The Chronic Effects of Neurotrauma Consortium (CENC) multi-centre observational study: Description of study and characteristics of early participants.

PRIMARY OBJECTIVES: To establish and comprehensively evaluate a large cohort of US veterans who served in recent military conflicts in order to better understand possible chronic and late-life effects of mild traumatic brain injury (mTBI), including those that may stem from neurodegeneration. RESEARCH DESIGN: Cross-sectional and prospective longitudinal. METHODS AND PROCEDURES: Inclusion criteria are prior combat exposure and deployment(s) in Operation Enduring Freedom, Operation Iraqi Freedom or one of their follow-on conflicts (collectively OEF/OIF). Effects of mTBI will be assessed by enr

Social Causation Versus Social Erosion: Comparisons of Causal Models for Relations Between Support and PTSD Symptoms.

Social support is a robust correlate of posttraumatic stress disorder (PTSD) symptoms and of general psychological distress (Ozer, Best, Lipsey, & Weiss, 2003). The nature of the causal relationship between support and PTSD remains the subject of debate, with 2 models, social erosion and social causation, often used to explain findings. Despite extensive research using these models, no studies of which we are aware have included tests of both models within the same series of analyses, across more than 2 time points, in veterans. These competing models were tested in a sample of National Guard

Can we solve the mysteries of the National Vietnam Veterans Readjustment Study?

The National Vietnam Veterans Readjustment Study (NVVRS) researchers reported that 30.9% of all men who served in that conflict developed posttraumatic stress disorder (PTSD) even though only about 15% had been assigned to combat units. Scholars, mainly historians, have questioned the accuracy of the PTSD prevalence rate. The purpose of this article is to evaluate the merits of several hypotheses adduced to explain the high apparent PTSD prevalence in the NVVRS. Empirical and conceptual analysis suggests that malingering is unlikely to account for many cases. Also, deployment to Vietnam in the

Long-Term Postdeployment Clinical Subtypes of Risk and Resiliency in Brain Injury and Neurodegeneration.

IMPORTANCE: While research has focused on veterans after deployment, fewer studies have examined longitudinal outcomes associated with neurodegeneration. Questions remain about how these trajectories may connect and inform overall risk for later-life neurodegeneration and possible dementia. OBJECTIVE: To analyze acute, 1-year, 5-year, and 10-year longitudinal outcomes and magnetic resonance imaging (MRI) data from US military personnel to understand profiles of neurodegenerative resilience and risk. DESIGN, SETTING, AND PARTICIPANTS: This prospective longitudinal, cohort study included data

Advocacy for Change: An Osteopathic Review of Traumatic Brain Injury Among Combat Veterans.

As a "signature injury" of the Iraq and Afghanistan wars, traumatic brain injury (TBI) remains a major health concern among military service members. Traumatic brain injury is associated with a wide range of symptoms which may be cognitive, emotional, psychological, biochemical, and social in nature. Mild TBI (mTBI) ranks as the most common traumatic brain injury among veterans. Due to the absence of specific symptoms, mTBI diagnosis may be challenging in acute settings. Repetitive traumatic brain injury during combat deployments can lead to devastating chronic neurodegenerative diseases and o

The Impact of Aeroallergens on Military Readiness: A Case Report.

Seasonal aeroallergens commonly cause allergic conjunctivitis, rhinorrhea, sinusitis, and cough in sensitized individuals. These clinical symptoms are the result of IgE-mediated type I hypersensitivity reactions, which trigger the degranulaton of mast cells and basophils. In contrast, aeroallergens are not common precipitants of urticarial dermatitis, which is driven by a cell-mediated type IV hypersensitivity reaction. In this case report, we describe an Active Duty Air Force member stationed in San Antonio, TX, who presented to the dermatology clinic with a three year history of recalcitrant

Interpersonal and Trauma-Related Guilt moderate the relationship between intensity of combat experiences and suicidality.

Suicide rates in military-affiliated communities remain elevated since the dawn of the Global War on Terror, despite substantial efforts by clinicians and researchers. While some risk factors have been identified, mixed results need to be clarified. The current study builds on previous research by testing a structural equation model of suicide risk associated with combat experiences that by incorporates risk factors with the most empirical support (combat experiences, guilt, PTSD, depression, and the Interpersonal Psychological Theory of Suicide [IPTS] factors of Perceived Burdensomeness, Thwa

Potential Biomarkers for Asymptomatic Visceral Leishmaniasis among Iraq-Deployed U.S. Military Personnel.

Visceral leishmaniasis (VL) is a chronic infection caused by Leishmania (L.) donovani or L. infantum parasites. Despite having the infection, most individuals never develop the clinical disease and are able to control the parasite and remain asymptomatic. However, some progress to symptomatic VL, leading to death if untreated. The host immune response has a major role in determining the progression and severity of the clinical manifestations in VL; several immune biomarkers of symptomatic VL have been described with interferon-gamma release as a surrogate biomarker of host cellular immunity. Accuracy of vaginal symptom self-diagnosis algorithms for deployed military women.

BACKGROUND: Deployed military women have an increased risk for development of vaginitis due to extreme temperatures, primitive sanitation, hygiene and laundry facilities, and unavailable or unacceptable healthcare resources. The Women in the Military Self-Diagnosis (WMSD) and treatment kit was developed as a field-expedient solution to this problem. OBJECTIVES: The primary study aims were to evaluate the accuracy of women's self-diagnosis of vaginal symptoms and eight diagnostic algorithms and to predict potential self-medication omission and commission error rates. METHOD: Participants incl

Increase in Neck and Back Pain in Fighter Pilots After the Introduction of the F-35 Platform.

INTRODUCTION: Fighter pilots face unique occupational challenges, including exposure to high G-forces in suboptimal seating postures and wearing heavy equipment around neck and head. These factors contribute to a high prevalence of neck and back complaints, which can impair flight performance and prevent deployment of pilots. METHODS: A cross-sectional survey was performed on the self-reported prevalence of neck and back pain among F-16 and F-35 pilots in the Royal Netherlands Air Force. Of the 121 pilots who were invited to participate, 50% (N = 61) were included. The questionnaire was based

Particle Analysis by Scanning Electron Microscopy With Energy Dispersive X-Ray Analysis in Pulmonary Pathology Specimens From U.S. Military Service Members Deployed During the Global War on Terror 2002 to 2015.

INTRODUCTION: Between 2001 and 2015, 2.77 million U.S. military service members completed over 5 million deployments to Southwest Asia. There are concerns that deployment-related environmental exposures may be associated with adverse pulmonary health outcomes. Accurate pulmonary diagnosis often requires histopathological biopsy. These lung biopsies are amenable to chemical analysis of retained particulates using scanning electron microscopy with energy dispersive X-ray analysis (SEM/EDXA). METHOD: A retrospective review of SEM/EDXA data collected in conjunction with pathologic diagnostic cons

Risk and protective factors associated with mental health among female military veterans: results from the veterans' health study.

BACKGROUND: This study focuses on factors that may disproportionately affect female veterans' mental health, compared to men, and is part of a larger study assessing the prevalence of mental health disorders and treatment seeking among formerly deployed US military service members. METHODS: We surveyed a random sample of 1,730 veterans who were patients in a large non-VA hospital system in the US. Based on previous research, women were hypothesized to be at higher risk for psychological problems. We adjusted our results for confounding factors, including history of trauma, childhood abuse, co

Family Resilience Matters: How Individual and Family Resilience Improve Health Outcomes for Veterans with Complex Conditions.

INTRODUCTION: Veterans with traumatic brain injury (TBI), epilepsy, and other complex comorbidities often face increased risks of long-term physical and mental difficulties. Prior research has highlighted benefits of individual resilience among Veterans with respect to maintaining positive health outcomes in the face of stress and adversity. However, 1 area that has been underexplored is how family resilience may also contribute to better well-being post-deployment, especially when facing complex conditions like TBI or other post-traumatic sequelae. Considering the efforts of the Department of

Enhancing Combat Surgical Readiness: The Development and Impact of the Combat Craniomaxillofacial Trauma Surgery Course.

INTRODUCTION: Craniomaxillofacial (CMF) trauma is significantly rising from drone delivered blasts in military and civilian settings. CMF traumatic injuries from airburst blasts present uniquely complex surgical challenges. The abrupt transmission and dissipation of energy causes barotrauma to the orbit, paranasal sinuses, tympanic membrane, inhalation injury, crush injuries, fractures and dislocations. High velocity projectiles cause deep lacerations, comminuted fractures, embedded foreign bodies, avulsion, degloving and other complex wounds. Surgical management is complicated by contaminatio

Incidence and Predictors of Postpartum Depression Diagnoses among Active-Duty U.S. Army Soldiers.

Introduction: Understanding the incidence and predictors of postpartum depression (PPD) among active-duty service members is critical given the importance of this population and its unique stressors. Methods: We conducted a retrospective cohort study of all active-duty U.S. Army soldiers with a record of at least one live-birth delivery between January 2012 and December 2013. Multivariate logistic regression models were used to estimate associations between demographic, health-related, and military-specific variables and diagnoses of PPD in the total population (N = 4,178) as well as in a subp
