing the different clinical condit …

[Potassium channelopathies and Morvan's syndromes].

Interest in Morvan's disease or syndrome has grown, owing to its close links with various potassium channelopathies. Potassium is crucial fo … commonest, and especially those belonging to the Shaker group (neuromyotonia and Morvan's syndrome, limbic encephalitis, and type 1 episodic ataxia). Brain and heart K+ channelopathies are a separate group due to KCNQ1 mutation (severe type 2 long QT syndrome). Kv7 channel mutations (in KNQ2 and KCNQ3) are responsible for benign familial neonatal seizures. Mutation o …

[Paraneoplastic neurologic syndromes. Diagnostic and pathogenetic significance of autoantibodies].

Paraneoplastic neurologic syndromes (PNS) are rare disorders of the nervous system that cannot be ascribed to metastases or to destruction o … cific antibodies. In certain neurologic syndromes (sensible and autonomic neuropathy, cerebellitis, limbic encephalitis, Opsoclonus-Myoclonus syndrome, Stiff-man syndrome, neuromyotonia, subacute amaurosis) specific autoantibodies are highly predictive for a neoplasm. Pathogenic relevance of these autoantibodies has been demonstrated only for antibodies against (1) v …

Early Neuroaxonal Damage in Neurologic Disorders Associated With GAD65 Antibodies.

OBJECTIVES: Neurodegeneration is considered a relevant pathophysiologic feature in neurologic disorders associated with antibodies against g … ding disease. Increased sNfL levels were observed in patients presenting with cerebellar ataxia and limbic encephalitis, but not in those with stiff person syndrome. Using MRI volumetry, we identified atrophy predominantly of the cerebellar cortex, cerebellar superior posterior lobe, and cerebral cortex with similar atrophy patterns throughout all clinical phenotypes …

Multiplex family with GAD65-Abs neurologic syndromes.

OBJECTIVE: Neurologic autoimmune syndromes associated with anti-glutamate acid decarboxylase 65 antibodies (GAD65-Abs) are rare and mostly sporadic. METHODS: We describe a niece and her aunt with GAD65-Abs neurologic syndromes. High-resolution HLA typing of Class I and Class II alleles was performed using next-generation sequencing. RESULTS: The proband had cerebellar ataxia and probable limbic encephalitis features, whereas her niece had stiff-person syndrome. Both had a high titer of GAD65-Abs in serum and CSF and showed signs of inflammation in CSF. Both affected members carried the same …

Paraneoplastic neurological autoimmunity and survival in small-cell lung cancer.

The autoimmune disorder of Lambert-Eaton myasthenic syndrome (LEMS) associates with small cell lung carcinoma (SCLC) in 50-60% of cases. It … tected in 10 patients, however only 4 had clinical and electrophysiological features of LEMS, 1 had limbic encephalitis, whilst the remaining 5 had no neurological signs. A further 6 patients had onconeural antibodies; only one had a paraneoplastic syndrome, sensory neuropathy. The median survival of the four antibody positive LEMS patients (19.6 months) was consider …

Clinical spectrum and outcomes of anti-metabotropic glutamate receptor 5 encephalitis in Chinese patients: a case report and literature review.

BACKGROUND: Anti-metabotropic glutamate receptor 5 (mGluR5) encephalitis is a rare autoimmune disorder that targets the metabotropic glutamate receptor. It is frequently linked to limbic encephalitis and paraneoplastic syndromes, such as Ophelia syndrome associated with Hodgkin lymphoma. Due to its rarity, the complete clinical spectrum and regional variations of this condition, particularly among Chinese populations, remain inadequately understood. CASE DESCRIPTION: We present a case of a 39-year-old Chinese male diagnosed with anti-mGluR5 encephalitis. The patient initially presented with p…

Anti-adenylate kinase 5 encephalitis: Case series and review of 18F-DPA714 PET/MRI radiographic changes.

OBJECTIVES: Anti-adenylate kinase 5 (AK5) encephalitis is a rare, predominantly limbic autoimmune disorder featuring subacute cognitive and psychiatric decline. Given the limited literature and MRI's suboptimal sensitivity to neuroinflammation, often precluding timely therapeutic escalation, we sought to delineate the clinical and multimodal imaging spectrum of this condition and propose stage-specific management protocols. METHODS: Two female patients (aged 15 and 65 years) with subacute cognitive and psychiatric-behavioral disturbances were prospectively evaluated with serial MMSE, CASE and

Olfactory impairment in autoimmune encephalitis: another piece of the puzzle.

BACKGROUND: Despite being long neglected, olfaction has recently become a focus of intense research in neuroscience, as smell impairment has been consistently documented in both neurodegenerative and neuroinflammatory diseases. Considering the close anatomo-functional correlations between the limbic system and the central olfactory structures, we investigated olfaction in a population of patients with autoimmune encephalitis (AE). METHODS: Nineteen adult subjects (14 males, median age 64 years) diagnosed with definite (14/19) or possible (5/19) AE and followed for ≥ 6 months were enrolled. Th

Autoantibodies to synapsin I sequestrate synapsin I and alter synaptic function.

Synapsin I is a phosphoprotein that coats the cytoplasmic side of synaptic vesicles and regulates their trafficking within nerve terminals. Autoantibodies against Syn I have been described in sera and cerebrospinal fluids of patients with numerous neurological diseases, including limbic encephalitis and clinically isolated syndrome; however, the effects and fate of autoantibodies in neurons are still unexplored. We found that in vitro exposure of primary hippocampal neurons to patient's autoantibodies to SynI decreased the density of excitatory and inhibitory synapses and impaired both glutama…

The occurrence of vacuolated neurons in the brains of hamsters affected with subacute sclerosing encephalitis following measles or Langat virus infection.

Hamsters infected by the intranasal route with either hamster adapted Langat virus passaged at least 10 times in hamsters and the HNT strain of measles virus passaged 149 times in hamsters, developed subacute sclerotic lesions in the pyriform cortex, and beginning from 2 months after infection, were accompanied by neuronal vacuolation with ballooning of the cytoplasm, especially in parts of the brain in close proximity of the sclerotic lesions. The vacuolated neurons resembled those seen in scrapie, especially of sheep and goats, suggesting a similarity between the effects of slow and subacute

Case Report: Nystagmus as a core presenting sign in pediatric anti-Ma2 antibody-associated cerebellar ataxia: diagnostic implications from clinical and serological profiling.

BACKGROUND: The anti-Ma2 antibody is a well-known, specific marker of paraneoplastic limbic and brainstem encephalitis, mainly described in adult, especially in males with testicular germ cell tumor. Pediatric cases remain exceptionally rare. We present a child with anti-Ma2 antibody-associated cerebellar ataxia in whom nystagmus was identified as a core presenting symptom; the diagnosis was confirmed via cell-based assay (CBA). CASE PRESENTATION: An 11-year-old boy sought medical attention for symptoms such as vomiting, nystagmus, dizziness, slurred speech, and limb weakness. Routine laborat

[Herpes simplex encephalitis presenting as a stroke-like episode following a migraine attack: a case report].

A 23-year-old woman, who had been suffering from migraine since primary school age, presented with left arm paralysis three days after one such migraine attack. On admission, brain MRI diffusion-weighted imaging (DWI) demonstrated high-signal-intensity lesions in the white matter of the right fronto-parietal lobe, and no abnormal lesions were evident in the limbic system. Although the patient had a fever of 38.7°C, the CSF cell count was not elevated. On the 4‍th day, the left arm paralysis worsened, with an increase in body temperature to 39.8°C. Brain MRI revealed that the white matter lesio

Limbic-predominant Age-related TDP-43 Encephalopathy and Distinct Longitudinal Profiles of Domain-specific Literacy.

PURPOSE: Emerging evidence suggests that limbic-predominant age-related TAR DNA-binding protein-43 (TDP-43) encephalopathy impacts domain-specific literacy, a complex ability not assessed in traditional cognitive evaluations. We examined longitudinal profiles of financial and health literacy in relation to limbic-predominant age-related TDP-43 encephalopathy neuropathologic change (LATE-NC). PARTICIPANTS: A total of 275 community-dwelling older persons who had completed annual literacy assessments, died and undergone brain autopsy. METHODS: Financial and health literacy was assessed using a

[Lung cancer and paraneoplastic syndromes].

Paraneoplastic syndromes (PNS) are a relatively common manifestation of cancer, and in some cases they may be the first symptom. Lung cancer has the highest incidence of paraneoplastic syndrome. This fact is important considering a non explained endocrinological and neurological syndrome, it may facilitate a prompt diagnosis, and in some cases an adequate treatment. PNS evolution seems to be parallel to the subjacent cancer. PNS management requires specific measures, because in some cases, it may compromise the patient life. Neurological and endocrinological PNS associated to lung cancer are r

Pseudopseudoseizures: conditions that may mimic psychogenic non-epileptic seizures.

BACKGROUND: Video electroencephalography (vEEG) has proven to be a valuable tool in the differentiation of pseudoseizures or psychogenic non-epileptic seizures (PNES) from epileptic seizures. Clinicians should, however, be aware of a number of conditions that may be misdiagnosed as pseudoseizures. OBJECTIVE: To review a number of somatic conditions that can present with disturbances of consciousness and unusual motor activity without epileptiform findings on vEEG. METHOD: We report a case of a young woman who presented with a number of neuropsychiatric symptoms, including episodic posturing
