Assessing cognitive functioning in females with Rett syndrome by eye-tracking methodology.

BACKGROUND: While many individuals with severe developmental impairments learn to communicate with augmentative and alternative communication (AAC) devices, a significant number of individuals show major difficulties in the effective use of AAC. Recent technological innovations, i.e., eye-tracking technology (ETT), aim to improve the transparency of communication and may also enable a more valid cognitive assessment. OBJECTIVES: To investigate whether ETT in forced-choice tasks can enable children with very severe motor and speech impairments to respond consistently, allowing a more reliable

Image: Extensive Pneumocephalus.

A 53-year-old man was admitted to our hospital following a traffic accident. He had been riding a bicycle and was knocked down by a motorcycle, injuring the right side of his forehead. Upon arrival at our department, he was conscious and we found no cerebrospinal fluid rhinorrhea or otorrhea. His Glasgow Coma Scale score was 15. Neurologic examination was normal, except for reduced vision in his right eye. Head computed tomography showed extensive pneumocephalus involving the cisterna ambiens, prepontine and suprasellar cisterns, and temporal, frontal, parietal, and occipital lobes. A right fr…

Clinimetric testing of the comprehensive cervical dystonia rating scale.

INTRODUCTION: The aim of this study was to test the clinimetric properties of the Comprehensive Cervical Dystonia Rating Scale. This is a modular scale with modifications of the Toronto Western Spasmodic Torticollis Rating Scale (composed of three subscales assessing motor severity, disability, and pain) now referred to as the revised Toronto Western Spasmodic Torticollis Scale-2; a newly developed psychiatric screening instrument; and the Cervical Dystonia Impact Profile-58 as a quality of life measure. METHODS: Ten dystonia experts rated subjects with cervical dystonia using the comprehensi

[A neonate with a sacrococcygeal mass].

A neonate was born with a sacrococcygeal mass. Initially, spina bifida was suspected. However, neurological examination was unremarkable. An MRI of the neuraxis showed a large cystic presacral lesion without signs of spina bifida. Surgical resection of the lesion was performed. Pathologic evaluation confirmed the diagnosis of a sacrococcygeal teratoma.

Transient Visual Hallucinations due to Posterior Callosal Stroke.

BACKGROUND: Stroke of the corpus callosum is uncommon, known to be associated with disturbance of higher brain function including hemifacial metamorphosia, but has never been reported to cause visual hallucinations. METHODS: We report on a case of a 71-year-old woman who presented to the emergency department with a 3-day history of intermittent, formed visual hallucinations in the left visual field, often triggered by eye closure or darkness. RESULTS: Neurologic examination was normal including cognition and affect, visual acuity, and visual fields. Electroencephalogram was normal. Magnetic …

PURPOSE OF REVIEW: This article offers a pragmatic roadmap to the practicing neurologist on how to approach the patient with double vision. … gnments of the eyes, including cover and Maddox rod testing, are primary components of the efferent neurologic examination. Further testing designed to detect myasthenia (eg, lid testing and fatigable upgaze) and orbital disease (eg, measuring proptosis, testing for resistance to retropulsion) may be necessary. RECENT FINDINGS: Recent advances in the diagnosis of diplo …

Kennedy disease with biphasic clinical course and rapid progression.

We report a case of spinal and bulbar muscular atrophy (SBMA), also known as Kennedy disease, with a 38 CAG-repeat expansion in exon-1 of the androgen receptor gene, presenting with a 2-year history of mild speech difficulty, dysphonia, and occasional choking. Initial clinical features and complementary studies were consistent with SBMA. The disease progression, as assessed by the Amyotrophic Lateral Sclerosis Functional Rating Scale-Revised, remained stable over the first 5 years from the onset but showed a rapid decline (from 42 to 24 points) over the next 18 months before his death. In the

Nosography of the "essential": volitional palatal tremor.

A 19-year-old heavy metal singer with a history of congenital micrognathia, post-traumatic stress disorder, and anxiety developed isolated movements of the soft palate after lower mandible corrective surgery (osteotomy and genioplasty). He endorsed ear clicks and control over the movements. On examination, there were rhythmic movements of the distal soft palate, characteristic of essential palatal tremor (EPT). However, their change in frequency and amplitude on command (video on the Neurology® Web site at www.neurology.org) indicated volitional control. Unlike psychogenic EPT, whereby variabi

[A case of cavernous sinus aspergillosis].

We reported a case of cavernous sinus aspergillosis. A 62-year-old man complained of trigeminal neuralgia in the right V1 region. Neurological examination on admission showed ptosis, loss of light reflex and ophthalmoplegia externa in the right side. MRI enhanced with gadolinium demonstrated sphenoid sinusitis and mass lesion in the right cavernous sinus. MRA revealed right internal carotid artery occlusion. An open biopsy using the extradural temporopolar approach was performed. Pus discharge was observed from the cavernous sinus and histological examination showed hypha of Aspergillus. With

The Pulfrich phenomenon; clumsiness and collisions which can be ameliorated.

The Pulfrich phenomenon has been described many times but usually just as an interesting academic issue with only diagnostic significance. The phenomenon is due to the fact that patients with unilateral optic neuropathy due to ischaemia, facial injury or multiple sclerosis may have delayed transmission of retinal responses to light from one eye to the brain. This leads to an altered and incorrect interpretation of the pathway and direction of moving targets. Clinicians should be aware of the unique symptomatology of the Pulfrich phenomenon because it is often difficult for patients to describe

[Thumb/big-toe localizing test: examination for deficit of proprioception through the posterior column-medial lemniscal system].

"Thumb localizing test" (TLT) and "big-toe localizing test" (BLT) are bedside examinations to detect abnormalities in proprioceptive afferent pathways from the limbs. In TLT, the patient is asked to close the eyes, one upper limb of the patient is placed in a fixed position by the examiner, and the patient is asked to localize the thumb of the fixed upper limb with the thumb and index finger of the other upper limb. In BLT, the patient is asked to close the eyes, a lower limb is passively immobilized by the examiner, and the patient is asked to locate the big toe with either index finger. Norm

A case of diabetic neuropathy combined with Guillain-Barre syndrome.

A 59-year-old man was admitted with numbness, pain, and a tingling sensation in both lower legs. He was initially diagnosed with diabetic peripheral neuropathy based on a symptom questionnaire and a quantitative sensory test. Despite symptomatic treatment of diabetic neuropathy, he complained of worsening sensory symptoms and additional motor weakness in both lower extremities. As the motor weakness of both extremities became more aggravated over time, brain and spine imaging tests and a nerve conduction test were performed. The nerve conduction study revealed motor and sensory axonal neuropat

Vertical weight-bearing MRI provides an innovative method for standardizing Spurling test.

Although Spurling test, a foraminal compression test, is commonly used in clinical practice in patients with a suspected cervical radiculopathy, its protocol is still obscure. In undergoing this test, patients extend, laterally flex and slightly rotate neck to the symptomatic side, and then a pressure is applied on the top of patient's head by examiner. The test is scored as positive if it causes pain or tingling that starts in the shoulder and radiates distally to the elbow. But the range of neck motion and level of load are not clearly defined. Magnetic resonance imaging (MRI) has proved to

Acute confusional migraine may be a presenting feature of CADASIL.

OBJECTIVE: Characterize the phenomenon of acute confusional migraine (ACM) among Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) patients and emphasize the possibility of CADASIL in adults with ACM. BACKGROUND: ACM, well described in children, has rarely been reported in adults. Although 30-60% of CADASIL patients have migraine, acute confusional state during migraine has not been described. We describe 7 patients with ACM that complicated up to 50% of the migraine episodes. DESIGN/METHODS: Detailed neurologic evaluation was performed in 2

A case of tactile agnosia with a lesion restricted to the post-central gyrus.

Tactile agnosia has been described after lesions of the primary sensory cortex but the exact location and extension of those lesions is not clear. We report the clinical features and imaging findings in a patient with an acute ischemic stroke restricted to the primary sensory area (S1). A 73-year-old man had a sudden onset of a left alien hand, without left hemiparesis. Neurological examination showed intact primary sensory functions, but impaired recognition of shape, size (macrogeometrical) and texture (microgeometrical) of objects; damage confined to the post-central gyrus, sparing the post

Acute lumbar disk pain: navigating evaluation and treatment choices.

Acute lumbar disk herniations are the most common cause of sciatica. After excluding emergent causes, such as cauda equina syndrome, epidural abscess, fracture, or malignancy, a six-week trial of conservative management is indicated. Patients should be advised to stay active. If symptoms persist after six weeks, or if there is worsening neurologic function, imaging and invasive procedures may be considered. Most patients with lumbar disk herniations improve over six weeks. Because there is no difference in outcomes between surgical and conservative treatment after two years, patient preference

The neurological determination of death: what does it really mean?

The recent Canadian forum's recommendations regarding "neurological determination of death" claim to have determined a "Canadian definition, criteria, and minimum testing requirements for neurological determination of death." In this review the problems with this statement are discussed. The criterion of neurological determination of death does not fulfill the definition of death, because there is continued integration of the organism as a whole. The tests for neurological determination of death do not fulfill the criterion of neurological determination of death because they do not show the ir

Trigeminal nerve injury following accidental airbag deployment and assessment with quantitative sensory testing.

High velocity motor vehicle accidents are associated with an increase in mortality rates and a significant number of facial injuries. Accidental deployment of airbags and the associated release of hot gases can result in both thermal and mechanical injuries. The more commonly reported maxillofacial injuries include temporomandibular joint fractures and dislocations, dental trauma, facial nerve paralysis, and other orofacial pain complaints. The following case report describes a patient with facial trauma from the accidental deployment of an airbag resulting in complaints consistent with a neur
