INTRODUCTION TO THE TOPIC: Inverted papilloma is a rare condition of the middle ear. In this paper, the authors present a case report of a patient at a Midwestern health system with inverted papilloma. To supplement the case report, a literature review was also performed to identify clinical trends predisposing such cases to recurrence, malignant transformation, and response to radiation. In addition, the authors also propose a surveillance algorithm derived from this case and previously published surveillance strategies.

BACKGROUND: The application of mastoid pressure dressings following mastoid and middle ear surgery is widely practised to reduce the risk of haematoma and seroma. There are a number of minor morbidities associated with use of the dressings, as well as a financial cost associated with an overnight stay in hospital or a return appointment for removal of the dressings. The benefit of having these dressings in situ overnight is questionable.
METHODS: A retrospective review of 133 patients who had their mastoid dressing removed 2 h after their procedure was undertaken at our Hospital. The patient …

RESULTS: The existence of air cells at the petrous apex, as well as fluid collection in the mastoid antrum detected by a postoperative CT scan, were possible risk factors for CSF leakage. Tracts that directly connected to the antrum from the squamous part of the temporal bone and petrous apex, rather than through numerous air cells, were significa …

INTRODUCTION: Otalgia is a common presenting disorder. We describe a pain entity characterized by localized pain at the anterolateral tip of the mastoid (ATOM), which can be diagnosed by location and response to lidocaine (Xylocaine) injection. Long-term therapeutic results are described following a single injection of lidocaine. We compared 11 patients with this pain location to 24 patients with nonspecific otalgia.
MATERIALS AND METHODS: In a tertiary otology centre, patients with severe ATOM-related pain were treated using 3 cc of 1% lidocaine injection; pain scores were measured before an…

We present a case involving a 36-year-old man who had a feeling that his right ear was obstructed and who had associated conductive hearing loss that was exacerbated by nose blowing. On otoscopy, a soft-tissue swelling was seen in the posterosuperior aspect of the right external auditory canal. Computed tomography revealed the presence of hyperaerated mastoid air cells and an air-containing cavity connected with the enlarged mastoid air cells, narrowing the external auditory canal. A radiographic diagnosis of a mastoid pneumocele with secondary pneumatocele was made. A novel hypothesis is pres…

OBJECTIVES: We recorded and compared the vestibular evoked myogenic potentials (VEMPs) before use of an endolymphatic mastoid shunt (EMS) and 1, 12, and 48 months after placement of the shunt.
METHODS: Air-conducted VEMPs were recorded in 28 patients affected by intractable Meniere's disease and treated with placement of an EMS.
RESULTS: One month and 12 months after the surgery, VEMPs were not detectable in the operated ear in 100% and 86% of the patients, respectively. Forty-eight months after the surgery, they were elicited in 79% of the patients.
CONCLUSIONS: We conclude that VEMPs are …

Facial nerve paralysis developed in a man with tumor-stage mycosis fungoides (MF). Mastoidectomy disclosed that MF had involved the mastoid and middle ear. Meningeal lymphoma, confirmed by the finding of Sézary cells in the CSF, was subsequently established. Autopsy disclosed MF lymphoma in the leptomeninges, medulla, spinal cord, and cranial nerves. A unique feature was the formation of a communicating hydrocephalus. Case reports of 23 patients with MF of the CNS, including 21 autopsies, are reviewed. Practically all had tumor-stage or erythrodermic MF. Atypical mononuclear cells were found a…

Otogenic infections remain a potentially under-recognized cause of pneumococcal meningoencephalitis in adults. In elderly patients, acute mastoiditis may present without overt otological findings ("masked mastoiditis"), delaying diagnosis and increasing the risk of intracranial complications. An 86-year-old unvaccinated male patient with multiple comorbidities initially presented to the emergency department with non-specific upper respiratory symptoms and normal otoscopy and was discharged with presumed viral infection. Four days later, he returned with acute neurological deterioration and hem…

Subdural empyema, mastoiditis, and sinus venous thrombosis are rare infections that may lead to serious and potentially life-threatening complications. A 2-year-old male patient with no known underlying disease presented with a 2-day history of ear pain, sore throat, and fever. Physical examination revealed no significant pathology other than hyperemia of the left tympanic membrane. Intravenous ceftriaxone was initiated, and the patient was hospitalized because of persistent fever and poor oral intake. However, despite 3 days of treatment, the fever persisted, and an increase in acute phase re…

Lymphomas originating in the ear are extremely rare. As with other malignancies of the temporal bone, diagnosis of lymphoma in the middle ear and temporal bone is often delayed, negatively impacting treatment and prognosis. The delay in diagnosis is primarily due to its clinical presentation overlapping with benign infectious conditions of the middle ear. Here, we present a rare case of a 46-year-old woman with prolonged otorrhea, hearing disturbances, intermittent vertigo, and headache. She underwent surgery to investigate suspected chronic otomastoiditis, and subsequent pathological examinat…

PURPOSE: The location of the sigmoid sinus may change according to the ventilation status of the mastoid bone, in which case the relationship of the sigmoid sinus to the facial nerve and semicircular canals is predicted to change. The purpose of this study was to evaluate the importance of sigmoid sinus distances to the facial nerve and semicircular canals concerning mastoid surgery.
METHODS: The relationship of the sigmoid sinus and the facial nerve with the semicircular canals was analyzed from the cone beam computed tomography images of 75 male and 95 female subjects, aged 18-65 years. Fir…
