Cystic complex odontoma associated with an impacted canine.

An odontoma is a benign tumor containing all the various component tissues of teeth. Although it is the most common odontogenic tumor, it is usually discovered upon radiographic examination or later in life when it has developed into a cyst causing facial asymmetry and swelling of the face. Here is a case of a cystic complex odontoma associated with an impacted permanent canine.

[Solitary plasmocytoma of the mandible].

The authors present a case of solitary bony plasmocytoma of the mandibule, with local amyloid deposition. Diagnosis was made at extemporaneous pathological examination, then confirmed by the examination of the fixed surgical sample. Immunoenzymologically the proliferation proved to be monoclonal, with secretion of kappa light chain. The patient was treated by surgical curettage and homologous cancelous bone filling, followed by 5,500 rads of electrontherapy. He was then regularly controlled during 8 years and remained free from local recurrence. All investigations in search of dissemination (K

A solitary bone cyst of the mandibular condyle.

A review of the literature concerning solitary bone cysts of the jaws is presented. Our patient is typical except for the location of the cyst. No previous case of a solitary bone cyst confirmed to the mandibular condyle has been reported. No precise etiology for solitary bone cyst is known. The use of routine panoramic radiography has made detection of such cysts more common. Surgical exploration is indicated for diagnosis and treatment.

[Epidermoid cysts of the mandible (author's transl)].

The authors present three cases of epidermoid cyst of the mandible, one of which had undergone malignant change. These cysts are rare. They are most often situated at the angle of the jaw and present as a deep swelling of the cheek. The only treatment is excision, the authors advising the external approach which gave them the best results.

Successful Surgical Treatment of Pediatric FUS::TFCP2-Positive Rhabdomyosarcoma of the Mandible: A Case Report.

FUS::TFCP2-positive rhabdomyosarcoma (RMS) is a rare and aggressive subtype that was recently classified as a distinct entity. It is resistant to conventional chemotherapeutic regimens for RMS, and optimal treatment strategies have not been established. Here, we describe an 11-year-old boy with mandibular RMS with a FUS::TFCP2 fusion gene. At the referring hospital, the patient initially underwent standard induction chemotherapy for RMS, but it was ineffective. Radical resection, including segmental mandibulectomy and reconstruction with a rectus abdominis free flap, was performed. Histologica

Application of digital design and 3D printing technology in repairing mandibular defects with fibula musculocutaneous flap.

BACKGROUND: Mandibular tumors are common in oral and maxillofacial surgery. Following tumor resection, significant bone tissue defects often occur, severely compromising both facial appearance and function. Currently, fibular myocutaneous flaps are widely employed by head and neck surgeons to repair various types of mandibular defects. However, accurately restoring the mandibular contour remains a major challenge. To achieve optimal clinical outcomes, we applied digital design and 3D printing technology in conjunction with fibula musculocutaneous flaps for mandibular defect reconstruction. In

Scapula harvest in the supine position for immediate mandibular reconstruction.

PURPOSE: The scapula is the second most popular donor site for mandibular reconstruction after the fibula. Scapula harvest is generally performed in the lateral decubitus position and the requirement of positional changes hamper the widespread use of the scapula. This study compared scapula harvest for immediate mandibular reconstruction between the supine and lateral decubitus positions. METHODS: We reviewed the outcomes of 16 patients who underwent segmental mandibulectomy and immediate reconstruction of the scapula based on the angular branch of the thoracodorsal artery. The scapula was ha

Accuracy and clinical outcomes of mandibular reconstruction with a virtually planned deep circumflex iliac artery flap with stock temporomandibular joint prosthesis.

The repair of hemimandibulectomy defects involving the temporomandibular joint (TMJ) is challenging. This study compared the functional outcomes and reconstruction accuracy using a deep circumflex iliac artery (DCIA) flap with and without a virtually planned stock TMJ prosthesis (TMJP) after hemimandibulectomy. Ten patients were assessed: five with a TMJP (TMJP group) and five without (control group). A three-dimensional comparison revealed a mean deviation of 0.11 ± 0.04 mm between the planned and actual DCIA flap with TMJP. The planned and actual TMJP positions differed by 0.56 ± 0.57 mm in

Endoscope-assisted intraoral excision of osteochondroma of the mandibular condyle.

Osteochondroma (OC) is one of the most common bony lesions of the skeleton. It is rare in the maxillofacial region and reported infrequently in the literature. This lesion is considered the most common of benign tumors of the temporomandibular joint. It is usually associated with the coronoid, followed by the condyloid process. OC can affect any bone formed from cartilage and can arise spontaneously or as a result of osseous trauma. It usually protrudes from the surface of bone and is covered with cartilage. Most condylar OCs present as progressive limitation of mandibular movements, facial as

Adenoameloblastoma: a dilemma in diagnosis.

Adenoameloblastoma or adenomatoid odontogenic tumor (AOT) is an uncommon, benign, epithelial lesion of odontogenic origin. It is a rare benign odontogenic tumor of the jaw affecting mostly young individuals with predominance in female. It occurs mostly in maxillary anterior region. On the basis of clinical and radiographical picture, it is often misdiagnosed as an odontogenic cyst. We report on a rare case of a 13-year-old male patient with a follicular variety of AOT in mandibular left anterior region which is unusual for the same. Clinically and radiographically, the lesion was mimicking as

Diagnosis and treatment of an infant case with temporomandibular joint osteoarthritis caused by tumor.

INTRODUCTION: We reported an infant with temporomandibular joint (TMJ) osteoarthritis who required long-term tracheostomy. CASE FINDINGS: At 1 year of age, he received urgent tracheostomy because of apnea due to pneumonia. The tracheal cannula could not be removed thereafter due to trismus. Computed tomography (CT) images demonstrate irregular bone formation. However, we could not diagnose osteoarthritis of temporomandibular joint because TMJ adhesion was not clear on CT images. INTERVENTIONS: Arthroplasty was performed under general anesthesia, but there was no significant improvement. A 3-

[Hyperbaric oxygen therapy. Clinical use in treatment of osteomyelitis, osteoradionecrosis and reconstructive surgery of the irradiated mandible].

The aim of hyperbaric oxygenation (HBO) is to increase transported oxygen in the blood over the rise of the physiologically solved part. In oral and maxillofacial surgery, treatment-resistant osteomyelitis and osteoradionecrosis are indications for HBO (according to Undersea and Hyperbaric Medical Society). Furthermore, there is an indication to apply HBO to compromised transplants of bone or soft tissue. Since 1994, 28 patients have been treated with HBO in the department of oro-, maxillofacial surgery of the Medical School Hannover. Sixteen patients had a treatment-resistant osteomyelitis. T

Adenomatoid odontogenic tumor presenting as periapical disease.

Adenomatoid odontogenic tumor commonly occurs in association with the crowns of unerupted teeth. An extrafollicular variant, radiographically in relationship to root apices, has been reported. However, clear association with the root apices at surgery has not been demonstrated. We report a case of adenomatoid odontogenic tumor in the anterior mandible in a 21-year-old woman that presented radiographically at the root apices and at surgery as a radicular cyst. We believe this represents the first reported case of adenomatoid odontogenic tumor presenting as periapical disease both clinically and

[Long-term results of mandibular reconstruction with avascular spongiosa transfer and titanium mesh].

Severe and mild complications could only be detected in damaged transplant bed (radical operation and radiation exposure, traumatic lesions) in osteoplastic reconstruction of the mandible with avascular cancellous bone transfer using a titanium carrier. Altogether, we rate this method as more favorable compared to an avascular block transplant because of advantages in handling the system in the surgical technique, contouring of the problem zones in the chin and mandibular angle region, more simple transplant extraction and greater resistance to infections. Owing to the high osteogenic potency

Mandibular involvement in patients with multiple myeloma.

A survey of changes in the jaw bones of 38 patients with multiple myeloma revealed five cases with mandibular involvement. One case showed a pathological fracture of the mandibular angle, and in another case, a radiolucent area was found in the body of the mandible. These two clinical findings led to the diagnosis of multiple myeloma. The other three cases had no symptoms related to the oral region. All five cases also showed typical "punched-out" lesions in the skull. It is suggested that a skeletal radiologic survey including the jaw should be performed in patients with multiple myeloma.

Treatment of osteoradionecrosis combined with pathologic fracture and osteomyelitis of the mandible with electromagnetic stimulation.

Osteoradionecrosis, complicated by pathologic fracture and osteomyelitis, is difficult to treat. Electromagnetic stimulation therapy may present a solution to this problem. A case of a 66-year old woman, suffering from osteoradionecrosis, pathologic fracture and osteomyelitis of the mandible, 6 years after irradiation therapy and partial resection of the mandible for squamous cell carcinoma of the gingiva, treated by electromagnetic stimulation, is presented. After 9 months of treatment, the patient was asymptomatic. Radiographic examination and bone scintigraphy carried out upon termination o

Results of bone grafting after rigid fixation.

Results of bone grafting in ten cases of primary mandibular reconstruction using rib grafts and 62 cases of secondary mandibular reconstruction using iliac bone grafts or rib transplants are reported. Two (20%) of the primary transplants were lost and one of the secondary transplants was totally lost and another was partially lost (3.2%) because of infection. All other transplants healed without infection. Pseudarthrosis was observed in 30% of the cases after primary reconstruction and in 15% of the cases after secondary reconstruction.
