Late-onset Acute Cellular Rejection after Lung Transplantation Triggered by a Fluctuation in the Tacrolimus Trough Level Which Was Successfully Treated with Steroid Pulse Therapy.

Acute cellular rejection (ACR) usually develops within the first year of lung transplantation. We report a case of biopsy-proven ACR occurring 18 months post-transplant that was successfully treated with steroid pulse therapy and intensified immunosuppression. Onset followed azithromycin and clofazimine for Mycobacterium abscessus, which caused the tacrolimus trough levels to drop. The patient presented with dyspnea, diffuse ground-glass opacities, and reduced lung function. Although C4d staining was positive, the absence of donor-specific antibodies and histological findings supported the dia…

The trend and the disease prediction of vascular endothelial growth factor and placenta growth factor in nontuberculous mycobacterial lung disease.

Nontuberculous mycobacteria (NTM)-lung disease (LD) is an increasing health problem worldwide. The diagnosis of this disease remains difficult, however the application of placenta growth factor (PlGF) and vascular endothelial growth factor (VEGF) has not yet been studied. We screened patients with Mycobacterium avium complex or M. abscessus isolated from sputum, and enrolled 32 patients with NTM-LD and 93 with NTM pulmonary colonization. The NTM-LD group had a lower body mass index, higher proportion of bronchiectasis, more respiratory symptoms and pulmonary lesions, and higher titers of sputu

Rapidly growing mycobacterial infections after pedicures.

BACKGROUND: Rapidly growing mycobacteria (RGM) can cause a variety of cutaneous and systemic diseases. The causative organisms are typically Mycobacterium fortuitum or Mycobacterium chelonae (also known as Mycobacterium abscessus). Primary cutaneous lesions may develop after a variable latent period, from weeks to several months, and usually result from direct inoculation after trauma, from injections, or during surgery via contaminated medical instruments. Recently, investigators from the Centers for Disease Control and Prevention, Atlanta, Ga, and the California Department of Health Services…

Cochlear Implant Complicated by Nontuberculous Mycobacteria Infection: Report and Literature Review.

Objective: Nontuberculosis mycobacteria (NTM) infection of cochlear implants are exceedingly rare. Here, we report one such case and review the literature surrounding previous reports. Methods: Case report. Case Report: A 76-year-old female underwent right cochlear implantation. Her course was complicated by wound dehiscence, three surgical debridements, and ultimately explantation. Cultures ultimately grew Mycobacterium abscessus, which was effectively treated with Azithromycin, Omadacycline, and 3 months of injectable Cefoxitin. At the latest follow-up, the patient is without evidence of fur…

Drug susceptibility testing of rapidly growing mycobacteria in extrapulmonary tuberculosis.

There has been an increasing awareness of the rapidly growing mycobacteria (RGM), of which numerous species and phylogenetic groups are clearly established human pathogens. It is important to appropriately distinguish RGM from other mycobacteria, as first-line antituberculous drugs are ineffective for their treatment. Variability in susceptibility of RGM is seen in relation to species, different geographical areas, and time. Therefore, we conducted a study to speciate the isolates of RGM and perform antimicrobial susceptibility testing. The study was carried out in the department of microbiolo

Beyond the Needle: Unmasking Atypical Mycobacteria in Postvaccination Abscesses in Children.

INTRODUCTION: Atypical or nontuberculous mycobacteria (NTM) are an environmental organism responsible for opportunistic infection. Rapid-growing NTM are more commonly associated with hospital-acquired infections. Many of the organisms responsible for diseases in immunocompromised patients and hospital-acquired infections originate from tap water, such as Mycobacterium kansasii, Mycobacterium xenopi, Mycobacterium gordonae, Mycobacterium simiae, Mycobacterium mucogenicum, Mycobacterium fortuitum, Mycobacterium chelonae, and Mycobacterium abscessus. NTM is a rare organism responsible for the inj…

Exposure of Mycobacteriodes abscessus clones to mucin affects bacterial phenotype.

In the past 20 years infections caused by Mycobacterioides abscessus have become increasingly common in patients with chronic lung conditions. The microorganisms are also resistant to a number of antibiotic classes, making treatment challenging. To begin understanding how the bacterium adapts to the lung environment, pure colonies of M. abscessus strain 19,977 were grown in 7H9 broth with or without mucin over 30-day intervals for 6 months and analyzed for colony morphology, 7 day-growth curves, the ability to form biofilms after 14 days, and the susceptibility to antibiotics determined using

Intra-abdominal Abscess Caused by Mycobacteroides abscessus in a Peritoneal Dialysis Patient: A Case Report.

A 57-year-old man was diagnosed with peritoneal dialysis (PD)-associated peritonitis 2 months after surgery for tunnel infection caused by Mycobacteroides abscessus (M. abscessus). The patient was treated with multiple antibiotics, and the cell count in the PD effluent decreased. However, the patient experienced abdominal pain and developed hiccups. Computed tomography revealed an encapsulated fluid collection, indicating intra-abdominal abscess (IAA) formation. Percutaneous drainage was performed to treat IAA, and the catheter was removed. This case suggests that PD-associated peritonitis cau

Challenges in diagnosing bovine tuberculosis through surveillance and characterization of Mycobacterium species in slaughtered cattle in Kolkata.

BACKGROUND: Tuberculosis in cattle is caused by Mycobacterium tuberculosis complex (MTBC) species. Apart from MTBC, different Nontuberculous Mycobacteria (NTM) species have also been isolated from cattle. The presence of NTM infection in bovines makes the diagnosis of bovine tuberculosis (bTB) a cumbersome task. Therefore, a cross sectional study was conducted to isolate and characterize different Mycobacterium spp. from a slaughterhouse situated in Kolkata, a city in the eastern part of India. RESULTS: Out of 258 morbid samples, 98 isolates were found to be positive for bacterial growth, and

Rapid identification of mycobacteria to the species level by polymerase chain reaction and restriction enzyme analysis--a case report of corneal ulcer.

We report a case of corneal ulcer caused by nontuberculous mycobacteria which was confirmed by smear and culture. We attempted a new method for the rapid identification of mycobacteria to the species level on the basis of evaluation by the polymerase chain reaction of the gene encoding. The method is involved with restriction enzyme analysis of PCR product obtained with primers common to all mycobacteria. Using the restriction enzyme Bst EII and Hae III, clinically relevant and other frequent laboratory isolates were differentiated to the species or subspecies level by PCR-restriction enzyme a

[The Prevalence of Nontuberculous Mycobacterial Lung Disease with orwithout Reflux Esophagitis].

BACKGROUND/AIMS: Gastrointestinal reflux disease (GERD) is defined as 'a condition that develops when the reflux of stomach contents causes troublesome symptoms and/or complications'. It is associated with various lung diseases, including bronchial asthma, chronic bronchitis, and bronchiectasis. GERD may also affect nontuberculous mycobacterial (NTM) lung disease. We presumed that the normal mucosal barrier of the bronchial epithelium is broken due to the aspiration of gastric juice and consequent chronic bronchial inflammation. This study investigated the prevalence of GERD in accordance with
