Defining a Willingness-to-transplant Threshold in an Era of Organ Scarcity: Simultaneous Liver-kidney Transplant as a Case Example.

BACKGROUND: Organ scarcity continues in solid organ transplantation, such that the availability of organs limits the number of people able to benefit from transplantation. Medical advancements in managing end-stage organ disease have led to an increasing demand for multiorgan transplant, wherein a patient with multiorgan disease receives >1 organ from the same donor. Current allocation schemes give priority to multiorgan recipients compared with single-organ transplant recipients, which raise ethical questions regarding equity and utility. METHODS: We use simultaneous liver and kidney (SLK) t

Dynamic neural circuit disruptions associated with antisocial behaviors.

Antisocial behavior (ASB) is believed to have neural substrates; however, the association between ASB and functional brain networks remains unclear. The temporal variability of the functional connectivity (or dynamic FC) derived from resting-state functional MRI has been suggested as a useful metric for studying abnormal behaviors including ASB. This is the first study using low-frequency fluctuations of the dynamic FC to unravel potential system-level neural correlates with ASB. Specifically, we individually associated the dynamic FC patterns with the ASB scores (measured by Antisocial Proces

An international comparison of deceased donor kidney utilization: What can the United States and the United Kingdom learn from each other?

In transplant, meaningful international comparisons in organ utilization are needed. This collaborative study between the United Kingdom (UK) and the United States (US) aimed to develop a kidney utilization metric allowing for legitimate intercountry comparisons. Data from the UK and US transplant registries, including all deceased donor kidneys recovered from 2006 to 2017, were analyzed. To identify a potentially comparable kidney utilization rate (UR), several denominators were assessed. We discovered that the proportion of transplanted kidneys from elderly donors in the UK (10.7%) was 18 ti

Comparing Traditional Service Delivery and Telepractice for Speech Sound Production Using a Functional Outcome Measure.

PURPOSE: Using American Speech-Language-Hearing Association's (ASHA's) National Outcomes Measurement System (NOMS) Functional Communication Measure (FCM) as a common metric, this investigation compared traditional service delivery and telepractice service delivery for children receiving therapy for the NOMS diagnostic category of "speech sound production." METHOD: De-identified cases were secured from ASHA's NOMS database and a proprietary database from a private e-learning provider. Cases were included if they met 3 criteria: (a) children received treatment exclusively for speech sound produ

Application of the limited-memory quasi-Newton algorithm for multi-dimensional, large flip-angle RF pulses at 7T.

OBJECTIVE: Ultrahigh field MRI provides great opportunities for medical diagnostics and research. However, ultrahigh field MRI also brings challenges, such as larger magnetic susceptibility induced field changes. Parallel-transmit radio-frequency pulses can ameliorate these complications while performing advanced tasks in routine applications. To address one class of such pulses, we propose an optimal-control algorithm as a tool for designing advanced multi-dimensional, large flip-angle, radio-frequency pulses. We contrast initial conditions, constraints, and field correction abilities against

Scoring system for periventricular leukomalacia in infants with congenital heart disease.

BACKGROUND: Currently two magnetic resonance imaging (MRI) methods have been used to assess periventricular leukomalacia (PVL) severity in infants with congenital heart disease: manual volumetric lesion segmentation and an observational categorical scale. Volumetric classification is labor intensive and the categorical scale is quick but unreliable. We propose the quartered point system (QPS) as a novel, intuitive, time-efficient metric with high interrater agreement. METHODS: QPS is an observational scale that asks the rater to score MRIs on the basis of lesion size, number, and distribution

[3-dimensional documentation of wound-healing].

The objective evaluation of the course of wound-healing represents a substantial parameter for the quality assurance of a modern wound management in chronic wounds. Established procedures exclusively based on a two-dimensional measurement of the wound surface with planimetry or digital photo documentation in combination with a metric statement of size. Thus so far an objective method is missing for the evaluation of the volumes of chronic wounds. By the linkage of digital photography, optical grid by means of digital scanner and an image processing software in co-operation with the company RSI

State-based prognostics with similarity enhancement for CT X-ray tubes using operational logs.

OBJECTIVES: The tube is a core component of the CT system and is characterized by high cost and limited lifespan. This study aims to develop a method for estimating the remaining useful life (RUL) of CT X-ray tubes without requiring a preset failure threshold or prior knowledge of tube life. METHODS: To reduce the impact of varying scan conditions, different scan types are first analyzed, and the one with the most stable tube current distribution is selected. Then, a composite metric is applied to degradation parameters to obtain a candidate health indicator (HI). Next, the candidate HI is no

Cardiovascular reserve as an integrative physiological construct: Current evidence, emerging technologies and future directions.

Cardiovascular reserve represents the integrated physiological capacity of the cardiovascular system to maintain tissue perfusion during increasing physiological stress. Although reserve has long been recognized conceptually, its direct assessment remains challenging. No universally accepted definition or gold-standard measure of cardiovascular reserve currently exists. Nevertheless, if cardiovascular reserve can be estimated reliably, reserve-based monitoring could permit earlier recognition of impending circulatory deterioration before conventional physiological indicators become abnormal. A

Dynamics of botnet propagation model in complex networks considering hybrid method for botnet detection.

In this paper, a dynamic epidemic model of botnet attack propagation in scale-free networks is introduced based on the epidemic model. The proposed attack propagation model is based on the Susceptible-Exposure-Infected-Improved-Vaccinated-Recovery (SEIRVS) epidemic model. Here, an Intrusion Detection System (IDS) for botnet attack detection is also presented. This method is based on a combination of machine learning and metaheuristic algorithms, the Golden Ratio Optimization (GRO) algorithm, Bat Algorithm (BA), and K-Nearest Neighbor (KNN) algorithms named (GRO-BA-K-NN), which includes three s

Descriptor-Guided Design of High-Efficiency Narrowband Phosphorescent Probes for Bioimaging.

Organic room-temperature phosphorescence (ORTP) with narrow emission bands is highly attractive for high-contrast biomedical imaging. However, most ORTP systems suffer from low efficiency due to weak spin-orbit coupling, and their emission spectra are often broadened by strong electron-vibration coupling. Herein, we propose a descriptor-guided design framework to achieve efficient and narrowband ORTP through intramolecular noncovalent conformational locks (NoCLs). Two fundamental descriptors are established to link structure to photophysical properties: the NoCL strength S, a rigidity metric t

Systematic Characterization of Transmon Qubit Stability with Thermal Cycling.

The temporal stability and reproducibility of qubit parameters are critical for the long-term operation and maintenance of superconducting quantum processors. In this work, we present a comprehensive longitudinal characterization of 27 frequency-tunable transmon qubits spanning over one year across four thermal cycles. Our results establish a distinct hierarchy of stability for superconducting hardware. We find that the intrinsic device parameters determining the qubit frequency and the baseline energy relaxation times (T1) exhibit high robustness against thermal stress, characterized by frequ

Unraveling the Metabolic-Cardiovascular Disease Nexus: An Epidemiological Network Analysis Identifies Central Indicators.

Cardiovascular diseases (CVD) remain a leading cause of death globally, yet assessing the combined impact of multiple metabolic disorders on CVD risk is challenging due to a lack of comprehensive tools. We aimed to develop and validate a novel CVD screening model based on metabolic clustering networks. Using data from 4066 adults enrolled in the STONE study (China) through stratified sampling, we constructed a holistic network map of metabolic health, incorporating obesity, dyslipidemia, glucose disorders, and hepatic, renal, thyroid, and bone conditions. Through network analysis, 12 central a

CD4/CD8 ratios as immune architects - building defenses against HIV: A narrative review.

The CD4/CD8 ratio is a pivotal metric in assessing the status of the immune system, crucially implicated in human immunodeficiency virus (HIV) infection dynamics. This review elucidates the multifaceted roles of CD4 and CD8 T cells in immune surveillance and response orchestration. We explore the intricate mechanisms governing their equilibrium and the consequential impact on immune homeostasis. Moreover, we dissect how HIV undermines this delicate balance, precipitating a cascade of immune dysregulation culminating in AIDS progression. Insights into the CD4/CD8 ratio's prognostic value for di

Classification accuracy of pain intensity induced by leg blood flow restriction during walking using machine learning based on electroencephalography.

Pain assessment in clinical practice largely relies on patient-reported subjectivity. Although previous studies using fMRI and EEG have attempted objective pain evaluation, their focus has been limited to resting conditions. This study aimed to classify pain levels during movement using a wearable device with three forehead electrodes and advanced machine learning. Twenty-five healthy participants performed walking tasks under tourniquet-induced pain. It was confirmed that pain increased as walking time extended. Walking time was used as an index of pain stimulus intensity, and EEG data were c
