A 30-year-old man with azoospermia underwent an examination for infertility. He had a history of right orchiopexy due to cryptorchism. Radiological examinations showed an absence of the left kidney and the left seminal vesicle. A testicular biopsy revealed normal spermatogenesis in the left testis, but no spermatogenesis in the right testis. A retrospective study was conducted for a period of 4 years during 1992-1995 at Medical College Hospital, Alappuzha, Kerala to find out whether boys with undescended testis got treated at ideal age. Discharge summaries of boys who were treated for undescended testis in this hospital were studied. There were 139 patients with 162 undescended testes. PURPOSE: We determined whether testis viability after a Fowler-Stephens orchiopexy depends on ligating the spermatic vessels high and far proximal to the undescended testis.
MATERIALS AND METHODS: Based on studies of testicular vascular anatomy we developed a technique of low spermatic vessel ligation and performed it on 39 high undescended testes in 33 patients.
RESULTS: Testis viability was 97% at 1 month and 93% (25 of 27) at 1 year.

We studied 48 prepubertal boys with cryptorchidism between 1 year 3 months and 11 years old who were treated with buserelin every other day for 6 months. Successful preservation of a testis that has undergone torsion is directly related to the duration and degree of torsion. From 1979 to 1991, 75 cases of testicular torsion were treated. In a prospective study the inter-observer variation in the diagnosis of undescended testis was analysed. Two physicians assessed independently the position and motility of the testes of 37 boys referred for undescended testis. The boys were examined in the supine and squatting positions. The management of the undescended testis in the adult patient presents problems in terms of locating the impalpable gonad, the risk of malignancy and prognosis with regard to fertility. CT scanning has been used to locate four intra-abdominal testes. The hospital records in a community of 63,000 were reviewed for a five-year period (1978 to 1983) with regard to the diagnosis of cryptorchidism. There were 123 hospitalized patients with unilateral cryptorchidism and 17 patients with bilateral cryptorchidism.
