Immediate tissue expander/implast breast reconstruction after salvage mastectomy for cancer recurrence following lumpectomy/irradiation.

BACKGROUND: The objective of this study was to analyze early complications and long-term outcomes in patients undergoing salvage mastectomy and immediate tissue expander/implant reconstruction for cancer recurrence following breast conservation therapy (lumpectomy/irradiation). METHODS: A review of all tissue expander/implant reconstructions performed by a single surgeon over an 11-year period from 1997 to 2008 was performed. Two patient cohorts were identified: (1) patients who underwent salvage mastectomy for a cancer recurrence following prior breast conservation therapy, and (2) patients …

Which eligible breast conservation patients choose mastectomy in the setting of newly diagnosed breast cancer?

PURPOSE: Breast conserving therapy (BCT) and mastectomy offer equivalent survival for women with newly diagnosed breast cancer (BrCa). Despite this, many women eligible for BCT elect mastectomy. Herein, we identify factors associated with choosing ipsilateral mastectomy instead of BCT when mastectomy is not required. METHODS: Between July 2007 and June 2010, 520 women with BrCa were treated by a single surgeon who prospectively documented patient eligibility for BCT. For patients who did not require mastectomy (n=392), we evaluated associations between treatment choice and potential predictor…

Clinical follow-up after bilateral risk reducing ('prophylactic') mastectomy: mental health and body image outcomes.

BACKGROUND: In Manchester, approximately 120 women at > or = 1:4 lifetime risk of breast cancer have considered preventative surgery since 1 … nificant mental health or body image problems in the first 3 years following Bilateral Prophylactic Mastectomy (BPM), but women who have complications warrant additional psychological help. Careful pre-operative preparation and long-term monitoring are advocated in this new field of cancer prevention.

Synthetic Mesh Use in Implant-Based Breast Reconstruction after Mastectomy: What Determines the Surgical Outcome?

INTRODUCTION: Synthetic mesh is frequently used in immediate implant-based breast reconstruction following skin-sparing or nipple-sparing mastectomy to enable immediate reconstruction, though concerns persist regarding associated complications. METHODS: We conducted a retrospective single-center registry study involving 220 consecutive patients, of whom 123 (55.9%) received synthetic mesh: 92 with semi-resorbable (Seragyn®) and 31 with non-resorbable (TiLOOP®) mesh. RESULTS: Postoperative complications occurred in 90 patients (40.9%), with seroma formation as the primary contributor to morbi…

Dual-Plane versus Endoscopic Latissimus Dorsi Muscle Flap Breast Reconstruction after Endoscopic Nipple-Sparing Mastectomy.

BACKGROUND: The authors developed an innovative integrated approach combining reverse-sequence endoscopic nipple-sparing mastectomy with reverse-sequence endoscopic latissimus dorsi muscle flap (R-E-LDMF) harvest for breast reconstruction (BR) through a discreet axillary incision. Both techniques demonstrate clinical feasibility, but no systematic comparison of patient-reported outcomes, complications, and cost efficiency between endoscopic implant-based BR and R-E-LDMF BR exists. METHODS: A prospective, 2-center cohort study was conducted from July of 2021 through March of 2024, enrolling pa…

ASO Author Reflections: Enhancing Technical Reproducibility and Ergonomics in Single-Port Endoscopic Nipple-Sparing Mastectomy.

Minimally invasive nipple-sparing mastectomy (NSM) continues to evolve beyond aesthetic refinement toward technical precision and reproducibility. Although robotic platforms have expanded instrument articulation and visualization, access to such systems remains variable, and pure endoscopic approaches still face ergonomic challenges and steep learning curves. Our recent multimedia article focuses on standardizing key technical steps in single-port endoscopic NSM and explores how enhanced instrument articulation within a confined operative field may support consistent surgical execution. As min…

Factors influencing uptake of risk-reducing mastectomy among unaffected Israeli BRCA1/BRCA2 pathogenic variant carriers.

Women harboring pathogenic variant (PV) in the BRCA1 or BRCA2 genes (= BRCA) have an elevated lifetime risk for breast cancer (BC). One of the main options for active BC risk reduction is bilateral risk-reducing mastectomy (RRM). Understanding the factors influencing that decision is important for genetic-counselling and risk mitigation strategy planning. A structured questionnaire was circulated to BRCA carriers, members of the Good Genes NGO in Israel. Data on RRM uptake and timing, factors previously reported to be associated with decision to undergo RRM (e.g., psychosocial, family history,…

Does Breast Implant Size Larger Than Mastectomy Specimen Size Increase the Risk of Flap Necrosis?

BACKGROUND: Mastectomy skin flap necrosis (MSFN) is a critical postoperation complication in postmastectomy breast reconstruction. High replacement tissue expander fill volume and large mastectomy specimen size are commonly assumed to increase MSFN risk. Thus, a common recommendation in the practicing surgeon's office is not to increase breast volume during reconstruction. However, there is minimal literature on the question of whether postsurgical large or larger breast size is a risk factor for MSFN. OBJECTIVES: This study asked whether risks of MSFN and other complications were increased w…

Intraoperative Prediction of Mastectomy Skin Flap Necrosis using Standardized ICG Grading and Quantitative Perfusion Mapping.

BACKGROUND: Mastectomy skin flap necrosis (MSFN) remains a common complication after immediate breast reconstruction. Indocyanine green (ICG) angiography enables real-time perfusion assessment, but lack of standardized interpretation limits clinical utility. This study evaluates a standardized qualitative grading system combined with quantitative perfusion (QP) mapping for intraoperative risk stratification. PATIENTS AND METHODS: A single-center cohort study was conducted in patients undergoing skin- or nipple-sparing mastectomy with immediate reconstruction. Intraoperative ICG angiography wa…

Prognosis of Implant-Based Breast Reconstruction After Mastectomy Flap Necrosis: Predictors of Failure and Salvage.

BACKGROUND: In the realm of implant-based breast reconstruction, mastectomy flap necrosis (MFN) is a prevalent yet grave complication that poses a threat to the stability of the inserted prosthesis. Although numerous investigations have scrutinized the risk factors for MFN development, few have delved into the aftermath, specifically implant failure or salvage. This study seeks to appraise the prognosis of the implanted prosthesis following MFN occurrence, as well as identify predictors of such outcomes. METHODS: Among patients who underwent immediate implant-based reconstruction between 2010…

Oncoplastic Entirely Robot-Assisted Approach: Incorporating Robotic Surgery in Both Mastectomy and DIEP Flap Reconstruction.

BACKGROUND: Breast cancer surgery involves a minimally invasive approach with the assistance of robotic technology. Robots can be used for harvesting free deep inferior epigastric artery perforator (DIEP) flaps for breast reconstruction to minimize donor-site morbidity. For the first time, the authors' team applied robot-assisted surgery using the oncoplastic entirely robot-assisted (OPERA) approach for both mastectomy and free DIEP flap harvesting. METHODS: A retrospective chart review identified 14 patients with unilateral breast cancer who underwent robot-assisted mastectomy and robot-assi…

Comparative outcomes of prepectoral vs. subpectoral robot-assisted immediate prosthetic reconstruction following nipple-sparing mastectomy.

BACKGROUND: Robot-assisted surgical techniques are increasingly adopted in breast reconstruction, yet the optimal implant placement approach remains debated. Robot-assisted nipple-sparing mastectomy (RANSM) with immediate prosthetic breast reconstruction using the Da Vinci system has shown improved cosmetic outcomes and patient satisfaction. While prepectoral reconstruction offers easier implant placement and fewer complications, subpectoral reconstruction provides better upper pole contours and reduced acellular dermal matrix (ADM) requirements. This study compared surgical and esthetic outco…
