Binocular motility system and temporomandibular joint internal derangement: a study in adults.

INTRODUCTION: Ocular convergence defects have been confirmed in adults with temporomandibular joint (TMJ) dysfunction, but few studies of the relationship of the oculomotor apparatus to TMJ disc displacement have been reported. The purpose of this study was to examine the impact of disc displacement on the oculomotor capacity of the eyes in adults. METHODS: Fifty symptomatic patients with bilateral TMJ disc displacement (13 men, 37 women; mean age, 28.84 +/- 8.22 years; range, 18-40 years) were compared with the same number of asymptomatic volunteers with normal disc position (14 men, 36 wome

[Thalamic dementia due to infarct of the left thalamus and genum of the right internal capsule].

INTRODUCTION AND OBJECTIVES: Thalamic dementia is the clinical consequence of a disorder of both thalami. It is generally secondary to bilateral paramedial thalamic infarcts due to disorders of small blood vessels or cardioembolism. We report a case of dementia of acute onset involving the left thalamus and the genum of the right internal capsule. CLINICAL CASE: A 33 year old man, HIV positive, category B2, admitted to hospital for tuberculous meningitis presented with the acute onset of somnolence, followed by marked bradypsychism, personality changes, marked disorder of executive explicit m

Combined diagnosis for Parkinson's disease via gait and eye movement disorders.

BACKGROUND AND OBJECTIVES: With the discovery of the potential role of gait and eye movement disorders in Parkinson's disease (PD) recognition, we intend to investigate the combined diagnostic value of gait and eye movement disorders for PD. METHODS: We enrolled some Chinese PD patients and healthy controls and separated them into the training and validation sets based on enrollment time. Performance in five oculomotor paradigms and in one gait paradigm was examined using an infrared eye tracking device and a wearable gait analysis device. We developed and validated a combined model for PD di

A case of skew deviation and downbeat Nystagmus induced by Lithium.

BACKGROUND: Lithium salts have been commonly used for prophylaxis and treatment of bipolar disorder and have numerous side effects. However, there has been no report of skew deviation and downbeat nystagmus associated with lithium. Herein, we report the first case of lithium-induced skew deviation and downbeat nystagmus. CASE PRESENTATION: A 39 years-old woman presented with intermittent vertical diplopia and dizziness within 1-2 months. Ophthalmologic examination revealed downbeat nystagmus and 6 prism diopters of right hypertropia. Funduscopic examination showed mild incyclotorsion on right

Vertiginous headache and its management.

Vertiginous headache encompasses patients with dizziness or vertigo as well as headache, even though the symptoms may not occur in an obvious temporal relationship. The type of dizziness experienced by patients is different from the heavy-headedness experienced during rhinogenic headache. Patients may have a personal or family history of typical or atypical migraine. They should be evaluated for possible Meniere syndrome, migraine headaches, and/or eye movement disorders. Management is directed to treatment of the underlying abnormality. Long-term follow-up of these patients is necessary, beca

[Posterior cortical atrophy. Report of five cases].

Posterior cortical atrophy (PCA) is a neurodegenerative syndrome, usually due to Alzheimer's disease. The first symptoms are progressive impairment of visuo spatial (Balint's and Gertsmann's syndromes) or visuo perceptive (visual agnosia, alexia) function. Episodic memory and executive function are spared until later stages. We report two males aged 51 and 55years and three females aged 50, 54 and 56 years, with posterior cortical atrophy. Ophthalmologic study was normal in all. Presenting signs and symptoms were visual ataxia, simultagnosia, agraphia, acalculia, spatial disorientation and uni

Pursuit eye movement deficits in autism.

Oculomotor studies provide a novel strategy for evaluating the functional integrity of multiple brain systems and cognitive processes in autism. The current study compared pursuit eye movements of 60 high-functioning individuals with autism and 94 intelligence quotient, age and gender matched healthy individuals using ramp and oscillating target tasks. Individuals with autism had normal pursuit latency, but reduced closed-loop pursuit gain when tracking both oscillating and ramp targets. This closed-loop deficit was similar for leftward and rightward pursuit, but the difference between individ

[Contribution of oculomotor examination for the etiological diagnosis of parkinsonian syndromes].

Exploration of ocular motricity can be helpful for diagnosis in certain parkinsonian syndromes. Oculomotricity is perturbed in Parkinson's disease (PD) and in multiple system atrophy (MSA). The minimal anomalies, sometimes observed in these conditions, both clinically and on oculomotor recordings, cannot contribute significantly to diagnosis. In corticobasal degeneration (CBD), infraclinical oculomotor anomalies can be identified on oculomotor recordings and are relatively specific to atypical parkinsonian syndrome: particularly long latency of ocular saccades related to posterior parietal inv

Intracranial hypertension.

Intracranial hypertension is not a definitive diagnosis, but rather a syndrome that may result from a number of neurologic and systemic disorders. Intracranial hypertension refers to prolonged elevation of intracranial pressure, generally above 200 mm H2O. This condition may be recognized by the various clinical signs and symptoms that are manifest in most patients, including headache, papilledema, transient visual obscurations, diplopia, ocular motor disorders, tinnitus, nausea, vomiting, and mental irregularities, as well as dysfunctions of the circulatory and respiratory systems. [Clinical aspects and mechanisms of visual-spatial neglect].

Several interpretations have been advanced to explain the nature of unilateral spatial neglect. According to some authors, unilateral neglect could be due to a sensory deficit or to an oculomotor disorder hampering the patient from completely exploring the half space contralateral to the damaged hemisphere. According to other authors, the unilateral neglect syndrome could result either from a mutilation of the inner representation of the outside space or from a lack of attention for stimuli falling in the half space opposite to the hemispheric locus of lesion. We have wondered whether the rela

Rhabdomyolysis Presenting as Orbital Apex Syndrome.

Rhabdomyolysis is a condition in which striated muscle tissue breaks down rapidly and releases muscular cell constituents into extracellular fluid and the circulation. Renal symptoms, such as acute renal failure, are major complications of rhabdomyolysis. However, no previous report of rhabdomyolysis associated with orbital complication has been issued. Here, we report the first patient of rhabdomyolysis presenting as orbital apex syndrome. A 66-year-old man presented with right periorbital swelling with erythematous patches and conjunctival chemosis. In addition, swelling, redness, and vesicl

Visual signs and symptoms of corticobasal degeneration.

Corticobasal degeneration is a rare, progressive neurodegenerative disease and a member of the 'parkinsonian' group of disorders, which also includes Parkinson's disease, progressive supranuclear palsy, dementia with Lewy bodies and multiple system atrophy. The most common initial symptom is limb clumsiness, usually affecting one side of the body, with or without accompanying rigidity or tremor. Subsequently, the disease affects gait and there is a slow progression to influence ipsilateral arms and legs. Apraxia and dementia are the most common cortical signs. Corticobasal degeneration can be

Kufor-Rakeb syndrome-associated psychosis: a novel loss-of-function ATP13A2 variant and response to antipsychotic therapy.

Biallelic (autosomal recessive) pathogenic variants in ATP13A2 cause a form of juvenile-onset parkinsonism, termed Kufor-Rakeb syndrome. In addition to motor symptoms, a variety of other neurological and psychiatric symptoms may occur in affected individuals, including supranuclear gaze palsy and cognitive decline. Although psychotic symptoms are often reported, response to antipsychotic therapy is not well described in previous case reports/series. As such, we describe treatment response in an individual with Kufor-Rakeb syndrome-associated psychosis. His disease was caused by a homozygous no

Dystonic tics in patients with Tourette's syndrome.

Dystonic tics, in contrast to clonic tics, are relatively slow and temporarily sustained twisting, pulling, or squeezing movements producing briefly maintained abnormal postures. Because dystonic tics often present diagnostic difficulties, we studied these motor tics in 156 patients with Tourette's syndrome (TS). In addition to clonic motor and vocal/phonic tics, seen in all patients, 89 (57%) of them exhibited one or more dystonic tics. The most common dystonic tics were oculogyric deviations (43 patients), blepharospasm (23), and dystonic neck movements (11). Blinking (110) and facial twitch

Isolated Paroxysmal Right Upward Eye Deviation as the Sole Semiology of Childhood Occipital Epilepsy: An Electroclinical Case Report.

BackgroundIsolated paroxysmal eye deviation in children is often considered a benign nonepileptic phenomenon. When epileptic in origin, gaze deviation is usually accompanied by visual symptoms, autonomic features, impaired awareness, or additional motor manifestations.CaseWe report a 6-year-old boy with frequent daily episodes of brief right upward eye deviation without impaired awareness, visual symptoms, head version, or other ictal features. Neurological and ophthalmological examinations, routine laboratory investigations, and brain magnetic resonance imaging were unremarkable. Video-electr

[Ophtalmoplegia secondary to ponto-mesencephalic infarction : clinical-radiological correlation in the WEBINO syndrome].

We report on a case of posterior midbrain stroke revealed by WEBINO ("Wall-Eyed Bilateral InterNuclear Ophthalmoplegia") syndrome, a relatively rare neuro-ophthalmological clinical entity combining bilateral internuclear ophthalmoplegia and exotropia. Encephalic MRI unraveled a bilateral paramedian ischemic lesion located at the posterior area of the ponto-mesencephalic junction, perfectly correlating to the theoretical localization of the medial longitudinal fasciculi. Clinical identification of this syndrome is key to efficiently guide MRI reading and proper management.

Visual midline gauge validity and repeatability: Comparison to a current clinical method.

SIGNIFICANCE: Visual midline shifts are thought to occur post-stroke and be a risk factor for falls. This study investigates a new method for quantifying visual midline shifts, a first step toward developing greater understanding of visual midline shift. PURPOSE: This study standardized the parameters of a novel visual midline gauge, compared the results with the current clinical method, and presents normative data and repeatability of both methods. METHODS: Ninety-three participants without neurological or ocular problems were recruited in Canada and Hong Kong. In experiment 1, horizontal a

Internuclear ophthalmoplegia as a presentation of procedural stroke: a case report.

BACKGROUND: Cardiac catheterization and endovascular procedures are extensively used in modern medicine, and procedural stroke is one of the major complications that the catheterization laboratory team may face in their everyday work. Recognizing the signs and symptoms of procedural stroke is crucial to ensuring appropriate management. We herein report a case of internuclear ophthalmoplegia that caused blurred vision, diplopia, and dizziness on lateral gaze as an unusual presentation of procedural stroke.
