Anatrophic nephrolithotomy and pyelolithotomy for dual staghorn calculi in bifid collecting system.

A bifid ureter is an atypical anatomical variation that occurs with an incidence of 1-10%. This anomaly is in a continuum of duplex collecting systems and most commonly involves a common distal ureter. This is usually asymptomatic and is predominantly an incidental diagnosis, nevertheless, is a potential risk factor for urolithiasis formation. Current surgical management of larger staghorn calculi favours percutaneous nephrolithotomy (PCNL) over traditional open surgery, however for multiple calculi and complex anatomy PCNL would require multiple punctures, with increased risk of bleeding, ple

Supine mini-PCNL using pneumatic lithotripsy for pediatric renal stones: safety and efficacy in low-income countries.

BACKGROUND: Mini-percutaneous nephrolithotomy (mini-PCNL) is an effective surgical technique for pediatric renal stones. However, data comparing different lithotripsy modalities in this population are limited. PURPOSE: To compare the efficacy, safety, and acute renal parenchymal impact of pneumatic lithotripsy versus Holmium: YAG (Ho: YAG) laser dusting during supine mini-percutaneous nephrolithotomy (mini-PCNL) in pediatric patients. PATIENTS AND METHODS: A double-masked, randomized clinical trial was conducted between March 2023 and October 2024. A total of 104 pediatric patients (ages 3-1

Efficacy of combined rigid and flexible ureteroscopy for complex ureteral stones in type 2 diabetes mellitus.

BACKGROUND: Type 2 Diabetes Mellitus (T2DM) patients with complex ureteral stones face significant challenges in terms of treatment outcomes, including higher risk for complications such as sepsis and prolonged recovery times. The efficacy of combined rigid and flexible ureteroscopy in managing these stones in T2DM patients remains underexplored. METHODS: A retrospective cohort study was conducted at our hospital from January 2021 to August 2024. The study included patients aged 18-65 years diagnosed with T2DM and complex ureteral stones (size >1 cm, multiple stones, or those in difficult-to-

Nonopioid Pain Management Pathways for Stone Disease.

Introduction: New opioid dependency after urologic surgery is a serious adverse outcome that is well-described in the literature. Patients with stone disease often require multiple procedures because of recurrence of disease and hence are at greater risk for repeat opioid exposures. Despite this, opioid prescribing after urologic surgery remains highly variable and in an emergency setting, opioids are still used commonly in management of acute renal colic. Methods: Two literature searches were performed using PubMed. First, we searched available literature concerning opioid-sparing pathways in

Ureteroscopy for treatment of upper urinary tract stones in children: technical considerations.

The incidence of pediatric urolithiasis is increasing. While many smaller stones may pass spontaneously, surgical therapy is sometimes warranted. Surgical options include shock wave lithotripsy, ureteroscopy, percutaneous nephrolithotomy, and open surgery. Ureteroscopy represents a minimally invasive approach, and it is increasingly being used to treat pediatric upper tract calculi. Ureteroscopy is performed under anesthesia and fluoroscopic guidance, with basket extraction or lithotripsy of the calculi. Technical considerations include active or passive ureteral dilatation, the use of uretera

Transdiaphragmatic nephropleural fistula: A rare cause of empyema thoracic managed with single-stage surgical approach.

Empyema thoracic arising from a retroperitoneal source via a transdiaphragmatic nephropleural fistula is among its exceptionally rare causes. This report describes successful management with single-stage combined thoracic and urological surgery. A young female with long-standing nephrolithiasis and recent percutaneous nephrolithotomy, presented with respiratory distress. Initial management included emergent tube thoracostomy for a gross right pyopneumothorax confirmed by computed tomographic (CT) scan of chest. Escherichia coli was cultured from pleural fluid and urine samples. Further contras

Venous Air Embolism: A Rare but Possible Complication During Retrograde Intrarenal Surgery-A Case Report and Review of Literature.

Background: Venous air embolism (VAE) during various urologic surgeries such as transurethral resection of prostate, percutaneous nephrolithotomy (PCNL) and occasionally while performing air pyelogram during PCNL have been reported in the literature. Case Presentation: In this study, we present a case of 34 year-old gentleman who developed intraoperative VAE during retrograde intrarenal surgery (RIRS). The clinical suspicion and diagnosis were made by fall in end-tidal carbon dioxide and oxygen saturation, transient hypotension, and bradycardia. The patient was managed conservatively. The most

A comparison among PCNL, Miniperc and Ultraminiperc for lower calyceal stones between 1 and 2 cm: a prospective, comparative, multicenter and randomised study.

BACKGROUND: Conventional Percutaneous Lithotripsy (PCNL) has been an effective, successful and easy approach for especially > 1 cm sized calyceal stones however risks of complications and nephron loss are inevitable. Our aim is to compare the efficacy and safety of PCNL, MiniPerc (MP) and UltraMiniPerc (UMP) for lower calyceal stones between 1 and 2 cm with a multicenter prospective randomized study. METHODS: Between January 2015 and June 2018, 132 consecutive patients with single lower calyceal stone were enrolled. Patients were randomized in three groups; A: PCNL; B: MP; C: UMP. 44 patients

The heat is on: the impact of excessive temperature increments on complications of laser treatment for ureteral and renal stones.

OBJECTIVE: Technological advancements in the field of urology have led to a paradigm shift in the management of urolithiasis towards minimally invasive endourological interventions, namely ureteroscopy and percutaneous nephrolithotomy. However, concerns regarding the potential for thermal injury during laser lithotripsy have arisen, as studies have indicated that the threshold for cellular thermal injury (43 °C) can be exceeded, even with conventional low-power laser settings. This review aims to identify the factors that contribute to temperature increments during laser treatment using curren

[Treatment of renal and ureteral lithiasis].

The advent of extracorporal shock wave lithotripsy has entirely changed management of patients with renal or ureter calculi. It is indicated in all types of calculi and generally used as first line treatment. Contraindications are exceptional: pregnancy, uncorrectable coagulation disorders. For staghorn calculi in the kidney, lithotripsy alone is only effective in 50% of the cases and is often completed or preceded by percutaneous nephrolithotomy. Classical surgical procedures may be required in complex cases. Success rate (complete stone free with none residual) with lithotripsy is higher for

Conservative management of colon injury following percutaneous renal surgery.

OBJECTIVES: Colon injury during percutaneous renal surgery is rare and can result in significant morbidity. Our objective was threefold: (1) to identify risk factors for colon injuries; (2) to optimize prevention of such injuries; and (3) to devise a treatment strategy for optimal management of such colon injuries. METHODS: Between July 1990 and July 1995, all percutaneous renal procedures performed at three kidney stone centers were reviewed (Kaiser Permanente Medical Center, Los Angeles; Hospital of the Good Samaritan, Los Angeles; and University of California at San Francisco). In addition

Obstructive Uropathy: a retained urethral catheter within an ileal conduit.

Following an ileal conduit (IC) urinary diversion for intractable urinary incontinence, a woman in her 50s presented with deteriorating renal function during a routine follow-up. This decline prompted further investigation with an ultrasound scan (USS), which demonstrated bilateral hydroureteronephrosis and an atrophic left kidney. Although the mercaptoacetyltriglycine (MAG-3) renogram was inconclusive in revealing urinary obstruction, a subsequent computed tomography (CT) scan uncovered a calcified tube-like structure within the IC and hydroureteronephrosis extending to this level. To address

Ureteral calculi secondary to a gradually migrated acupuncture needle.

We herein presented a case of calculi secondary to a migrated acupuncture needle. A 74-year-old woman with a history of acupuncture therapy for lumbago was referred to our hospital for treatment of ureteral and renal pelvic calculi. Abdominal multi-detector computed tomography scans showed ipsilateral hydronephrosis and two calculi secondary to a migrated acupuncture needle. First, a percutaneous nephrolithotomy was performed to extract two calculi and fine needle fragments from the pelvis. Subsequently, residual needle fragments and calculi in the ureter were then removed by flexible transure

Intestinal metaplasia of the renal pelvis: A case report and literature review.

Metaplastic changes in the renal pelvis are infrequent and may be malignant transformations to adenocarcinoma. The current study reports a case of intestinal metaplasia in the right renal pelvis, which was associated with staghorn calculi, in a 56-year-old female. The patient underwent a percutaneous nephrolithotomy. Immunohistochemical assessment of the mucosa of the renal pelvis revealed the positive expression of carcinoembryonic antigen, cytokeratin (CK)-7 and CK20, but negative expression for CK5/6 and vimentin. Furthermore, Ki67 expression was diffusely positive, while p53 was negative.

[Laparoscopic management of complex lithiasis in horseshoe kidneys].

Based on the fact that the results obtained with extracorporeal shock wave lithotripsy and percutaneous nephrolithotomy are uncertain in cases of complex lithiasis in horseshoe kidney, and counting on a wide laparoscopic experience as the most suitable solution in these cases, the authors present their initial experience in two cases of double lithiasis in right hemikidney. Approach was done through transperitoneal laparoscopy, after placement of a stent catheter and under the support of a radiological C-arc. After removal of the stones, the renal pelvis was closed with loose 4 zero reabsorbab

Modified biplanar (0-90°) endoscopic-guided puncture technique for percutaneous nephrolithtomy: refinement with endoscopic combined intrarrenal surgery to reduce fluoroscopy and operative time.

INTRODUCTION: We aim to publish our innovative modified biplanar 0-90 endoscopic guided puncture technique for percutaneous nephrolithotomy in supine recorded with a GoPro® camera for standardization of the technique. It solves drawbacks of the fluoroscopic technique, i.e., in kidneys with complex anatomy, it may be challenging to distinguish calyces as they are often superposed, and it does not allow for all benefits of a combined endoscopic approach (1, 2). Our technique shortens puncture and fluoroscopic time and is easy to teach and reproduce. METHODS: A 77-year-old female patient had pre

New ex-vivo organ model for percutaneous renal surgery.

OBJECTIVES: Percutaneous Renal Surgery (PRS) is a demanding procedure and success is mostly hampered by the lacking of training facilities. Thus, the purpose of the study was to evaluate a significantly improved pre-existing porcine kidney-training model for percutaneous renal access and PRS. MATERIALS AND METHODS: A biologic training model using porcine kidneys coated by a full-thickness porcine skin flap was prepared. The ureter was dissected, stones were placed into the collecting system using an 18F amplatz sheath, and a catheter was placed in the ureter for further irrigation with saline
