Screening for colorectal cancer using the faecal occult blood test, hemoccult.

BACKGROUND: Colorectal cancer is a leading cause of illness and death in the Western world. In Australia, the United Kingdom and the United States, it is the second commonest cancer for women after breast cancer (age-standardised incidence 22-33 per 100,000), and men after prostate or lung cancer (age-standardised incidence 31-47 per 100,000) (Jeffs et al, 1996; Parkin et al, 1992). Just under half of all persons affected will die from their disease (Jeffs et al, 1996; Parkin et al, 1992) The human and financial costs of this disease have prompted considerable research efforts to evaluate the

What's new in colorectal cancer screening?

There is now good evidence from case control studies and randomized controlled trials that screening average-risk subjects for colorectal cancer using faecal occult blood tests reduces mortality. There is limited data indicating that screening sigmoidoscopy can also achieve this. There is no evidence yet that screening by colonoscopy or double contrast barium enema can reduce mortality. Calculations of cost-effectiveness suggest that all of the above strategies should be economically worthwhile but there is no convincing evidence to suggest that one strategy is markedly more cost-effective tha

Retinopathy of prematurity: guidelines for screening and treatment. The report of a Joint Working Party of The Royal College of Ophthalmologists and the British Association of Perinatal Medicine.

Following the report that severe retinopathy of prematurity (ROP) can be treated effectively, a working party of the Royal College of Ophthalmologists and the British Association of Perinatal Medicine was convened in 1990 to draw up guidelines for screening [1]. These have been revised by a reconvened working party (Appendix D) and the document expanded to include other relevant issues such as the practicalities of treatment, counselling and end-stage retinopathy. Screening is recommended for all babies at risk of severe ROP, i.e. those of birth weight < or = 1500 g and or < or = 31 weeks gest

[Serum biological markers and screening of viral hepatitis].

The diagnosis of hepatitis A is based on IgM anti-HAV positivity. IgG anti-HAV indicates immunity from further infections of hepatitis A. In acute hepatitis B, HBs Ag is the first marker to appear in the serum, followed by HBe Ag and anti-HBc. After recovery, HBs Ag disappears, anti-HBs appears with anti-HBe and anti-HBc and persists for many years. Anti-HBs is indicative of recovery and immunity. Chronic hepatitis B infection is defined as the persistence of HBs Ag for more than 6 months. The presence of HBeAg and HBV DNA in the serum of HBsAg positive carriers has been considered as indicati

Canadian contributions to cancer control.

Cancer control comprises prevention, screening, treatment, rehabilitation and palliative care. Preventive approaches need to be congruent with those adopted for other chronic diseases, with a major impact to be expected from smoking control and dietary modification. Canadian studies have contributed substantially to knowledge in both areas. Increasing interest is now being paid to other environmental causes of cancer, and to gene-environment interactions. Canadian studies have shown that screening for cancer of the cervix is effective, but improved organization of services is required to make

The management of early skin cancer in the 90s.

Skin cancer has become a major source of illness and death in Australia. Recent advances in the early detection of skin cancer and in its treatment have significantly altered management, particularly of melanoma. Evidence that a high proportion of all skin cancers and most life-threatening skin cancers arise on the normally protected areas of the trunk and limbs has underlined the need for complete skin examination as a part of the care of long term patients in general practice.

The Psychological Impact of Screen-Detected Cancer: A Systematic Review.

BACKGROUND: The benefits and harms of cancer screening must be balanced for all participant groups, including those who go on to have cancer diagnosed. The psychological impact of having cancer diagnosed through screening, rather than via another route, is currently unclear. AIMS: We conducted a systematic review to describe the psychological impact of detecting cancer through screening (screen-detected) compared to other routes (non-screen-detected). METHODS: Eligible studies investigated the psychological impact of screen-detected cancer. PubMed, Embase, and PsycINFO were searched. Two rev

Bridging the gap in caregiver support: A systematic review of screening practices in primary care.

BACKGROUND: Family caregivers of older adults with cognitive impairment often experience significant emotional, physical, and psychological burden. Although primary care plays a crucial role in early identification, caregiver burden frequently goes unrecognized due to time constraints, lack of standardized protocols, and limited provider awareness. METHODS: A systematic literature review was conducted using PubMed, CINAHL, and Google Scholar to identify studies published between 2013 and 2023 on the topic of assessing for caregiver burden in primary care. RESULTS: Nine studies focusing on th

Beyond the clinic: a community-embedded, multidomain framework for early detection of glaucoma.

Glaucoma remains one of the leading causes of acquired irreversible blindness worldwide, with normal-tension glaucoma representing the dominant subtype in Japan and several East Asian populations. The insidious, asymptomatic progression of this condition, combined with the demonstrated inadequacy of intraocular pressure alone as a screening criterion, creates a critical gap between disease burden and case detection. Population-based epidemiological studies consistently reveal that the majority of individuals with glaucoma are undiagnosed. Two responses have been suggested: incorporation of ret

Is screening for Alzheimer's disease ready for prime time? Ask Wilson and Jungner.

Population screening is an effective strategy for disease prevention, early diagnosis and treatment; however, the benefits and harms of disease screening need to be carefully evaluated before clinical implementation. Various national and international bodies, including the U.S. Preventive Services Task Force (USPSTF), periodically develop recommendations for screening after reviewing the available published evidence and, in some instances, expert opinions. In 1968, Wilson and Jungner formulated a set of 10 rules that must be considered and fulfilled before introducing screening for any disease

Key considerations for who, when, why, and how to screen for malnutrition.

PURPOSE OF REVIEW: Confusion and disagreement about malnutrition screening, how, when, why, who to screen, and which screening tool to use, have persisted since the Global Leadership Initiative on Malnutrition (GLIM) established the malnutrition diagnosis in 2019. In this review, we discuss the purpose of malnutrition screening, presents recent validations of screening tools and how the choice of tools affects malnutrition prevalence within the GLIM framework. RECENT FINDINGS: The screening for malnutrition should consider the results of recent validations of screening tools against the GLIM
