OBJECTIVES: To determine our complication rate in pediatric femoral shaft fractures treated with flexible elastic nailing and to determine fracture characteristics that may predict complications.
DESIGN: Retrospective cohort study.
SETTING: One Level 1 and One Level 2 academic trauma centers.
PATIENTS/PARTICIPANTS: One hundred one pediatric femoral shaft fractures treated from 2006 to 2018.
MAIN OUTCOME MEASUREMENT: Major and minor complications.

A finite element analysis was performed to evaluate the stresses around nails and cortical bones in subtrochanteric (ST) fracture models fixed using short cephalomedullary nails (CMNs). INTRODUCTION: The insertion of 2 elastic stable intramedullary nails (ESINs) is a common treatment for pediatric femur fractures. However, the use of this technique in length-unstable or metadiaphyseal fractures has historically been associated with higher complication rates. To improve stability, the addition of a third ESIN has been assessed biomechanically and clinically, but the addition of a fourth nail has only been evaluated biomechanically. Dynamisation of a previously interlocked intramedullary nail is believed to stimulate an osteogenic response due to increased load across the fracture site. We report 15 years follow-up of a girl born with nails hypothrophy and primary hypoaldosteronism. The failure to thrive, troubles with feeding and vomiting were observed since the first weeks of life. The results of the laboratory examination confirmed isolated hypoaldosteronism. BACKGROUND: Ipsilateral femoral neck and shaft fractures are rare injuries and the treatment is complicated and versatile. No single device has been considered absolutely superior to others.
METHODS: Fifteen combined fractures in 15 patients were treated with Russell-Taylor reconstruction intramedullary nails (recon nails). Twelve surgeries were performed within 24 hours of trauma and the other 3 were delayed for 4-7 days due to associated life-threatening injuries. We report three cases of fracture of the humerus that occurred at the tip of interlocking humeral nails inserted previously for humeral shaft fractures. The fractures occurred at 8, 10, and 26 weeks postoperatively, and all occurred through the distal interlocking screws after the patient sustained a rotational force to the arm. All three patients had further surgery, two subsequently healed their fractures, and one is still in active treatment. Orthopaedists should be aware of this complication when choosing interlocking humeral nails for the treatment of humeral shaft fractures.

The upper extremities of two elderly cadavers were amputated at the fore-quarter level and underwent placement of the Biomet uniflex humeral nail in the left extremities and the Richards Russell-Taylor humeral nail in the right extremities.
