Middle ear adenoma with facial palsy: A case report and a review of the literature.

A 52-year-old man presented to our emergency department with an acute onset of right-sided facial nerve (FN) palsy of House-Brackmann grade V. Electroneurography (ENoG) was conducted with no response at the right FN, as compared with the left FN (0%). We performed a biopsy of the right middle ear mass and histological studies showed the tumor to be neuroendocrine tumors (NET) of the middle ear. We resected the tumor with canal wall down mastoidectomy and reconstructed the posterior meatal wall with soft tissue. Three months after surgery, the FN paralysis had improved with House-Brackmann grad…

Malignant melanoma of the external auditory canal.

Primary malignant melanoma of the external auditory canal is rarely reported. Malignant melanoma of the ear is estimated to occur in 1-4% of … thought to be an aural polyp in the right ear. Excision of the tumour was accomplished by a radical mastoidectomy. It was confirmed to be malignant melanoma after histopathological examination and Immunohistochemistry. Despite all efforts, the patient succumbed to the disease after receiving three cycles of chemotherapy. Even though this malignancy is rarely fo …

Long-term results of the transattical approach: an alternative technique for cochlear implantation.

Cochlear implantation through the standard posterior tympanotomy approach may sometimes be challenging. In cases with complicated anatomy or malformations, an alternative surgical technique may be chosen. The transattical approach is a simple technique based on a mastoidectomy and a transmeatal cochleostomy. The electrode is driven to the middle ear through the attic. This way it overcomes some of the standard technique's drawbacks. Our aim is to assess the value of this alternative approach in cochlear implant surgery. We conducted a comparative retrospective study; we included 74 patients op…

Temporal bone erosion in patients with chronic suppurative otitis media.

OBJECTIVES: To analyse temporal bone erosion sites (including scutum, labyrinth, facial canal, mastoid tegmen, posterior fossa dural plate and sigmoid sinus plate) in patients with chronic suppurative otitis media (CSOM). METHODOLOGY: Retrospective case review in a tertiary referral centre. Medical records were reviewed from 905 patients (121 complicated; 784 non-complicated) who received a mastoidectomy as a minimum intervention for the treatment of CSOM. RESULTS: All types of temporal bone erosion were found to be more frequent in patients with complicated CSOM. Erosion in the scutum, mast…

Cochlear implant device failure secondary to cholesterol granuloma-mediated cochlear erosion.

STUDY DESIGN: Case report. SETTING: A tertiary care pediatric hospital. PATIENT: A 7-year-old boy complained of retroauricular pain 3.5 years after cochlear implantation. Temporal bone computed tomographic scan revealed a soft tissue density filling the mastoid and middle ear space. There was extensive osseous erosion involving the cochlear promontory that resulted in lateral displacement electrode array into the middle ear space. INTERVENTION: Mastoidectomy with removal of cochlear implant. RESULTS: Pathologic examination of the soft tissue filling the mastoid cavity was consistent with a…

BACKGROUND: Tumours and metastases of the lateral skull base are rare findings. Clinically, the progressive involvement of ipsilateral crani … e lateral skull base. The findings suggested a metastasis of the tracheal carcinoma. We performed a mastoidectomy with specimen collection, and the pathological analysis revealed infiltration of a squamous cell carcinoma. [Recurrent facial paresis with facial neurinoma].

PATIENT: A clinical case of a 49-year-old man is described who presented with a right facial paresis for 4 weeks. The patient had undergone a decompression of facial nerve and a radical mastoidectomy elsewhere in 1998 because of a facial paresis and acute mastoiditis. In the following years a complete right facial paresis occurred several times improving with prednisolone. In addition to topognostic examinations a high resolution computed tomography of the temporal bone was made. CT showed a mass in the right tympanic cavity with close contact to the ossicular chain. Diagnostic tympanotomy dis…

Acute mastoiditis in a Norwegian population: a 20 year retrospective study.

We have retrospectively examined the nature of acute mastioditis (in western Norway) during a 20 year period (1980-2000). Sixty-one cases of AM were identified in 57 patients with a mean age of 3.6 years. We found no significant change in the incidence of AM during the last 20 years. Seven patients were treated solely with intravenous antibiotics and myringotomies. Fifty patients also underwent cortical mastoidectomy, four cases with bilateral surgery. Antibiotic treatment was given to 31 of the patients before admission to hospital and this group had a significant longer duration of symptoms …

[A case of jugular foramen neurinoma originating from glossopharyngeal nerve].

A 39-year-old man was admitted with right hearing loss, tinnitus and vertigo. Neurological examination on admission revealed right facial pa … men. The tumor was resected totally and was approached through a right suboccipital craniectomy and mastoidectomy on July, 1994. Surgery confirmed that the tumor was a neurinoma originating from the glossopharyngeal nerve. After the operation, right facial palsy developed and transient fugitive CSF leakage was observed, but the patient is doing well. There was …

CT detection of facial canal dehiscence and semicircular canal fistula: comparison with surgical findings.

OBJECTIVE: The purpose of this study was to determine the accuracy of high resolution CT (HRCT) in the detection of facial canal dehiscence and semicircular canal fistula, the preoperative evaluation of both of which is clinically very important for ear surgery. MATERIALS AND METHODS: We retrospectively reviewed the HRCT findings in 61 patients who underwent mastoidectomy at Yamagata University between 1989 and 1993. The HRCT images were obtained in the axial and semicoronal planes using 1 mm slice thickness and 1 mm intersection gap. RESULTS: In 46 (75%) of the 61 patients, the HRCT image-b…

A Case of Acute Coalescent Mastoiditis With Early Diagnosis of Lupus Anticoagulant-Hypoprothrombinemia Syndrome Prompting Immediate Surgical Drainage.

Acute coalescent mastoiditis is a severe complication of acute otitis media characterized by bone destruction in the mastoid process and a r … ssessment revealed low bleeding risk, supporting the decision to proceed with surgical drainage and mastoidectomy. The procedure was completed safely, and cultures identified Streptococcus intermedius and Fusobacterium nucleatum as the causative organisms. This case highlights the importance of rapid diagnosis, careful evaluation of bleeding risk, and timely su …

Case Report: Challenging Otologic Surgery in Patients With 22q11.2 Deletion Syndrome.

Patients with 22q11.2 deletion syndrome frequently have conductive hearing loss and/or chronic otitis media. Otologic surgery is often opted for. We present two patients undergoing otologic surgery. This case report outlines the typical otologic surgical challenges in patients with 22q11.2 deletion syndrome. Case one is a 52 year old male patient with chronic otitis media who underwent a mastoidectomy. The pre-operative CT scan showed a fused lateral semicircular canal and vestibule. Peroperatively, the lateral semicircular canal could not be used as a landmark to identify the facial nerve. Ca…

"Mucosal pocket" myringoplasty: a modification of underlay technique for anterior or subtotal perforations.

PURPOSE: Chronic otitis media surgery is the most common procedure in otology in developing countries. Subtotal and total tympanic membrane (TM) perforation with inadequate anterior remnant is associated with higher rate of graft failure. It was the goal of this study to test the anatomical and functional outcomes of a modified underlay myringoplasty technique. MATERIALS AND METHODS: In a prospective clinical study, 45 patients with subtotal or total TM perforation and inadequate anterior remnant underwent tympanoplasty (+/-mastoidectomy).
