We reported a case of optic neuritis with the persistence of severe visual loss and central scotoma in a 26-year-old woman who was proven to have biologic false positive test for syphilis, and the elevated serum titres of IgG and IgM anticardiolipin antibodies. C.S.F. findings showed the absence of oligoclonal bands and the presence of IgM anticardiolipin antibody. We reviewed the records of 10 children with optic neuritis in whom recovery of vision was poor or incomplete. Our cases were otherwise similar to those described in previous studies in that they were always bilateral, often accompanied by a viral prodrome (seven of 10), and usually associated with disc oedema (seven of 10). We report a 2-year-4-month-old boy with retrobulbar optic neuritis. He had a sudden onset of impaired vision, which progressed to total blindness within a day. The visual evoked potential (VEP) showed no activity, but the electroretinogram was normal. Three patients with acute monocular central scotoma and headache had the initial diagnosis of optic neuritis. Computed tomography showed large pituitary adenomas with necrosis and cyst formation consistent with the clinical symptoms of a previous pituitary apoplexy. After transsphenoidal adenomectomy the visual disturbances disappeared.

Serial brain MRI was performed in 53 patients with clinically isolated optic neuritis. Using clinical and imaging evidence for relapse, multiple sclerosis developed within a mean of 12 months in 19 of 34 cases (56%) with brain lesions at presentation, and in only 3 of 19 cases (16%) without (Relative Risk = 6.8, p less than 0.005).

A 63-year-old man developed bilateral paresis of horizontal and upward eye movements. He was found to have a small oat cell carcinoma of the … nerve. No tumor cells were seen. These findings were consistent with a diagnosis of paraneoplastic optic neuritis and paraneoplastic encephalomyelitis. The present case confirms the existence of paraneoplastic optic neuritis and illustrates the clinical course of the disease.

The temporal contrast sensitivity function and temporal frequency discrimination have been studied in normals and in 7 cases of multiple sclerosis or optic neuritis. Pattern ERGs and pattern VECPs were recorded in 12 patients who had suffered from optic neuritis in childhood. When each individual's eyes were compared, the eye with the longer P100-latency in pattern VECP was found to have the smaller amplitude in pattern ERG. The latency differences correlated with the amplitude differences. These findings confirm that after optic neuritis the amplitude of pattern ERG is reduced.

We measured contrast sensitivity by means of Arden grating plates in 41 eyes with acute optic neuritis and in 51 eyes with resolved optic neuritis. The scores were abnormal in 93% of eyes in the acute phase and 78% in the resolved phase irrespective of visual acuity. Even when acuity improved to 6/7.5 or better with resolution of the neuritis, 67% of eyes still showed abnormal contrast sensitivity.
