Provocative motor nerve conduction testing in presumptive carpal tunnel syndrome unconfirmed by traditional electrodiagnostic testing.

In this controlled prospective study, 22 consecutive surgical candidates with clinically diagnosed CTS and negative findings on median nerve-sensory and motor-conduction velocity tests in both hands were reexamined with a protocol incorporating 5 specific positions of the wrist. Four of the 5 positions represented maximum physiologic ranges of motion for the patient. These positions were neutral (unstressed), extension, flexion, radial deviation, and ulnar deviation. Motor latency was recorded in each of the 5 positions using otherwise standard technique. The least latency value in the test se

Electrophysiological studies in children with paralytic poliomyelitis.

Ten healthy children (Group I), twenty children with acute spinal poliomyelitis (Group II and III) and twenty children of post-polio residual paralysis (Group IV) between the age group of 9 months to 4 years were the study subjects. The motor nerve conduction velocity (MNCV), standardized distal motor latency (SDML) and motor latency (M-RL) were similar in all the groups studied No H-reflex could be elicited in children with acute spinal poliomyelitis seen within one week of onset of paralysis (Group II). The H-max from 4-6 week group (III) improved with time in 9-12 month (Group IV) but did n

Segmental near nerve sensory conduction studies of the medical and lateral plantar nerve.

Tarsal Tunnel Syndrome (TTS) can be difficult to diagnose: electrophysiologic corroboration is important and has therapeutic implications. Conventional electrodiagnostic techniques are insensitive: motor latency abnormalities exist in only 52%; sensory responses are frequently absent (a nonlocalizing finding). Additionally, previously described near nerve techniques do not isolate conduction velocity (CV) measurement to the short segment across the flexor retinaculum (FR), which would theoretically improve sensitivity. We describe a technique which allows for the determination of segmental sen

[Potassium channel dysfunction at the lesion site in multifocal motor neuropathy as revealed by threshold electrotonus].

Multifocal motor neuropathy presents as a treatable muscular atrophy and is characterized by persistent conduction block and fasciculations or myokymia. Although the pathological findings at the lesion site revealed perivascular demyelination with little evidence of remyelination, the mechanism of conduction block is still unknown. We explored the threshold electrotonus, which registers membrane potential changes through threshold variation, at the lesion site. The findings were consistent with focal potassium channel dysfunction, with some evidence of depolarization block. The disrupted blood

Classic neurogenic thoracic outlet syndrome in a competitive swimmer: a true scalenus anticus syndrome.

Neurogenic thoracic outlet syndrome (TOS) is caused by compression of the lower brachial plexus usually by a cervical rib or a fibrous band. We describe a 16-year-old girl with weakness and wasting of her right hand, which progressed over the ensuing years. She had been a competitive long distance freestyle and butterfly swimmer since age 8 years. A neurological exam at age 20, revealed severe atrophy and weakness of all intrinsic right hand muscles, more so of the thenar muscles, and hypesthesia along the ulnar aspect of the hand and forearm. EMG, which showed a severe chronic axon loss lower

Electrophysiological evidence of "nerve entrapment syndromes" and subclinical peripheral neuropathy in progressive systemic sclerosis (scleroderma).

We report the electrophysiological findings and the management of 5 subjects with progressive systemic sclerosis (PSS) and clinical evidence of nerve entrapment. Three had carpal tunnel syndrome (CTS), 1 bilateral CTS and right tarsal tunnel syndrome (TTS) and 1 Guyon's canal syndrome. Only 1 patient (with CTS) showed significant clinical improvement after surgical decompression; the other 4 demonstrated a slight recovery of conduction without lasting clinical relief after conventional treatment. To explain these failures we hypothesized that these entrapment syndromes were the clinical expres

Autosomal recessive hereditary sensory neuropathy with spastic paraplegia.

Five patients are described with a progressive sensory neuropathy in association with a spastic paraplegia and a mutilating lower limb acropathy. Disease onset was in childhood. Two pairs of siblings were both the offspring of normal consanguinous parents, suggesting autosomal recessive inheritance. The fifth case was sporadic; her parents were normal and non-consanguinous. Nerve biopsy in three patients showed an axonopathy with a loss of myelinated nerve fibres of all diameters and also of unmyelinated axons. In combination with the previous report by Cavanagh et al. (Brain 1979; 102: 79-94)

Fetal nerve healing: an experimental study.

An experimental study was performed to assess fetal nerve repair and regeneration both qualitatively and quantitatively. The posterior tibial nerves in one hindlimb were transected in 16 midgestational fetal lambs and in their mothers. The nerves were then repaired with epineurial sutures and allowed to progress to 2, 4, 6, and 8 weeks postinjury. Qualitative assessment was performed through standard nerve histologic staining, including Luxol fast and toluidine blue for myelin and Bielschowsky stain for axons, and quantitative assessment through nerve conduction velocity studies and morphometr

Splinting for carpal tunnel syndrome: in search of the optimal angle.

Carpal tunnel syndrome (CTS) is the most common of the compression neuropathies. Several studies have demonstrated the efficacy of wrist splinting in relieving the symptoms of CTS; however, the chosen angle of immobilization has varied. Wick catheter measurements of carpal tunnel pressures suggest that the neural position has less pressure and, therefore, greater potential to provide relief from symptoms. This study is a prospectively gathered, blind trial comparing the symptom relief experienced by wearers of splints immobilized at 20 degrees extension and at neutral. The results indicate tha

[Results of electroneuromyographic study in patients after replantation of the lower extremities on the crus level].

We applied ENMG (electroneuromyography) in dynamics in the longterm postoperative period on 7 replanted segments of the lower limbs in 5 patients (in 2 patients both lower limbs were replanted). Seven nerve trunks were restored in 7 replanted segments. Analysis of the ENMG findings showed that restoration of the nerve trunks in replantation of amputated segments of the lower limbs through the crus restored conduction along the sensory and motor fibers in 56.3% of cases; in 37.5% of cases conduction along the motor fibres was restored partly; conduction was completely absent along the sensory f

Across-tarsal-tunnel motor-nerve conduction technique.

Tarsal tunnel syndrome is a commonly considered compression of the tibial nerve and its plantar divisions as the nerve curves behind the medial malleolus underneath the flexor retinaculum. Motor, sensory, and/or mixed-nerve conduction studies are used to confirm or exclude the presence of compression of the posterior tibial nerve and its plantar divisions. In previous studies, stimulation has been done either proximal to the tunnel or distally in the sole of the feet or in the toes. Thus, differentiation between compression of the nerve within the proximal tarsal tunnel, as distinguished from

[Median nerve lesions at wrist in cyclists].

The purpose of this study is to detect the median nerve lesions at wrist in professional cyclists. Totally 14 national representative cyclists were included. Each subject was questionnaired and asked to draw their site of hand discomfort, followed by hand grip power test with grasp, lateral pinch and palmar pinch. A control group of 30 medical students were also tested for grip power. Electrodiagnostic examination of bilateral median nerves was then done on every cyclist, including motor and sensory nerve conduction studies, and needle EMG on abductor pollicis brevis and other associated muscl

Cytomegalovirus neuropathy in acquired immunodeficiency syndrome: a clinical and pathological study.

We have observed typical cytomegalovirus cytopathology associated with multifocal inflammatory and necrotic lesions of peripheral nerve in biopsy specimens from 4 patients who developed a rapidly progressive, multifocal neuropathy late in the course of human immunodeficiency virus infection. The inflammatory infiltrates, which contained numerous polymorphonuclear cells, were associated with mixed, axonal, and demyelinative lesions of nerve fibers. One of these patients improved on treatment with DHPG (9-[2-hydroxy-l(hydroxymethyl) ethoxymethyl] guanine) and remains stable after 18 months. The

Responses of presumed cholinergic mesopontine tegmental neurons to carbachol microinjections in freely moving cats.

The effects of microinjections of a cholinergic agonist, carbachol (0.2 microgram/0.2 microliter), were examined on three different types of rostrally projecting tonic neurons that we have reported previously in the dorsal part of the pontomesencephalic tegmentum known to contain numerous cholinergic cell bodies: 1) tonic type I slow (Type I-S); 2) tonic type I rapid (Type I-R); and 3) tonic type II (Type II) (El Mansari et al. 1989). Microinjections of carbachol near unit recording sites in freely moving cats induced within a few minutes a complete suppression of the spontaneous activity and

Somatosensory evoked potential measures of conduction in peripheral and central pathways in children with protein-calorie malnutrition.

The effects of malnutrition on conduction in peripheral and central somatosensory pathways in humans, as measured by short-latency somatosensory evoked potentials (SEPs) have not been previously reported. A group of 28 children with kwashiorkor were compared to a control group of 35 children, aged 6-36 months. The malnourished group had longer reciprocal conduction velocities (ms/m) for pathways between the wrists and the brachial plexi (CL1 response) and between the brachial plexi and the upper cervical region (CL1-CVN). While an individual child with kwashiorkor had abnormal intracranial rec

Entrapment neuropathies of the foot. Electromyographic techniques.

Entrapment neuropathies frequently involve the foot. The diagnosis can be established by electrophysiological techniques for the exploration of muscles and nerves. Needle recording from the intrinsic foot muscles is a valuable asset in the detection of neurogenic syndromes. The motor and sensory nerve conduction velocities are determined using surface electrodes. The sensory nerve conduction study can be supplemented with somatosensory-evoked-potential studies. Based on these investigations, the location, severity and prognosis of the nerve lesion can be determined. In the presence of tunnel a
