Paget's disease of bone: an update.

PURPOSE OF REVIEW: The review summarizes the recent findings relevant to the clinical management, genetic predisposition, and molecular mechanisms implicated in Paget's disease of bone (PDB). RECENT FINDINGS: PDB is characterized by focal regions of increased bone remodeling and abnormal bone architecture. PDB is treated effectively with amino-bisphosphonates, which can produce very prolonged disease remission. The disease has a strong genetic component and a large number of studies focus on the cellular mechanisms affected by mutations in the SQSTM1 (sequestosome 1) gene which are associated

A rare emergency condition in neurosurgery: foot drop due to Paget's disease.

Paget's disease is a chronic, focal skeletal disorder that usually affects the pelvis and spine. Spinal cases are generally asymptomatic; in the symptomatic cases, the neurological dysfunctions are related to non-compressive vascular defects, hemorrhage, sarcomatoid degeneration, spinal stenosis, or pathological fractures, primarily in the lumbar region. The Neurosurgeon should have a fundamental understanding of the complications of Paget's disease and should be familiar with the indications for treatment, as well as available medical and surgical therapies. In the present paper, we report a

Sarcomas arising in Paget disease of bone: a clinicopathologic analysis of 70 cases.

CONTEXT: Sarcomatous transformation is a rare complication of Paget disease of bone. Prognosis in patients with other types of sarcomas arising in bone has improved in the last several decades because of therapeutic advances. However, because of the rarity of Paget sarcoma, outcome studies in these patients are limited. OBJECTIVE: To determine whether prognosis for Paget sarcoma has improved. DESIGN: Seventy cases of sarcomas arising in the setting of Paget disease were collected, and the histologic and clinical findings were reviewed. Clinical follow-up was obtained in 67 cases. RESULTS: S

The effect of alendronate on renal tubular reabsorption of phosphate.

We report the effects of alendronate on phosphate homeostasis in two patients. In a woman with postmenopausal osteoporosis, the infusion of alendronate (7.5 mg intravenously daily for 4 consecutive days) was not associated with secondary hyperparathyroidism despite a reduction in serum calcium. This was associated with a rise in serum phosphate and TmP/GFR. This response contrasted with those observed in 14 other patients with osteoporosis, in whom PTH rose significantly following the infusion of alendronate in association with a significant fall in serum phosphate and TmP/GFR. The second pati

Etidronic acid. A review of its pharmacological properties and therapeutic efficacy in resorptive bone disease.

Etidronic acid is an orally and intravenously active bisphosphonate, which is believed to inhibit resorption of bone via a number of cellular mechanisms, including alteration of osteoclastic activity. In studies of patients with symptomatic Paget's disease, etidronic acid 5 to 20 mg/kg/day administered orally rapidly decreased the biochemical indices of bone turnover. Mineralisation defects in forming bone may be avoided by the use of an initial dosage of 5 mg/kg/day for up to 6 months; dosages above 10 mg/kg/day should be limited to 3 months' duration, and dosages greater than 20 mg/kg/day sh

Binding of serum osteocalcin to hydroxyapatite in Paget's disease of bone.

Serum osteocalcin (serum bone Gla protein, sBGP) is elevated in diseases characterized by an enhanced bone turnover. However, in Paget's disease of bone there is a discrepancy between the low increase of sBGP and the high increase of bone remodelling assessed by serum alkaline phosphatase and urinary hydroxyproline. It is possible that the relatively low levels of sBGP reflect an abnormal binding of the molecule to the bone due to an alteration of the BGP molecule and/or of the pagetic mineral phase of bone. Another possibility would be a disregulation of BGP synthesis. In the present work we

Isotope retention for assessment of bone turnover in involutional osteoporosis.

A continuous bone remodelling takes place throughout life at different turnover speed according to age, physiological and pathological conditions. The evaluation of bone turnover may be of value for a prognostic and therapeutical assessment. Calcium bone exchange may be considered a suitable marker of bone turnover; for this reason 47Ca or 45Ca kinetics may be used; these methods have been employed in the past. Labelled diphosphonates, and in particular 99Technetium-methylene-diphosphonate (99TcmDP) are simpler and safer, because these substances are strongly and almost completely stored in bo

Sarcoma complicating Paget's disease of bone. A clinicopathological study of 62 cases.

Out of 21 900 cases filed at the Latin-American Registry of Bone Pathology between April 1940 and July 1981, there were 987 with Paget's disease (4.51 per cent); 62 of these (6.28 per cent) were complicated by sarcoma and two were associated with giant-cell tumours of bone (osteoclastoma) without signs of malignancy. There was a slight predominance of men and the ages ranged from 45 to 87 years, with an average of 66 years. The most frequent sites were the femur (23 cases), the humerus (nine), the pelvis (10), and the tibia (nine). The low incidence of vertebral involvement (five cases) is not

Primary disorders of bone with 'spinal block'.

Two hundred and thirty-two cases of compression of the spinal cord or cauda equina included 11 cases where compression was entirely due to a primary bony disorder and 20 further cases where developmental spinal stenosis was the main cause of compression. The former group consisted of two cases of Paget's disease and one each of polyostotic fibrous dysplasia, chondrosarcoma, achondroplasia, spondylo-epiphyseal dysplasia, Klippel--Feil syndrome, haemangioma of bone, primary bone reticulosarcoma, osteoporotic fracture and vertebral osteomyelitis. Three patients, with fibrous dysplasia, chondrosar

[Unusual aspects of bone fractures in Paget's disease].

Because of their specific biological and vascular condition, bones affected by Paget's disease require special treatment in case of fracture. Observations on four patients are reported because of their originality; the two first involved a rare type of pelvic fracture, one ischiatic, the other iliac. One of the others was a deformation that required amputation. The last patient, a victim of a diaphyseal femoral fracture, had to undergo centromedullary pinning; but because there was massive destruction of the periostial and medullary circulations during the operation, extensive osteonecrosis du

Value of plasma chloride concentration and acid-base status in the differential diagnosis of hyperparathyroidism from other causes of hypercalcaemia.

A study is reported of the estimation of plasma chloride concentration and acid-base status in the differentiation of primary hyperparathyroidism from all other causes of hypercalcaemia. In the two groups of patients studied, all of whom had hypercalcaemia, there was complete separation between the two groups on the basis of plasma chloride concentration and acid-base status. In 16 patients with primary hyperparathyroidism the increase in plasma chloride concentration and associated metabolic acidosis could have been accounted for by the known renal tubular effects of parathyroid hormone. In 1

Cardiovascular risk is increased in patients with Paget's disease of bone: Insights from the CARTaGENE cohort.

INTRODUCTION: We aimed to assess whether Paget's disease of bone (PD) increases the risk of major adverse cardiovascular events (MACE) and arterial stiffness in middle-aged men and women. METHODS: We analyzed data from the CARTaGENE cohort, including individuals aged 40-69 years, recruited in the province of Quebec, Canada in 2009/2010, linked to healthcare administrative databases to identify PD and MACE (both before and after recruitment, 1997-2021). Comorbidities and arterial stiffness parameters (augmentation index, estimated pulse wave velocity) were measured at recruitment. Cardiovascul
