Facial sensibility of patients with trigeminal neuralgias.

OBJECTIVES: Idiopathic trigeminal neuralgia (ITN) is an excruciating shock-like paroxysmal pain restricted to the trigeminal area of innervation, with discrete loss of sensibility (thermal, tactile and painful). Trigeminal postherpetic neuralgia (PHN) is a neuropathic pain at the trigeminal territory that persists after Herpes zoster infection, which also is associated to sensorial compromise. The objective of this study was to evaluate the somesthetic facial sensibility (pain, thermal and tactile) and to compare the findings between PHN and ITN. METHODS: 18 patients with PHN and 26 patients …

Occurrence of trigeminal neuralgia increases with age.

Trigeminal neuralgia is characterised by severe unilateral facial pain that is paroxysmal (recurrent sudden attacks). It is nearly twice as common in women and increases with age to nearly one in 1,000 people over 75 years. The pain, lasting from seconds up to two minutes, can be evoked by trivial stimulation of the trigeminal nerve. Pain may be in the cheek or jaw or both. After each burst of pain there is a brief refractory period.

[Vascular decompression in the treatment of trigeminal neuralgia].

Trigeminal neuralgia is characterized by unilateral paroxysmal facial pain. Patients often describe this pain as "the most sever pain one can suffer". Vascular compression of trigeminal nerve root directly near brainstem by artery or vein is considered the main etiology of trigeminal neuralgia. Consequently, basic pathogenetic treatment is vascular decompression of trigeminal nerve root that allows to eliminate the cause of pain. 140 patients with trigeminal neuralgia were operated in Burdenko Neurosurgical Institute since 1998 to 2007. Pain relief after surgery was observed in 96% of cases. T…

[Improvement of diagnosis and treatment of glossopharyngeal neuralgia].

Differential diagnosis of neuralgias affecting the cranial nerves and of facial pain is often difficult. Glossopharyngeal neuralgia is much less common than trigeminal neuralgia and is not well known. Idiopathic neuralgia of the glossopharyngeal nerve sometimes occurs in association with neurovascular compression syndrome of the vagus and trigeminal nerves. High-resolution MRI of the brain stem with three-dimensional visualization allows a secure diagnosis of neurovascular compression and is useful in the planning of appropriate microsurgical decompression (Jannetta's operation).

SUNCT and trigeminal neuralgia attributed to meningoencephalitis.

We describe a 46-year-old female with viral meningoencephalitis (likely varicella-zoster virus) who developed a SUNCT syndrome followed a few days later by trigeminal neuralgia. Both disorders resolved in parallel with the resolution of encephalitis, which suggests a causal link. In conclusion, headache attributed to intracranial infection may have the clinical features of SUNCT or TN.

The trigeminal vasculature pathology in patients with neuralgia.

OBJECTIVE: To examine the possible pathological changes of the trigeminal vasculature in patients with neuralgia. BACKGROUND: Such a study has never been performed before. The alterations of the trigeminal vessels could have important pathophysiological implications in the trigeminal neuralgia pathogenesis. METHODS: The biopsy specimens for the electronmicroscopic (EM) and immunohistochemical examination were taken during a partial rhizotomy in 6 patients with trigeminal neuralgia and in 2 persons with trigeminal neuropathy. In addition, the 32 normal trigeminal nerves were used as the contr…

Efficacy of pregabalin in the treatment of trigeminal neuralgia.

This prospective, open-label study aimed to evaluate the efficacy of pregabalin treatment in patients suffering from trigeminal neuralgia with and without concomitant facial pain. Fifty-three patients with trigeminal neuralgia (14 with concomitant chronic facial pain) received pregabalin (PGB) 150-600 mg daily and were prospectively followed for 1 year. The primary outcome was number of patients pain free or with reduction of pain intensity by > 50% and of attack frequency by > 50% after 8 weeks. Secondary outcome was sustained pain relief after 1 year. Thirty-nine patients (74%) improved afte…

Treatment of obturator neuralgia with laparoscopic neurolysis.

PURPOSE: Obturator neuralgia is a cause of pelvic and perineal pain that is rarely suggested, probably because it is poorly understood. We report the results of a pilot study of laparoscopic treatment for obturator neuralgia. MATERIALS AND METHODS: Seven patients (8 nerves) with obturator neuralgia were treated at our department. The etiology was idiopathic in 3 cases and iatrogenic following inguinal hernia surgery in 3 and TVT(R) repair in 2. The diagnosis was based on allodynic pain of the anterior and internal surface of the thigh, which was associated with limping with a sidestepping gai…

Trigeminal neuralgia in two patients with glioblastoma.

Headache in glioblastoma patients often indicates raised intracranial pressure by either tumor edema or tumor progression. We report local glioblastoma growth causing cranial nerve lesions as well as trigeminal neuralgia, and highlight pain management in these patients.

Epidemiology of typical and atypical craniofacial neuralgias.

Trigeminal neuralgia (TN) has a prevalence of 0.1-0.2 per thousand and an incidence ranging from about 4-5/100,000/year up to 20/100,000/year after age 60. The female-to-male ratio is about 3:2. A review of several case series shows that pain is more predominant on the right side, but the difference is not statistically significant. TN is significantly associated with arterial hypertension, Charcot-Marie-Tooth neuropathy, glossopharyngeal neuralgia (GN) and multiple sclerosis. GN has an incidence of 0.7/100,000/year and epidemiological studies have shown it to be less severe than previously th…

Diagnosis and treatment of patients with trigeminal neuralgia.

BACKGROUND: The authors review the literature regarding the etiology, diagnosis and treatment of trigeminal neuralgia, or TN. They also describe a patient with the disease. TYPES OF STUDIES REVIEWED: A MEDLINE search was conducted to identify articles published within the last 10 years regarding current thinking on the etiology, diagnosis and treatment of TN. The authors also reviewed historic articles to gather information about the initial discovery and descriptions of this condition. RESULTS: The authors identified more than 50 articles on the topic. Generally, there was concordance regar…

Therapeutic potential of cannabinoids in trigeminal neuralgia.

Trigeminal neuralgia is a disorder of paroxysmal and severely disabling facial pain and continues to be a real therapeutic challenge to the clinicians. While the exact cause and pathology of this disorder is uncertain, it is thought that trigeminal neuralgia caused by irritation of the trigeminal nerve. This irritation results from damage due to the change in the blood vessels, the presence of a tumor or other lesions that cause the compression of the trigeminal root. The pain of trigeminal neuralgia is characterized by unilateral pain attacks that start abruptly and last for varying periods o…

The use of gabapentin for the treatment of postherpetic neuralgia.

BACKGROUND: Varicella-zoster virus causes chickenpox and can reemerge later in life to cause herpes zoster or shingles. One of the most common and disabling complications of herpes zoster is postherpetic neuralgia (PHN). OBJECTIVES: This article reviews the current primary literature about the efficacy and tolerability of gabapentin for the treatment of PHN. Gabapentin pharmacokinetics and drug interactions are also reviewed. METHODS: A literature search in the English language was conducted using OVID Web, which contained the following databases: MEDLINE (1966-present), EMBASE (1980-2002), …
