Leg length discrepancy in unilateral congenital clubfoot following surgical treatment.

Length discrepancy secondary to limb hypoplasia has been described as an associated finding in patients with unilateral clubfoot. In this manuscript we bring attention to limb length discrepancy as a result of surgical treatment in unilateral clubfoot. Three patients who underwent extensive posterior, medial and lateral release were noted to have an average discrepancy in foot height of 2.1 centimeters (range, 2.0-2.3 centimeters). A decrease in foot height in addition to baseline limb hypoplasia may lead to a significant discrepancy that may justify surgical treatment. In this manuscript we p

[The historical evolution of one-stage surgical treatment of congenital clubfoot in newborns].

The author presents a historical overview of the evolution of surgical treatment of congenital clubfeet, focussing in particular on the last 30 years. The paper reviews the pathological and biomechanical problems related to the surgical procedures introduced by Turco, McKay and Simons, as well as indications for procedures from a postero-lateral and Cincinnati approach.

[Foot surgery in Germany. Historical burden or scientific challenge?].

In Germany foot surgery was established in the late nineteenth century. To correct foot deformities due to paralytic disorders, i.e., poliomyelitis, and congenital deformities, different types of tenotomies and arthrodeses were developed to obtain plantigrade weight-bearing and to avoid external orthosis. A description of surgical techniques during the last century is presented in detail. During those years resection arthroplasties for hallux valgus were common, although some authors recommended osteotomies of the first metatarsal as joint-sparing techniques. Nowadays, further knowledge in eti

[One hundred years of orthopedics in the Netherlands. IV. Spinal abnormalities].

In recent years there has been spectacular progress in the approach to various disorders of the spinal column. Owing to improved methods of osteosynthesis there is no longer so much need for long periods of postoperative bed rest. Of all the scolioses, idiopathic scoliosis is most common. The vast majority of these cases are not clinically significant. What is seen in the remaining cases if left untreated is a progression in the curvature during growth. Progressive idiopathic scoliosis can be effectively treated using conservative methods. Screening at school is an important part of this proce

Suspected Hypersensitivity Reaction to Titanium Screws Following Kalish Osteotomy for Hallux Valgus.

Titanium implants are generally regarded as biocompatible and inert; however, hypersensitivity reactions, although uncommon, have been increasingly recognized in orthopedic surgery. We report a case of chronic inflammatory reaction following a Kalish osteotomy for hallux valgus, ultimately attributed to titanium hypersensitivity. A 58-year-old woman underwent fixation of the first metatarsal with two titanium screws. The postoperative course was complicated by wound dehiscence, delayed healing, recurrent granulomatous tissue, and persistent swelling, despite antibiotic therapy. Over the months

Cauda Equina Syndrome Secondary to Incidental Hemorrhagic Ependymoma Following Spinal Anesthesia: A Case Report and Comprehensive Review.

Spinal ependymomas are rare primary central nervous system tumors that often exhibit vague symptoms before being identified. In extremely rare situations, it can be identified after a neurological decline following a history of spinal anesthesia, indicating intraspinal hemorrhages from an incidental lumbar ependymoma that was not previously diagnosed. Spinal anesthesia is widely utilized in numerous orthopedic surgical procedures, as it is a well-tolerated invasive procedure with a low risk of complications. The patient in this case study underwent elective orthopedic surgery under general ane

Inflammatory Biomarkers Following Orthopedic Surgery: Current Evidence, Clinical Applications, and Future Perspectives-A Narrative Review.

Orthopedic procedures trigger a complex inflammatory response that plays a central role in tissue repair and postoperative recovery. However, excessive or dysregulated inflammation may contribute to complications such as periprosthetic joint infection, thromboembolic events, delayed healing, or systemic organ dysfunction. Therefore, accurate perioperative monitoring of inflammatory activity has become increasingly important in orthopedic surgery. Evidence discussed in this review was identified through a literature search of PubMed, Scopus, and Web of Science databases covering publications fr…

Elevated risk of wound and systemic complications in MRSA-colonized patients undergoing lower extremity orthopedic trauma surgery.

PURPOSE: Preoperative colonization with methicillin-resistant Staphylococcus aureus (MRSA) is a known risk factor for adverse outcomes in arthroplasty but remains under-investigated in orthopedic trauma. This study evaluates the impact of preoperative MRSA colonization on postoperative complication rates in patients undergoing lower extremity orthopedic trauma surgery. METHODS: We conducted a retrospective cohort study using the TriNetX Research Network, identifying adult patients who underwent lower extremity orthopedic trauma procedures over a 20-year period. Patients with a documented MRSA

Vertebral body clinico-morphological features following percutaneous vertebroplasty versus the conservatory approach.

Most percutaneous vertebroplasty procedures are being performed in order to relieve pain in patients with severe osteoporosis and associated stable fractures of one or more vertebral bodies. In addition, vertebroplasty is also recommended for patients suffering from post-traumatic symptoms associated with vertebral fractures, patients with large angiomas positioned inside the vertebral body, with an increased risk for collapse fracture and also patients presenting with pain associated with vertebral body metastatic disease. On another aspect, it is possible that in isolated cases, an orthopedi

[Metatarsus primus double osteotomy, a logical and non-disabling surgical technique for treatment of hallux valgus].

The procedure known as the Metatarsus Primus Double Osteotomy ("L'ostéotomie métatarsienne bipolaire" of Schnepp-Carret) for the treatment of the hallux valgus is a logical, efficient and non-mutilating procedure. The technique consists in reducing the hallux valgus deformity by a subcapital bone wedge resection of the metatarsus primus. The bone wedge is then pinched into a second proximal osteotomy of the metatarsus primus, correcting the varus deformity in a valgus direction. This procedure is little known and is not even mentioned in the classic orthopedic literature of German and English

Wrist and elbow arthroscopy case volume is low and declining in orthopedic residency and fellowship training.

PURPOSE: Previous research has clearly shown that surgeon case volume is directly correlated with surgical outcomes in arthroscopic surgery. This study aimed to evaluate the exposure to wrist and elbow arthroscopy that graduating orthopedic surgery residents and fellows receive. METHODS: Accreditation Council for Graduate Medical Education (ACGME) case log data from 2014 to 2024 was reviewed for graduating orthopedic residents, hand surgery fellows, and orthopedic sports medicine fellows in the United States. Mean and median numbers of wrist and elbow arthroscopy cases logged per resident or

Basal cell carcinoma and squamous cell carcinoma of the hands in an orthopedic surgeon with occupational radiation exposure from C-arm fluoroscopy in Korea: a case report.

BACKGROUND: Occupational radiation exposure among medical professionals who use fluoroscopic devices is a well-recognized concern. While radiation-induced skin cancer has been reported in interventional radiologists, cases involving orthopedic surgeons with prolonged C-arm fluoroscopy exposure have rarely been described. We present a case of an orthopedic surgeon who developed basal cell carcinoma (BCC) and squamous cell carcinoma (SCC) on the hands after approximately 30 years of occupational radiation exposure from C-arm use during orthopedic surgeries in Korea. CASE PRESENTATION: A 62-year

Effect of Antibiotic Prophylaxis on Surgical Site Infections Following Removal of Orthopedic Implants Used for Treatment of Foot, Ankle, and Lower Leg Fractures: A Randomized Clinical Trial.

IMPORTANCE: Following clean (class I, not contaminated) surgical procedures, the rate of surgical site infection (SSI) should be less than approximately 2%. However, an infection rate of 12.2% has been reported following removal of orthopedic implants used for treatment of fractures below the knee. OBJECTIVE: To evaluate the effect of a single dose of preoperative antibiotic prophylaxis on the incidence of SSIs following removal of orthopedic implants used for treatment of fractures below the knee. DESIGN, SETTING, AND PARTICIPANTS: Multicenter, double-blind, randomized clinical trial includ

Urban Bicyclist Trauma: Characterizing the Injuries, Consequent Surgeries, and Essential Sub-Specialties Providing Care.

In the United States in 2013, nearly 500,000 bicyclists were injured and required emergency department care. The objectives of this study were to describe the types of injuries which urban bicyclists sustain, to analyze the number and type of surgeries required, and to better delineate the services providing care. This is an observational study of injured bicyclists presenting to a Level I trauma center between February 2012 and August 2014. Most data were collected within 24 hours of injury and included demographics, narrative description of the incident, results of initial imaging studies, I
