Difficult decisions in the intensive care unit: an illustrative case.

Difficult clinical decision-making is a common experience in intensive care units. There is often considerable pressure on time and decision … hich high quality evidence was lacking. Examples of cognitive bias are identified and techniques of metacognition (thinking about thinking) that can be useful in limiting the effects of bias on complex decision-making are reviewed. The intensivist's evaluation of management alternatives has an important role in steering medical management towards optimal patien …

A non-neurodegenerative REM parasomnia with immersive dreaming and dream-reality confusion: a case report.

BACKGROUND: REM sleep behavior disorder (RBD) is characterized by loss of normal muscle atonia during REM sleep, often associated with dream enactment behaviors, and is typically a prodromal neurodegenerative condition in middle-aged and older adults. However, emerging case reports and case series suggest that not all RBD presentations follow this trajectory, particularly in younger individuals. CASE PRESENTATION: A case of 7-year history of vivid, immersive dreaming perceived as continuous with waking life, accompanied by persistent dream-reality confusion, is described. The patient frequent

An Event-Related Potential Study on Differences Between Higher and Lower Easy of Learning Judgments: Evidence for the Ease-of-Processing Hypothesis.

Easy of learning (EOL) judgments occur before active learning begins, and it is a prediction of how difficult it will be to learn new material in future learning. This study compared the amplitude of event-related potential (ERP) components and brain activation regions between high and low EOL judgments by adopting ERPs with a classical EOL judgment paradigm, aiming to confirm the ease-of-processing hypothesis. The results showed that (1) the magnitudes of EOL judgments are affected by encoding fluency cues, and the judgment magnitude increases with encoding fluency; (2) low EOL judgments are

Frontoparietal and Default Mode Network Contributions to Self-Referential Processing in Social Anxiety Disorder.

Social anxiety disorder (SAD) is characterized by negative self-referential processing, which triggers excessive emotional reactivity. In healthy individuals, positive self-views typically predominate and are supported by regions of the default mode network (DMN) that represent self-related information and regions of the frontoparietal control network (FPCN) that contribute to metacognitive awareness and emotion regulation. The current study used functional magnetic resonance imaging (fMRI) to examine patterns of DMN and FPCN activation during positive and negative self-referential judgments i

Measuring Awareness in People With Dementia: Results of a Systematic Scoping Review.

BACKGROUND: Awareness of the diagnosis or related changes in functioning varies in people with dementia (PwD), with implications for the well-being of PwD and their carers. Measuring awareness in a clinical setting could facilitate tailored support and optimize involvement in personal health and care decisions. This scoping review aimed to identify validated methods of assessing awareness in dementia and appraise their clinical utility. METHOD: A systematic search was conducted of English-language publications that measured awareness in PwD, in 6 electronic databases. Search terms included de

[Adaptive behaviour and learning in children with neurodevelopmental disorders (autism spectrum disorders and attention deficit hyperactivity disorder). Effects of executive functioning].

INTRODUCTION: Autism spectrum disorder (ASD) and attention deficit hyperactivity disorder (ADHD) present difficulties in adaptive functioning and learning, possibly associated with failures in executive functioning characteristic of both disorders. AIM: To analyze the impact of executive functioning in the adaptive behaviors of socialization and daily life and in learning behaviors in children with ASD and children with ADHD. SUBJECTS AND METHODS: The participants were 124 children matched in age and intellectual quotient: 37 children with typical development, 52 children with ASD and 35 chi

Dynamics of neural recruitment surrounding the spontaneous arising of thoughts in experienced mindfulness practitioners.

Thoughts arise spontaneously in our minds with remarkable frequency, but tracking the brain systems associated with the early inception of a thought has proved challenging. Here we addressed this issue by taking advantage of the heightened introspective ability of experienced mindfulness practitioners to observe the onset of their spontaneously arising thoughts. We found subtle differences in timing among the many regions typically recruited by spontaneous thought. In some of these regions, fMRI signal peaked prior to the spontaneous arising of a thought - most notably in the medial temporal l

The Impaired Response Inhibition and Salience Attribution Model of Drug Addiction: Recent Neuroimaging Evidence and Future Directions.

Originally postulated in 2001, the impaired response inhibition and salience attribution (iRISA) model of addiction highlights the prefrontal cortex (especially the orbitofrontal, dorsolateral, anterior cingulate, and inferior frontal regions) as central to drug addiction symptomatology. Accordingly, drug cues assume a heightened salience and value that overpower alternative reinforcers, with a concomitant decrease in inhibitory control, especially in a drug-related context. These processes may manifest in metacognitive impairments (e.g., self-awareness of choice), obstructing insight into ill

Mapping the neurocognitive landscape of pediatric Chiari malformation type I: a scoping review of the literature.

Due to advancements in neuroimaging, Chiari malformation type I (CM-I) has become more widely recognized in pediatric populations. While its neurological manifestations are well documented, research on its neurocognitive impact remains limited and poorly characterized. This scoping review facilitates a comprehensive mapping of the current evidence, identifies the neurocognitive domains most frequently assessed, evaluates the extent of available evidence regarding surgical outcomes, and highlights critical knowledge gaps. The most frequently examined domains were working memory, executive funct

No exaggerated tremor severity perception in functional tremor.

BACKGROUND: Symptoms of functional neurological disorder tend to be variable, yet patients often report them being present constantly and of permanently severe intensity. Furthermore, they typically worsen when they are mentioned or during clinical examination. Such phenomena are sometimes interpreted as indicating symptom exaggeration or even fabrication. METHODS: To test the notion of inaccurate symptom perception or reporting, we directly compared subjective to objective tremulousness of reaching movements in people with a functional action tremor, people with an organic action tremor and

Pathways from uncertainty to anxiety: An evaluation of a hierarchical model of trait and disorder-specific intolerance of uncertainty on anxiety disorder symptoms.

Uncertainty is central to anxiety-related pathology and intolerance of uncertainty (IU) appears to be a transdiagnostic risk and maintaining factor. The aim of the present study was to evaluate a hierarchical model to identify the unique contributions of trait and disorder-specific IU (i.e., uncertainty specific to generalised anxiety disorder, social anxiety, obsessive compulsive disorder, and panic disorder) to disorder-specific symptoms, beyond other disorder-specific cognitive vulnerabilities (i.e., negative metacognitive beliefs, fear of negative evaluation, inflated responsibility, and a

PURPOSE: Speech-language pathologists know much more about children's development of fictional narratives than they do about children's development of personal narratives and the role these personal narratives play in academic success, social-emotional development, and self-regulation. The purpose of this tutorial is to provide clinicians with strategies for assessing and developing children's and adolescents' personal narratives. METHOD: This tutorial reviews the literature on (a) the development of autobiographical event narratives and life stories, (b) factors that contribute to developmen

Event-related brain potentials reflect increased concentration ability after mindfulness-based cognitive therapy for depression: a randomized clinical trial.

Mindfulness-based cognitive therapy (MBCT) effectively prevents relapse/recurrence in major depression. The ability to deploy and maintain attention on a particular focus is considered as a prerequisite for 'mindful', 'metacognitive' awareness, and hence crucial for therapy success. Accordingly, sustained concentration is the skill most extensively taught in MBCT. The goal of the present study was to test whether this ability increases after MBCT, as assumed. The late component of the contingent negative variation (CNV), an event-related brain potential (ERP), known to reflect the allocation o

Enhancing orthographic knowledge helps spelling production in eight-year-old Chinese children at risk for dyslexia.

We investigated the effects of enhancing orthographic knowledge on the spelling of Chinese characters and words in 131 eight-year-old Chinese children at risk for dyslexia. The traditional approach (37 children) emphasizing memory and repeated writing was the control condition. The analytic and synthetic approach (ASA, 33 children) stressed insight into character structure. The integrated analytic and synthetic approach added to ASA self-correction and metacognitive activities (INA, 61 children). The children were first asked to write down as many words as possible associated with pictures of

Parent- and self-ratings of executive functions in adolescents and young adults with spina bifida.

This study examined the agreement and consistency of parent- and self-report of executive functioning (EF) (Behavior Rating Inventory of Executive Functions; BRIEF) in an adolescent cohort of youth with myelomeningocele and shunted hydrocephalus (MMH). A total of 30 youth participants with MMH and their parents were recruited during adolescence (age 11-18, mean age 14), and a smaller sample (n = 13) was re-evaluated during young adulthood (age 18-26, mean age 22). Parent- and self-report T-scores were moderately correlated during adolescence (General Executive Composite, GEC, r = .504, p = .00

Pilot study of Mindfulness-Based Cognitive Therapy for trainee clinical psychologists.

BACKGROUND: It is recommended that Mindfulness-Based Cognitive Therapy (MBCT) instructors should undertake MBCT themselves before teaching others. AIM: To investigate the impact of MBCT (modified for stress not depression) on trainee clinical psychologists. METHOD: Twenty trainees completed questionnaires pre- and post-MBCT. RESULTS: There was a significant decrease in rumination, and increases in self-compassion and mindfulness. More frequent home practice was associated with larger decreases in stress, anxiety and rumination, and larger increases in empathic concern. Only first-year train

Stuck in thought: Temporal interplay between cognitive building blocks of rumination.

Theoretical models of repetitive negative thinking suggest that multiple factors contribute to rumination, placing one at risk for depression. However, temporal dynamics of rumination and related cognitive risk factors remain poorly understood. Here, we aimed to model the time-varying associations between depressed mood, metacognitive beliefs, promotion focus, perfectionism, attentional control, and rumination in daily life using an ecological momentary assessment approach (EMA). Participants who had reported experiencing depression (n = 95) completed an intensive EMA procedure (6 times a day

Prospective memory functions in traumatic brain injury: The role of neuropsychological deficits, metamemory and impaired self-awareness.

A large body of evidence suggests that individuals with traumatic brain injury (TBI) have significant difficulties with prospective memory (PM), the memory for future intentions. However, the processes underlying this cognitive deficit remain unclear. This study aimed to gather further evidence regarding PM functions in TBI and clarify the role of neuropsychological deficits, metamemory, and mood disorders. We used a laboratory-based clinical measure, the Virtual Week, to examine PM function in 18 patients with TBI and 18 healthy control subjects. Measures of attention, processing speed, execu

Frontal scalp potentials foretell perceptual choice confidence.

When making decisions, people naturally ask two implicit questions: how soon can I make a decision, and how certain am I? In perception, people's confidence (how certain?) shows a nonmonotonic relationship with response time (how soon?), such that choice confidence can either increase or decrease with response time. Although a frontoparietal network has been implicated as a neural substrate that binds choice confidence and action (e.g., response time), the dynamic interplay between choice behaviors within such a network has not been clarified. Here, we show that frontal event-related potential
