Does more generous health insurance reimbursement lead to moral hazard? Evidence from large-scale administrative data in China.

This paper exploits the discrete increase in reimbursement rates at the statutory retirement age as a quasi-natural experiment to examine ex post moral hazard in inpatient care in China. Using nationally representative administrative data covering more than 450,000 hospitalization records across 29 provinces, we implement a cohort difference-in-differences design to identify the causal effect of insurance generosity on healthcare utilization. We find that a 5-percentage-point increase in reimbursement rates increases length of stay by 1.73 days and total inpatient expenditures by RMB 709.98. T

Depth-oriented group psychotherapy for moral injury with military veterans: relational psychoanalytic and psychodynamic theory, mechanisms of action, and clinical implications.

Some veterans are haunted by memories of action they have taken or betrayals they have experienced that violated deeply held moral beliefs; these experiences can lead to moral injury. We have developed a depth-oriented group psychotherapy for U.S. combat veterans, to address moral injury. Depth psychotherapy is an evidence-based form of psychoanalysis; the treatment we have developed is based on Relational psychoanalysis. The aim is for the group members to each develop an organized narrative about morally injurious events and their impact on their current lives to facilitate psychosocial reco

Weighing first- and second-hand information: How children and adults evaluate sources of moral and factual knowledge across two cultures.

How do children and adults decide when to think for themselves and when to rely on others? In two cross-cultural experiments (N = 424), I ask whether children and adults value independent thinking over dependence on testimony when evaluating how others justify moral and empirical beliefs. In Study 1, 4- and 6-year-old children and adults in the United States and China evaluated two reasoners who made the same moral judgments (e.g., pushing others is wrong), but one showed independence in her thinking, and the other relied on another person's testimony. Adults in both cultures preferred the ind

Implementing a Cognitively Grounded Artificial Moral Advisor: A Multi-LLM Multi-Agent Approach Based on the Cognitive-Reflective Equilibration Model.

Large language model (LLM)-based artificial intelligence is increasingly used in ethically consequential human decision-making, yet fully autonomous machine ethics remains unrealistic, motivating architectures that support rather than replace human ethical judgment. This study introduces the Cognitive-Reflective Equilibration Architecture (CREA), a cognitively grounded artificial moral advisor that operationalizes the Cognitive-Reflective Equilibration Model (CREM), in which reflective reasoning guides ethical judgment from intuitive cognition toward a more advanced equilibrium among competing

An Examination of the Association Between Callous-Unemotional Traits, Mechanisms of Moral Disengagement and Offending Behaviour Among a Sample of Young Serious Offenders.

BACKGROUND: Research has shown that young people with elevated callous-unemotional traits are also likely to be morally disengaged and may justify their offending behaviour. Moral disengagement may, thus, explain the cognitive processes leading to their offending. Moral disengagement, however, is a multifaceted construct, and research has yet to examine the role of its distinct components in the callous-unemotional traits/offending pathway. AIMS: This study aims to explore associations between callous-unemotional traits and components of moral disengagement in young offenders by testing a med

Bridging the Phenomenological Gap: Body Mapping as an Art-Based Reflection on Nursing Students' Moral Distress and Embodied Empathy During Simulation.

The primary objective of this qualitative study was to explore the phenomenological experiences of nursing students following a high-fidelity labor and delivery simulation, utilizing body mapping as an art-based reflective intervention. While simulation education bridges theoretical knowledge and clinical practice, it exposes students to psychological stressors, vicarious trauma, and moral distress that traditional cognitive debriefing cannot adequately resolve. Thirty-four nursing students participated in the study, creating visual artifacts and completing in-depth interviews. Data were analy

When dark triad rules the moral: How empathy mediates idealism and relativism- a latent profile and pathway analysis in medical training.

Personality factors, empathy and ethical perspectives are key factors in understanding medical students and healthcare professional's framework toward better patient care, medical communication and quality of life. One of these factors are Psychopathy, Machiavellianism, Narcissism and also Idealism and Relativism, however there is limited data evaluating these. The aim of this study was therefore to deeply investigate the links between Dark Traits and Ethics Positioning focusing on the mediating role of Empathy among medical students. Participants (N = 116, M age = 19,13 years, 70,68 % female)

Mitigating moral distress by enhancing healthcare workers' understanding of challenges faced by carers of children with disabilities in low-resource settings in Kenya.

BACKGROUND: Little is known about the psychological wellbeing and the potential moral distress faced by female carers of children with disabilities living in low-resource settings in East Africa. In such environments, caregiving often requires resilience and resourcefulness, yet can also increase the vulnerability of caregivers and their children. OBJECTIVE: The objective of this study is to identify factors affecting female caregivers' psychological well-being, and to suggest ways healthcare workers can support these caregivers' psychological well-being to alleviate moral distress. METHODS:

Comparison of psychopathology, purpose in life and moral courage between nursing home and hospital healthcare workers during the COVID-19 pandemic.

The COVID-19 pandemic deeply affected healthcare workers, although the impact may have differed according to different workplace contexts. The aim of this current research was to compare the psychopathology presented by hospital versus nursing home healthcare workers during the COVID-19 pandemic and to analyse the predictive role of purpose in life and moral courage in the appearance of psychopathology. This was an observational, cross-sectional study carried out on a sample of 108 healthcare workers, 54 each from a hospital or nursing homes, who were recruited during the 5 and 6th waves of th

Preventing burnout from moral distress among prehospital emergency personnel through action research and targeted clinical ethics support.

Ethical challenges are integral to health care and are associated with moral distress among health professionals. Moral distress can turn into burnout with a range of negative effects for professionals, patients, relatives, collaborators, and the organisation. Based on action research a focus group study was conducted inclucing prehospital personnel from the emergency services in the Region of Southern Denmark. Results showed that ethical challenges arise in contexts of (1) caring for patients, (2) managing organisational demands, (3) collaborating. Ethical challenges are addressed informally

"Syndemic moral distress": sexual health provider practices in the context of co-occurring, socially produced sexual and mental health epidemics.

BACKGROUND: 'Syndemic' refers to socially produced, intertwined, and co-occurring epidemics. Syndemic theory is increasingly used to understand the population-level relationships between sexual health (including HIV) and mental health (including problematic substance use) epidemics. Syndemic-informed clinical interventions are rare. METHODS: We therefore asked 22 sexual health practitioners from six sexual health clinics in British Columbia, Canada to define the word 'syndemic' and then asked how the theory related to their clinical practice. RESULTS: Responses to syndemic theory ranged wide

Moral Distress and Perceived Community Views Are Associated with Mental Health Symptoms in Frontline Health Workers during the COVID-19 Pandemic.

BACKGROUND: Sudden changes in clinical practice and the altered ability to care for patients due to the COVID-19 pandemic have been associated with moral distress and mental health concerns in healthcare workers internationally. This study aimed to investigate the severity, prevalence, and predictors of moral distress experienced by Australian healthcare workers during the COVID-19 pandemic. METHODS: A nationwide, voluntary, anonymous, single time-point, online survey of self-identified frontline healthcare workers was conducted between 27th August and 23rd October 2020. Participants were rec

Does naloxone provision lead to increased substance use? A systematic review to assess if there is evidence of a 'moral hazard' associated with naloxone supply.

BACKGROUND: Take home naloxone (THN) programs have been rapidly upscaled in response to increasing opioid-related mortality. One often cited concern is that naloxone provision could be associated with increased opioid use, due to the availability of naloxone to reverse opioid overdose. We conducted a systematic review to determine whether THN provision is associated with changes in substance use by participants enrolled in THN programs. METHODS: We conducted a systematic review of the literature to assess changes in heroin or other substance use by people who use opioids following THN provisi

Frequency of use, moral incongruence and religiosity and their relationships with self-perceived addiction to pornography, internet use, social networking and online gaming.

BACKGROUND AND AIMS: Moral incongruence involves disapproval of a behavior in which people engage despite their moral beliefs. Although considerable research has been conducted on how moral incongruence relates to pornography use, potential roles for moral incongruence in other putative behavioral addictions have not been investigated. The aim of this study was to investigate the role of moral incongruence in self-perceived addiction to: (i) pornography; (ii) internet addiction; (iii) social networking; and (iv) online gaming. DESIGN: A cross-sectional, preregistered, online survey using mult

What messages can foster safer sex among young women? Experimental evidence concerning the role of emotions and moral norms.

Through a 2 × 2 × 2 quasi experimental design (N = 254), this research investigated if a social campaign eliciting positive emotions and activating moral norms might enhance condom negotiation skills, intended and estimated condom among young women with or without past sexual experience with casual partners. Emotions had a main effect on one of the six condom negotiation strategies we considered; for most of the other variables an interaction effect with moral norms and/or past behaviour emerged. Concerning estimated condom use, positive emotions worked better than negative ones when moral nor
