Genetic characteristics and targeted treatments of primary bladder and urachal adenocarcinomas: a systematic review with pooled descriptive genomic analyses.

Adenocarcinomas are rare histological subtypes of bladder cancer mainly presenting as primary bladder adenocarcinomas (PBACs), or urachal carcinomas (UrCs). The lack of standardized, evidence-based clinical recommendations for therapy results in individualized treatment decisions. These personalized treatments may be tailored based on molecular analyses to target driver mutations. The genetic background of UrC and PBAC has been increasingly explored through molecular analysis of relatively small case series. Similarly, experiences with targeted treatments are available from individual case rep

CD45 as a multifunctional target in immune regulation and therapeutic delivery.

BACKGROUND: CD45, a receptor-type protein tyrosine phosphatase broadly expressed on leukocytes, is a critical regulator of signaling thresholds and immune cell activation. Dysregulation of CD45 has been implicated in a range of immune-related diseases and malignancies. MAIN BODY: Recent advances have focused on modulating CD45 for disease treatment through various strategies, including regulating its expression level, structural conformation, and spatial membrane distribution. Beyond the immunomodulatory role, as a leukocyte-specific and broadly expressed biomarker, CD45 also has broad applic

Roles of inflammation and metabolic dysfunction in cardiovascular diseases: molecular mechanisms and therapeutic targets.

Cardiovascular disease progression is shaped by a close interaction between metabolic adaptation and inflammatory signaling. In injured cardiac and vascular tissues, ischemia, pressure overload, lipid stress and metabolic excess reshape how cardiomyocytes, endothelial cells, immune cells and fibroblasts sense damage and communicate with their microenvironment. Cardiovascular inflammation follows a staged tissue response, from danger-signal sensing and nuclear factor-κB-related priming to inflammasome activation, cytokine amplification, leukocyte recruitment, efferocytosis and chronic remodelin

The prostatic acid phosphatase in prostate cancer: A novel theranostic target.

Prostate-specific membrane antigen (PSMA)-targeted theranostics have transformed the diagnostic and therapeutic landscape of prostate cancer; however, clinically relevant limitations persist, including heterogeneous or absent PSMA expression in a substantial subset of tumors and dose-limiting off-target uptake in salivary glands, lacrimal glands and kidneys. Prostatic acid phosphatase (ACP3), expressed in more than 95% of prostate cancers, has re-emerged as a promising complementary and potentially alternative theranostic target. ACP3 demonstrates favorable biological characteristics, includin

[Therapeutic Targets by Pathology AI and Spatial Transcriptomics in Breast Cancer].

Recent advances in pathology foundation models have markedly improved the accuracy and generalizability of histopathological image analysis in breast cancer. However, the mechanisms of resistance to CDK4/6 inhibitors in hormone receptor-positive, HER2-negative advanced breast cancer remain incompletely understood. This article outlines a strategy to identify therapeutic targets by integrating pathology AI with spatial transcriptomics. We developed AI-directed spatial transcriptomics (AID-ST), a framework that compares gene expression profiles between drug-sensitive and drug-resistant regions i

Targeting of kinases to treat neurodegenerative diseases.

Neurodegenerative diseases, including Alzheimer, Parkinson, and multiple sclerosis, represent a growing global health crisis with limited therapeutic options that address disease progression. Protein kinases, which are crucial regulators of diverse cellular processes such as endolysosomal trafficking, neuroinflammation, and mitochondrial homeostasis, are frequently dysregulated in these conditions, making them attractive drug targets. This review explores the therapeutic potential of targeting key kinases implicated in neurodegeneration, specifically p38 MAPK, BTK, c-Abl/ABL1, CDK5, GSK3, JNK,

Tumor Necrosis Factor: A Master Regulator in Inflammation and Disease-Pathways, Implications, and Breakthrough Therapies.

Tumor necrosis factor-alpha (TNF-α), a crucial proinflammatory cytokine, is involved in immune regulation and in the pathogenesis of a variety of chronic inflammatory and autoimmune diseases. TNF-α signaling dysregulation is linked to chronic inflammatory diseases, multiorgan dysfunction, and tissue destruction through activation of TNF receptor 1 (TNFR1)-mediated inflammatory, apoptotic, and necroptotic pathways, while TNF receptor 2 (TNFR2) stimulation promotes immune regulation and tissue repair. Understanding these divergent signaling mechanisms is crucial for augmenting therapeutic interv

Precision Oncology in Gastrointestinal and Colorectal Cancer Surgery.

Precision medicine is used to treat gastrointestinal malignancies including esophageal, gastric, small bowel, colorectal, and pancreatic cancers. Cutting-edge assays to detect and treat these cancers are active areas of research and will soon become standard of care. Colorectal cancer is a prime example of precision oncology as disease site is no longer the final determinate of treatment. Here, the authors describe how leveraging an understanding of tumor biology translates to individualized patient care using evidence-based practices.

The changing therapeutic landscape of small-cell lung cancer.

Small-cell lung cancer (SCLC) is an exceptionally aggressive malignancy: highly proliferative, heterogeneous, and frequently metastatic at diagnosis. Although initially responsive to chemotherapy, such responses are typically transient, and recurrent disease has been largely refractory to standard cytotoxics. The past decade has been notable for major advances in our understanding of SCLC biology, and this preclinical progress is now informing multiple novel therapeutic approaches for this disease. Of particular note, defining cell-surface proteins that are uniquely or differentially expressed

Therapeutic targeting of the protein tyrosine kinase-7 in cancer: an overview.

The protein tyrosine kinase-7 (PTK7) is an evolutionarily conserved transmembrane receptor that has emerged as a potential therapeutic target for human tumors. PTK7 is a pseudokinase that is involved in the modulation of the Wnt signaling pathway through interactions with other receptors. These interactions result in targeted gene activation that regulates cell polarity, migration, and proliferation during embryogenesis. Aside of this role during development, PTK7 has been shown as overexpressed in numerous cancers including colon carcinoma, leukemia, neuroblastoma, hepatoma, and ovarian cance

The Revolution of Targeted Therapies in Thyroid Cancer Treatment: Present and Future Promising Anti-Cancer Drugs.

Thyroid cancer prevalence has increased in the last few decades. Whereas the majority of well-differentiated histotypes have effective therapeutic options, the most advanced cases lacked successful treatment until recent years. Genomic alterations have emerged as targets for new anti-cancer drugs. This molecular knowledge is gradually being translated into sophisticated approaches for the stratification, management, and therapies of patients with thyroid carcinomas. The genomic characterisation of tumours in clinical assistance serves as a tool for enhancing the prognostic assessment of patien

Thyroid Cancer: Epidemiology, Classification, Risk Factors, Diagnostic and Prognostic Markers, and Current Treatment Strategies.

Thyroid cancer (TC) invariably remains the most prevalent endocrine cancer in the world. Major histological forms of TC include papillary (PTC), follicular (FTC), medullary (MTC), and anaplastic thyroid carcinoma (ATC), each of which has a unique clinical and molecular profile. The incidence rate of TC is higher in females, and unfortunately, it has tended to increase over the last several years. Yet the treatment of advanced or aggressive TC forms has improved recently because of developments in immunotherapy and targeted medicines, including PD-1 inhibitors and tyrosine kinase inhibitors (e.

From "lysosomal addiction" to targeted therapies: exploiting novel windows in colorectal cancer.

Patients with advanced-stage or treatment-resistant colorectal cancer (CRC) face limited therapeutic options with conventional therapies, necessitating novel treatment strategies. CRC cells exhibit "lysosomal addiction" through enhanced autophagy-lysosomal flux and upregulated lysosome-associated genes including lysosomal-associated membrane protein 1/2 and cathepsin B/D, creating therapeutic windows for selective targeting. Lysosome-dependent cell death represents a distinct mode of regulated cell death characterized by lysosomal membrane permeabilization and cathepsin-mediated cytotoxic casc

Advances in Cardiovascular Pharmacotherapy. V. Molecular Targets in Transthyretin Amyloid Cardiomyopathy.

Transthyretin amyloid cardiomyopathy (ATTR-CM) is an infiltrative disease that occurs when the tetrameric transthyretin complex dissociates into its constituent monomers, which then misfold, aggregate, and accumulate as amyloid fibrils in the myocardial extracellular matrix. The amyloid deposits thicken and stiffen the myocardium, interfering with cardiac function and leading to the development of heart failure. Chronic exposure to cytotoxic circulating amyloid precursors contributes to further myocardial damage. Once considered rare, ATTR-CM is now recognized as the most common form of cardia

Targeting the serotonergic system in the treatment of neurodegenerative diseases-emerging therapies and unmet challenges.

More than 65 million people worldwide experience neurodegenerative diseases, such as Alzheimer disease, frontotemporal dementia, Parkinson disease, and amyotrophic lateral sclerosis. As the risk of developing these diseases increases with age, increasing life expectancy will further accelerate their prevalence. Despite major advances in the understanding of the molecular mechanisms of neurodegeneration, no curative therapy is available to date. Neurodegenerative diseases are known to be associated with alterations in serotonergic neurotransmission, which might critically contribute to the path

The metabolism-immunity axis in Immune thrombocytopenia: from energy regulation to targeted therapy.

Immunometabolism has received significant interest as a field exploring the dynamic interation between the immune system and metabolic processes. Immune cell activation, proliferation, and function are tightly regulated by metabolism, and metabolic products influence immune responses. These two aspects interact to maintain internal homeostasis. Disruption of immunometabolism plays a crucial role in the onset and progression of autoimmune diseases. Metabolic abnormalities may cause abnormal activation and differentiation of immune cells, leading to attacks on self-tissues. Immune thrombocytopen
