Standardized Placental Pathology Reporting: Improving Quality and Clinical Utility Recommendations From the Society for Pediatric Pathology Placental Pathology Reporting Task Force.

BACKGROUND: Placental pathology has benefited greatly from the standardization of definitions provided by the Amsterdam consensus, however t … tion of definitions. Input from pathologists from different practice settings, obstetric providers, neonatologists, and patient advocates was integrated. CONCLUSIONS: The introduction of a standardized placental reporting template holds the potential to reform and advance placental pathology practice across the diverse range of settings in which it is currently …

Correlation of Transfontanel Ultrasonography and Brain Magnetic Resonance Imaging Measurements in Neonates with Hydrocephalus.

AIM: To evaluate whether transfontanel ultrasonography could serve as a practical and less complex alternative to brain magnetic resonance imaging in infants with hydrocephalus. MATERIAL AND METHODS: In this prospective study, 54 infants diagnosed with hydrocephalus underwent both transfontanel ultrasonography and brain magnetic resonance imaging. A neonatologist and a radiologist independently assessed ventricular measurements, including the Evans index, frontooccipital horn ratio, bilateral ventricular index, anterior horn width, thalamooccipital distance, callosal angle, and corpus callosu

Immediate Kangaroo Mother Care for Sick Newborns: Quality Improvement Implementation and Feasibility Study.

BACKGROUND: A systematic separation of late preterm and/or sick newborns from their mothers and families continues to occur in most neonatal units around the world. OBJECTIVE: The aim of this study was to implement immediate kangaroo mother care for sick newborns from birth. METHODS: This study comprised preterm newborns at ≥34 weeks of gestational age and weighing ≥1800 g, term newborns with noninvasive ventilation, and newborns at ≥34 weeks of gestational age and weighing ≥1800 g with congenital anomalies that did not require urgent medical or surgical attention. Newborns were excluded in

The "Breastfeeding Paradox" as a Guide for the Assessment of Premature Infants Growth: It Is More Than Just Weigh-Ins.

One of the neonatologist's greatest challenges is ensuring that the premature infant has adequate growth and is provided with a correct supply of nutrients. Thanks to the establishment of the INTERGROWTH-21st Preterm Postnatal Growth Standards, created longitudinally and prospectively on healthy premature babies, it now appears evident that preterms' growth follows a pattern different from that of a fetus of the same gestational age. In addition to growth, defined solely as weight gain, further significance must be given to the quality of growth, that is, lean mass apposition. This should be e

Neonatal Outcomes of Term Infants Born with Meconium-Stained Amniotic Fluid.

Background Meconium-stained amniotic fluid (MSAF) is considered an alarming sign of possible fetal compromise and it has recently been reported that neonatal outcome correlates with the degree of meconium thickness. Methods We retrospectively studied 400 term infants allocated in clear amniotic fluid and grade 1, 2, and 3 MSAF groups on the basis of color and thickness of AF. Multivariable logistic regression analysis was performed to evaluate the potential independent effect of delivery with MSAF of different severity on the risk of a composite adverse neonatal outcome. Results We found that

A 62-Year-Old Man Presenting with Bilateral Tremor of the Upper Limb After a Diagnosis of COVID-19 with Confirmed Volumetric Brain Loss.

There is no separate course in the medical curriculum summarizing all aspects of human reproduction in most medical school curricula. At the same time, such a course would logically connect knowledge from clinical embryology and assisted reproduction, encompassing the issue of female and male infertility, mechanisms of birth defect formation, their prenatal diagnosis and subsequent specialized neonatal care. The aim of a wide team of university teachers comprising embryologists, gynecologists, neonatologists, endocrinologists, geneticists and others was to create and implement a new course ent…

Critical Care Management of a Severe Acute Respiratory Distress Syndrome COVID-19 Patient With Control Cesarean Section.

We share our experience of one 29-year-old female, G2 P1, with acute respiratory distress syndrome (ARDS) and at 30 weeks of pregnancy. The 30-week gravid uterus in combination with a poor ventilation-perfusion ratio creates a restrictive lung pattern that may prove to be lethal for both the mother and baby. Due to her rapid deterioration and increased hemodynamic instability we opted for controlled delivery in the operating room with an ICU physician, a Neonatologist, and an Obstetric team. At 3.27 minutes from induction, the baby was born with Apgar scores of 7 and 8. The mother was placed o

Updated Australian consensus statement on management of inherited bleeding disorders in pregnancy.

INTRODUCTION: There have been significant advances in the understanding of the management of inherited bleeding disorders in pregnancy since the last Australian Haemophilia Centre Directors' Organisation (AHCDO) consensus statement was published in 2009. This updated consensus statement provides practical information for clinicians managing pregnant women who have, or carry a gene for, inherited bleeding disorders, and their potentially affected infants. It represents the consensus opinion of all AHCDO members; where evidence was lacking, recommendations have been based on clinical experience

Pragmatic approach to intersex, including genital ambiguity, in the newborn.

The evaluation and management of a newborn with ambiguous genitalia must be undertaken as quickly as possible and with great sensitivity for the child's family. Where possible, a comprehensive team approach with a pediatric urologist, endocrinologist, geneticist, neonatologist, and child psychiatrist/psychologist should work closely with the family to establish the diagnosis and determine gender. Although the preferred gender assignment is not always clear, a thorough examination of endocrine function, karyotype, and potential for fertility should guide the determination. While some disorders

Contributions to the dynamics of cervix remodeling prior to term and preterm birth.

Major clinical challenges for obstetricians and neonatologists result from early cervix remodeling and preterm birth. Complications related to cervix remodeling or delivery account for significant morbidity in newborns and peripartum mothers. Understanding morphology and structure of the cervix in pregnant women is limited mostly to the period soon before and after birth. However, evidence in rodent models supports a working hypothesis that a convergence of factors promotes a physiological inflammatory process that degrades the extracellular collagen matrix and enhances biomechanical distensib…

Three patients presenting with severe macrosomia and congenital hypertrophic cardiomyopathy: a case series.

BACKGROUND: Macrosomia and hypertrophic cardiomyopathy are two features often associated in neonates of diabetic mothers. We report the case … ophic cardiomyopathy in neonates. These three original observations indicate that gynecologists and neonatologists should pay attention to neonates from mothers with a high body mass index and when maternal diabetes is not documented.

Therapeutic apheresis in pregnancy: General considerations and current practice.

It is widely known that pregnancy does not represent a contraindication to therapeutic apheresis (TA) techniques. In fact, since the first experiences of TA in pregnancy for the prevention of hemolytic disease of the newborn, several diseases are at present treated with TA, mainly within 6 clinical categories: (a) TA is a priority and has no alternative equally effective treatment (e.g., thrombotic thrombocytopenic purpura); (b) TA is a priority but there are alternative therapies not contraindicated in pregnancy (e.g., myasthenia gravis); (c) TA is an effective tool of saving/avoiding drugs c

Fetal surgery: principles, indications, and evidence.

Since the first human fetal surgery was reported in 1965, several different fetal surgical procedures have been developed and perfected, resulting in significantly improved outcomes for many fetuses. The currently accepted list of fetal conditions for which antenatal surgery is considered include lower urinary tract obstruction, twin-twin transfusion syndrome, myelomeningocele, congenital diaphragmatic hernia, neck masses occluding the trachea, and tumors such as congenital cystic adenomatoid malformation or sacrococcygeal teratoma when associated with developing fetal hydrops. Until recently,
