Klebsiella pneumoniae in neonatal sepsis: a 3-year-study in the pediatric hospital of Tabriz, Iran.

Neonatal sepsis is a life-threatening emergency, and any delay in treatment may cause death. Because of the importance of the problem in Iran, the aim of this retrospective study was to determine the etiological agents of neonatal septicemia, and the prevalence and epidemiology of Klebsiella bacteremia in the neonatal wards. Two hundred and ten cases of neonatal sepsis occurred during the study period. The most common organism was coagulase-negative staphylococci. Gram-negative organisms were isolated in 66 cases (31.43%), and the most common Gram-negative organism causing neonatal sepsis was …

Fungal and bacterial sepsis and threshold ROP in preterm very low birth weight neonates.

OBJECTIVE: To determine whether an association exists between either fungal or bacterial sepsis and retinopathy of prematurity (ROP). STUDY DESIGN: Retrospective cohort study on all neonates with birth weight <1500 g admitted to a large Italian third Level Neonatal Intensive Care Unit in the years 1997-2001 and screened for ROP. Univariate analysis and multiple logistic regression were used to detect significant associations with ROP (all grades and threshold) in neonates with birth weight<1000 g (extremely low birth weight (ELBW)) and 1000-1500 g. RESULTS: Among 301 enrolled neonates, ROP (

Neonatal nosocomial sepsis in a level-III NICU: evaluation of the causative agents and antimicrobial susceptibilities.

Despite advances in supportive care and use of antibiotics, sepsis preserves its importance due to its high mortality and morbidity for neonates. Identifying the causative agents and antibiotic resistance yearly in a neonatal intensive care unit (NICU) helps the physician to choose the most appropriate empirical therapy. In this study we aimed to evaluate positive blood cultures and antibiotic susceptibilities of newborns with proven sepsis during the years 2000-2002 in our NICU. The charts of babies with sepsis were evaluated for clinical characteristics, positive cultures and antimicrobial s

Simple clinical criteria to identify sepsis or pneumonia in neonates in the community needing treatment or referral.

BACKGROUND: Sepsis, meningitis and pneumonia annually kill 1.1 million neonates in developing countries; most deaths occur at home. OBJECTIVES: To develop simple clinical criteria, enabling health workers in communities to identify neonates with potentially fatal sepsis; and to identify the danger signs alerting mothers to seek care. METHODS: In a field trial in 39 villages in Gadchiroli, India, trained health workers visited all neonates at home 8 times during the first 28 days of life, recording signs and outcome without interventions during 1995-1996 and with home-based management of sick

Trends in the epidemiology of neonatal sepsis of vertical transmission in the era of group B streptococcal prevention.

AIM: To assess trends in the epidemiology of culture-proven and clinical neonatal sepsis of vertical transmission in the era of intrapartum antibiotic prophylaxis. METHODS: Since 1995, the neonatal services of 28 acute-care teaching hospitals in Spain ("Grupo de Hospitales Castrillo") have been involved in the prospective surveillance of neonatal infection of vertical transmission. We here report the comparison of the incidence for the periods 1996-1997 and 2000-2001, and look separately at two groups of hospitals according to the time at which the official hospital policy to provide intrapar…

The effect of E coli virulence on bacterial translocation and systemic sepsis in the neonatal rabbit model.

In the surgical neonate, three factors that promote bacterial translocation and systemic infection are: (1) intestinal bacterial colonization and overgrowth; (2) compromised host defenses; and (3) disruption of the mucosal epithelial barrier. The newborn rabbit provides an excellent model to study these factors. Like the human, there is early closure of the gut mucosa to macromolecules, and nutrition can be maintained by breast or formula feeding. This study examines translocation and systemic sepsis after colonization with virulent K1 and avirulent K100 strains of Escherichia coli. New Zealan

Organism- and Mortality-Specific Cardiorespiratory Patterns in Late-Onset Neonatal Sepsis: A Four-Neonatal Intensive Care Unit Study of Very Low Birth Weight Infants.

Late-onset neonatal sepsis (LOS) in very low birth weight (VLBW) preterm infants varies in severity from mild to fatal and often presents with cardiorespiratory instability. We hypothesized that heart rate (HR) and systemic oxygenation (SpO2) patterns would differ based on pathogen type and mortality. We analyzed HR and SpO2 changes before LOS diagnosis for Gram-negative, Coagulase-negative Staphylococcus, and other Gram-positive bacteria (GN, CONS, OGP) and compared survivors and nonsurvivors. Using continuous every-2-second vital sign data from 365 VLBW infants in four neonatal intensive care…

Long-lasting effects of control measures on trends in incidence in neonatal late-onset sepsis due to multiresistant bacteria in a Brazilian neonatal unit.

BACKGROUND: In response to a 1995 outbreak of sepsis caused by multidrug-resistant (MR) Gram-negative bacteria (GNB), a Brazilian level III neonatal unit established a series of control and prevention measures. This study evaluated the long-term effects of these measures on late-onset neonatal sepsis (LONS) caused by MR bacteria from 2000 to 2020 and examined their impact on in-hospital mortality. METHODS: Newborns with LONS and positive cultures for Staphylococcus aureus, GNB, and Enterococcus sp were selected, adhering to Center for Desease Control and Prevention and local criteria. Clinical utility of the inflammatory burden index in differentiating neonatal pneumonia from pneumonia complicated by sepsis.

OBJECTIVE: Differentiating sepsis from uncomplicated pneumonia in neonates remains clinically challenging. The inflammatory burden index (IBI) is a novel composite biomarker integrating inflammatory status and immune function. This study aimed to evaluate the diagnostic utility of the IBI in differentiating neonatal pneumonia with and without concomitant sepsis. METHODS: We conducted a retrospective analysis of 1,383 hospitalized neonates, stratified into a pneumonia group (n = 935) and a pneumonia-complicated-by-sepsis group (n = 448). Clinical and laboratory data, including C-reactive prote

N-Acetylcysteine protects the developing brain in neonatal sepsis-like inflammation via a redox-neurovascular pathway.

BACKGROUND: Neonatal sepsis can disrupt brain development through oxidative stress, blood-brain barrier (BBB) dysfunction, peripheral leukocyte infiltration, and white matter injury. N-acetylcysteine (NAC), a glutathione precursor with antioxidant and immunomodulatory properties, is a promising neuroprotective candidate, but its effects in neonatal sepsis-like brain injury remain incompletely defined. The purpose of this study was to investigate whether early NAC pretreatment, followed by continued treatment, was associated with protection in a neonatal LPS model and examine the principal mech…

Lactic Acid Bacteria-derived Bacteriocins: A Promising Antimicrobial Strategy against Multidrug-resistant for Neonatal Sepsis Pathogens.

Neonatal sepsis continues to pose rising critical challenges to global health, particularly in low and middle-income countries, where it accounts for a substantial proportion of neonatal morbidity and mortality. The increasing prevalence of multidrug-resistant (MDR) pathogens in neonatal sepsis has significantly weakened the efficacy of conventional antibiotics, necessitating urgent exploration of alternative antimicrobial therapies for better clinical outcomes. Emerging research interest is growing to develop the microbial-derived peptides as novel antimicrobial agents, particularly "bacterio…

Contemporary Evidence on Interhospital Neonatal Transport as a Potentially Sepsis-Relevant Care Interval: A Scoping Review.

Background and Objectives: Neonatal sepsis is time-sensitive, yet the role of interhospital transport in its early care pathway is poorly characterised. This scoping review systematically identified and classified contemporary neonatal transport variables, interventions, and organisational practices potentially relevant to early sepsis care, examined the evidentiary proximity of the available literature to neonatal sepsis, and identified priorities for direct investigation. Materials and Methods: PubMed/MEDLINE, Embase, Scopus, Web of Science, and the Cochrane Library were searched for English…
