Advancing Maternal Health with Long-Acting Therapeutics: Priorities, Efficacy and Safety Considerations, and Emerging Technologies.

Maternal health remains a critical global concern, particularly in underserved populations and in low- and middle-income countries where access to safe and effective therapeutics is limited. Despite the use of medications by most women during pregnancy, the exclusion of pregnant and lactating women from clinical trials has resulted in significant data gaps, hindering informed treatment decisions. As long-acting therapeutics transition into mainstream treatment and prevention strategies, it is critical to ensure these disparities are neither perpetuated nor widened. This review synthesizes insi…

The registration status of maternal, newborn, and child health medical products: evidence from 9 countries.

BACKGROUND: Registration challenges in resource-limited countries can compromise the availability of life-saving maternal, newborn, and child health (MNCH) medical products - quality-assured generic products may not be registered or renewed. METHODS: To identify barriers, we assessed the registration status of MNCH tracer medicines in 9 countries through document review and interviewed country regulators and 11 pharmaceutical manufacturers using structured questionnaires. RESULTS: On average, 72% of the tracer medicines had at least one registered product across the 9 countries (ranging from

Development of an advanced maternal health risk detection system using enhanced XGBOOST and blending models.

OBJECTIVE: Mother's health risks should be identified early so that the outcome of the pregnancy can be enhanced and the complications experienced during pregnancy can be minimized. This paper will design and test a leakage-regulated hybrid machine learning model to predict maternal health risk using the optimized ensemble models. METHODS: It used the publicly available UCI Maternal Health Risk dataset (n = 1015). A fixed random seed was used to stratify the dataset to 80% training and 20% independent testing subsets (42). SMOTENN resampling was only done to the training data to avoid data le…

Placental iron transport under maternal stress: a missing link in foetal programming and mental health.

BACKGROUND: Environmental stress and iron deficiency are increasingly recognised as prevalent challenges during pregnancy, with significant implications for both maternal and foetal health. Environmental stressors such as chronic maternal anxiety can elevate cortisol levels and trigger inflammatory responses which might subsequently disrupt foetal brain development. Concurrently, iron deficiency during critical windows of gestation can hinder the formation of brain structures and neurotransmitter systems vital for emotional regulation and cognitive function after birth. Iron deficiency and exp

Preventing maternal deaths in Somalia requires stronger primary health care and functional emergency referral systems.

Maternal mortality remains a critical public health crisis in Somalia, driven by structural vulnerabilities, low utilization of essential maternity care, and fragmented health service delivery. The vast majority of maternal deaths result from preventable obstetric complications such as hemorrhage, hypertensive disorders, sepsis, and obstructed labor. Current national surveys demonstrate severe gaps across the continuum of maternal care, particularly among rural, pastoralist, and internally displaced populations. Reducing preventable maternal deaths requires moving away from isolated emergency

Maternal cold exposure improves offspring metabolic health via a milk lithocholic acid-microbiota-Th17 axis.

Metabolic diseases are rising with a trend toward earlier onset, yet effective preventive strategies remain limited. While cold exposure improves metabolic health in adults, its role during pregnancy in shaping offspring metabolic outcomes remains unknown. Herein, we demonstrate that maternal cold exposure in early pregnancy markedly improved offspring glucose tolerance, insulin sensitivity, and hepatic lipid metabolism when challenged with a Western diet, and the benefits persisted into late adulthood. Transcriptomic and immunophenotyping analyses revealed that offspring with cold-exposed dam

Women's health and maternal care services: seizing missed opportunities to prevent and manage preterm birth.

PROGRESS: The past ten years have seen uneven developments in women's and adolescents' health and reproductive rights. Globally, reductions of maternal and neonatal mortality rates and adolescent birth rates have been achieved along with improvements in coverage of key reproductive and maternal health services. However, preterm birth rates have not changed significantly. There is still large variation in these rates across the world, with the highest rates occurring in South Asia and sub-Saharan Africa. PROGRAMMATIC PRIORITIES: Effective interventions based on current clinical guidelines are …

Poor maternal health outcomes and associated factors among women undergoing cesarean section in Southern Ethiopia.

BACKGROUND: Cesarean section is the most commonly performed major surgical procedure in obstetrics and gynecology worldwide. Despite advancements in surgical techniques, cesarean section is still associated with higher maternal morbidity and mortality compared to vaginal delivery. Adverse maternal outcomes following cesarean section includes hemorrhage, infections, hysterectomy, uterine rupture, and pronged hospital stay. However, studies on maternal health outcomes following cesarean section are yet well studied in Ethiopia. Therefore, this study aimed to assess poor maternal health outcomes …

Unravelling symptom-specific polygenic effects on maternal mental health during the perinatal period and postpartum.

BACKGROUND: While genetic factors are important influences on maternal mental health, few studies have used symptom-level analyses to examine how genetic liability is related to the experience of specific mental health problems in mothers. A symptom-level approach can account for disorder heterogeneity and delineate key associations between genetic liabilities and mental health. METHODS: Three waves of data (30 weeks of gestation, 6 and 18 months postpartum) from the Norwegian Mother, Father and Child Cohort Study (MoBa) were used to assess item-level associations between genetic liabilities

Metabolomics Signatures and Subsequent Maternal Health among Mothers with a Congenital Heart Defect-Affected Pregnancy.

Congenital heart defects (CHDs) are the most prevalent and serious of all birth defects in the United States. However, little is known about the impact of CHD-affected pregnancies on subsequent maternal health. Thus, there is a need to characterize the metabolic alterations associated with CHD-affected pregnancies. Fifty-six plasma samples were identified from post-partum women who participated in the National Birth Defects Prevention Study between 1997 and 2011 and had (1) unaffected control offspring (n = 18), (2) offspring with tetralogy of Fallot (ToF, n = 22), or (3) hypoplastic left hear…
