Imaging of Odontogenic Sinusitis.

Odontogenic sinusitis (ODS) has been shown to be the most common cause of unilateral maxillary sinus opacification on computed tomography scans. Review of the maxillary alveolar ridge and maxillary tooth roots, and their relationships to the maxillary sinus floor is essential when interpreting paranasal sinus imaging. In recent years, ODS has received increased attention in the recent otolaryngologic and dental literature. This article will discuss technical considerations to optimize imaging of the paranasal sinuses and maxillary dentition, as well as both classic and nuanced radiologic findi

Subdural empyema resulting from displacement of a root into the maxillary antrum.

Subdural empyema is a rare but serious complication of paranasal sinusitis which may result in death or permanent disability in a significant proportion of cases. A case is presented in which displacement of a premolar root in the maxillary sinus led to a subdural empyema and a resultant left-sided hemiplegia. This case illustrates the necessity of early surgical intervention to remove displaced roots in the maxillary antrum in order to prevent the serious complications of paranasal sinusitis.

Use of a Braunula for maxillary sinus irrigation.

Puncture and lavage of the maxillary sinuses is a common procedure in ENT practice. Repeated irrigations are required in chronic sinusitis refractory to treatment. In our department we use a No. 14 gauge Braunula for such cases. Over 100 patients have been treated over the past two years with no significant complications. This device is inexpensive, sterile, safe and easy to use and we recommend its use for patients with chronic maxillary sinusitis requiring repeated irrigations.

Cefaclor vs amoxicillin in the treatment of acute, recurrent, and chronic sinusitis.

BACKGROUND: The treatment of acute, recurrent, and chronic sinusitis remains controversial because of the presence of a wide variety of aerobic and anaerobic bacteria in the sinuses. DESIGN: This double-blind, randomized trial compared cefaclor with amoxicillin in the treatment of acute, recurrent, and chronic maxillary sinusitis using clinical evaluation, roentgenography, and microbiologic evaluation of antral aspirates. SETTING: Outpatient office of five otorhinolaryngologists in Salt Lake City, Utah. PATIENTS: One hundred eight adult patients with acute, recurrent, or chronic maxillary s…

Sinusitis of odontogenic origin.

Odontogenic sinusitis is a well-recognized condition and accounts for approximately 10% to 12% of cases of maxillary sinusitis. An odontogenic source should be considered in individuals with symptoms of maxillary sinusitis with a history of odontogenic infection, dentoalveolar surgery, periodontal surgery, or in those resistant to conventional sinusitis therapy. Diagnosis usually requires a thorough dental and clinical evaluation including appropriate radiographs. The most common causes of odontogenic sinusitis include dental abscesses and periodontal disease that had perforated the Schneidari…

CT features of alveolitis and sinusitis in horses.

Sinusitis is a common disorder in horses and may result from trauma, dental diseases, or space-occupying lesions. Radiography can only provide a limited amount of information. Computed tomography (CT) has been documented as an alternative imaging method. Eighteen horses (mostly Warmblood) with signs of chronic sinusitis were examined preoperatively with CT to assist in diagnosis of the underlying cause. There was a group of common CT features in horses with dental disease and sinusitis. The first molar was the most frequently affected maxillary cheek tooth. Hypoattenuation of the cementum, des

The maxillary sinus and its endodontic implications: clinical study and review.

OBJECTIVES: Endodontic infections of posterior maxillary teeth sometimes spread to the maxillary sinus, generating severe complications. The aim of this study is to present the various problems encountered during endodontic treatment of these teeth. METHODS: The files of 125 cases of odontogenic chronic maxillary sinusitis were reviewed retrospectively. RESULTS: Chronic apical periodontitis was the cause in 99 cases and traumatising endodontic treatment in 26 cases. Foreign intra-sinusal bodies were occasionally seen as a consequence of different endodontic treatments of posterior maxillary …

[Roentgen diagnosis of mucosal cysts of the maxillary sinus].

The most common cystic lesion of the maxillary sinuses is the mucosal cyst. In reviewing a 5-years material of 5026 pathological X-ray examinations of the sinuses 12.2% of these were found to have mucosal cysts. These cysts usually do not give rise to symptoms and require no treatment, but are often erroneously diagnosed as polyps, compact maxillary sinuses, hypertrophy of the lining mucosa or tumors. As radiologic examination of the sinuses is a common procedure and the diagnosis of a mucosal cyst is possible other diagnoses may not be given in most cases.

Evaluation of the microbiology of chronic sinusitis.

Chronic sinusitis is one of the most common diseases treated in outpatient centres. In this prospective study, 49 patients with the diagnosis of chronic maxillary sinusitis were evaluated microbiologically by using sinus swab, irrigation fluid and sinus mucosal tissue specimens obtained during endoscopic sinus surgery. There was no bacterial growth in seven cases. In the remaining 42 cases a total of 89 bacteria were isolated, 28 of them being classical pathogens and 61 being non-classical pathogens. Among the classical pathogens Staphylococcus aureus was the most common one. The correlation b…

[Ultrasonography of the nose and paranasal sinuses].

UNLABELLED: Pathological changes of the sinuses and the nasal bones are rarely examined sonographically. However, both acute and chronic sinusitis in maxillary and frontal sinuses and fractures of the nose could all be diagnosed correctly by diagnostic A-mode ultrasound, which is of clinical importance if children or pregnant patients are examined. The aim of the study was to analyse the reliability of A-mode ultrasonography in diagnosing sinusitis and nasal bone fractures, compared to results of CT, X-ray and clinical symptoms. MATERIAL AND METHODS: 86 patients were included in the study: 20
