Neuroradiology: Differential Diagnosis, Follow-Up, and Reporting.

Cavernous cerebral malformations (CCMs) can show typical and characteristic findings at neuroradiology, above all at magnetic resonance imaging, but differential diagnosis with other lesions of similar appearance can be challenging and should be taken into consideration. Management of CCMs can be conservative in most cases, and thus appropriate follow-up timing and modality is required. Growing input from neurologists, neurosurgeons, neuroradiologists, and patients recommend to offer a standard neuroradiological report, to enhance interpretation and comparability in daily clinical practice. Th…

Spontaneous resolution of large subdural hematoma with midline shift: a case report.

Subdural hematomas with mass effect and midline shift are typically offered urgent surgical evacuation. The authors report a 6-month-old baby who was referred for macrocephaly and found to have a large subdural hematoma with midline shift. Given her lack of symptoms and normal neurological examination, the patient was observed without neurosurgical intervention. She remained asymptomatic, and serial brain imaging showed progressive decrease of the subdural hemorrhage and complete resolution of midline shift. This case is presented to alert neurosurgeons that observation of a large subdural hem…

Hydranencephaly: Considering Prolonged Survival and Treatment by Endoscopic Choroid Plexus Coagulation.

Arachnoid collapse is a previously unreported complication of endoscopic choroid plexus coagulation (ECPC) treatment of hydranencephaly and is demonstrated in this case report. The variable anatomy found in hydranencephaly supports the use of ECPC as the preferred treatment option. However, the same anatomical anomalies predispose the procedure to this unique complication. A brief literature review of hydranencephaly has also been performed and is reported through the discussion. Neurosurgeons must be aware of this very dangerous complication and a discussion of preventative measures are made.…

Reduction cranioplasty for macrocephaly with long-standing hydrocephalus and non-fused fontanelle in Chiari malformation type I.

INTRODUCTION: Because hydrocephalus is diagnosed and treated at an early stage in pediatric patients, pediatric neurosurgeons rarely encounter patients with hydrocephalic macrocephaly. There are even fewer cases of infants with long-standing hydrocephalus in whom macrocephaly progresses and is accompanied by skull defect due to malunion of suture lines despite long-term CSF diversion treatment. CASE REPORT: We report the case of a male infant with Chiari malformation type I who presented with congenital hydrocephalus and occipital encephalocele that progressed to hydrocephalic macrocephaly wi…

Superficial temporal artery to middle cerebral artery bypass.

The superficial temporal artery to middle cerebral artery bypass is an elegant procedure that was developed and first performed by M. Gazi Yaşargil. It has been used by neurosurgeons for more than 30 years in the management of neurovascular disorders such as cerebrovascular ischemic disease, moyamoya disease, and complex intracranial aneurysms. Mastering the technique requires not only precise and fine skills but also devoted training in the microsurgery laboratory. The technique presented in this article evolved from the long and vast experience of the senior author (DWN) in performing superf…

[Diagnosis and surgical treatment of mediastinal neoplasms].

The authors share their experiences with examination and surgical treatment of 163 patients with various neoplasms of the mediastinum. The first place among the neoplasms is occupied by tumors of the lymphatic apparatus (33%), the second--by tumors of the thymus (21%). The video-thoracoscopic technique was used in 17 of 131 operations performed. Complications after the operations took place in 6 patients (4.5%), two patients died (1.5%). A conclusion is made that the patients of this category must be treated not only by thoracic surgeons but also by hematologists, neurologists, neurosurgeons a…

[Current status of the diagnosis and treatment of laterobasal skull fractures].

Latero-basal fractures of the skull base due to a blunt head injury may cause vascular and nervous lesions. The most important lesion is rupture of the middle meningeal artery in the foramen spinosum leading to an acute epidural haematoma. Its early diagnosis is of vital importance. Lesions of the cranial nerves are rarely surgically remediable. Diagnostic steps in the management of these lesions are outlined. Close cooperation between ENT specialists and neurosurgeons is of the highest importance, particularly when dealing with petrous bone lesions producing C.S.F. otorrhea or the prolapse ap…

Treatment challenges in pediatric Cushing's disease: Review of the literature with particular emphasis on predictive factors for the disease recurrence.

Cushing's disease (CD) is a rare endocrine condition caused by a corticotroph pituitary tumor that produces adrenocorticotropic hormone. The current state of knowledge of CD treatment is presented in this article including factors that can be helpful in predicting remission and/or recurrence of the disease. The primary goals in CD treatment are quick diagnosis and effective, prompt treatment as the persistent disease is associated with increased morbidity and mortality. Cooperation of a team consisting of experienced pediatrician/adult endocrinologist, neuroradiologist, transsphenoidal neurosu

Endoscopic third ventriculostomy for hydrocephalus: a review of indications, outcomes, and complications.

Endoscopic third ventriculostomy (ETV) has been in vogue for the past two decades, as a tool in the armamentarium of the neurosurgeon, for the management of hydrocephalus. Its utility has been proven consistently in congenital / acquired aqueductal stenosis, although the outcomes in communicating hydrocephalus as well as hydrocephalus secondary to other etiologies have not been as impressive. It is a relatively safe procedure with the appropriate selection of patients with a low rate of permanent morbidity. This review aims to define the current indications, management outcomes, and complicati

Gliomas of the anterior visual pathways.

Gliomas of the anterior visual pathways include the more commonly encountered benign tumors of children and the rare and highly aggressive malignant tumors of adults. Diagnostic biopsy is not required in most cases of childhood gliomas, especially those unassociated with visual loss or other neurologic symptoms, because the clinical and neuroimaging features are often sufficiently characteristic. Because of the infiltrative nature of these tumors, the neurosurgeon has little to offer symptomatic patients beyond debulking large exophytic masses and shunting if hydrocephalus is present.

Predictability of the result of surgical intervention in patients with low back pain.

Patients who were to undergo surgical intervention for low back pain were investigated preoperatively. The effect of surgical intervention was determined after 6 months. The judgement of the neurosurgeon and of the patients themselves were used to assign patients to three outcome categories: satisfactory, moderate and unsatisfactory. Differences in several psychological and biological factors were found between the three categories. On the basis of preoperatively assembled data on psychological and other variables it is possible to predict the outcome of surgical intervention in 80% of the pat

Silent duodenal perforation. A difficult diagnosis in iatrogenic barbiturate coma.

Iatrogenic barbiturate coma for the control of intracranial hypertension is a relatively new addition to the neurosurgeon's therapeutic armamentarium. Although initial studies are quite promising, the induction of coma, especially for prolonged periods (ranging from days to weeks), is a major therapeutic endeavor which requires sophisticated monitoring of both neurological and physiological function. A multidisciplinary approach prevents many possible complications of this form of therapy and facilitates treatment when untoward physiological complications do occur. The following case report il

Craniofacial resection for malignant tumors of ethmoid and anterior skull base.

A combined craniofacial approach is described for resection of tumors that involve the ethmoid sinuses and adjacent base of the skull in the frontal fossa. This operation provides a more thorough en bloc excision than by conventional methods with reasonable safety since both the neurosurgeon and the head and neck surgeon are guided by each other during the mobilization of the specimen, and adequate repair of dural defects can be accomplished. Reconstruction of the bony defect in the skull base is performed with a pedicled periosteal flap from the scalp and a skin graft.

Neurosurgical management of cranial and spinal pathologies during civil war: Lessons from an isolated neurosurgical department during the insurrection in New Caledonia.

INTRODUCTION: Since November 2023, a neurosurgery department has been created in Nouméa, New Caledonia. Meanwhile, the 13th of May, 2024 a civil war and a subsequent transient humanitarian crisis dramatically influenced the surgical activity. Several technical and ethical limits have been faced during the "war induced" isolation. CIVIL WAR NEUROSURGICAL EXPERIENCE: During the 2 months long civil war, 52 urgent surgeries have been performed: 27 were cranial and 25 were spinal procedures. Surgical indications were mostly traumatic, infectious, tumoral, CSF related disorders and vascular patholo

Microneurosurgical Anatomic Study of the Horizontal Fissure of the Cerebellum.

OBJECTIVE: The horizontal fissure of the cerebellum, which is the largest and most prominent fissure, has received less interest from anatomists and neurosurgeons. Hence, the current study aims to provide comprehensive detail about the horizontal fissure and its anatomic and surgical relationship with deeper structures such as the dentate nucleus and middle cerebellar peduncle for the benefit of the neurosurgeon. METHODS: Ten whole formalin-fixed human cadaveric cerebellar hemispheres were obtained from human cadavers donated to the institution. Different parameters of the horizontal fissure …

Transfrontal External Ventricular Drainage Combined with OMMAYA Sac Implantation under Laser Navigation were Performed: A Technical Note.

BACKGROUND: Hydrocephalus caused by spontaneous intracerebral hemorrhage (ICH) is an independent risk factor with adverse effects on the progression of the disease. Until now, the choice of intraventricular catheter placement and intraventricular fibrinolysis (IVF) has been mainly based on the personal experience of the neurosurgeon. OBJECTIVE: We will introduce the clinical effect of the new external ventricular drainage (EVD), an independent innovation of our medical center, on ICH patients, hoping to inspire more neurosurgeons to apply our method. METHODS: In this open retrospective study…
